In briefExhaustive endurance training may suppress testicular function. This tentative conclusion was drawn from a study of six endurance athletes who underwent 2 weeks of prolonged running and cycling. No significant changes were noted for strength, weight, or body fat. However, persistent fatigue developed in all subjects. Their plasma testosterone decreased 17% (from 673 to 558 ng/dl, p<.05), and sperm count decreased, though not significantly. All reported a decline in sexual activity and sex drive, which may be related to the development of general fatigue and could be an early sign of overtraining stress.

In briefSuperficial peroneal nerve palsy developed in a football player after he was hit in the lower leg by an offensive lineman. The patient failed to regain feeling in his leg after conservative treatment with an exercise program emphasizing proprioception and coordination skills. The author discusses this unusual case as well as treatment that commonly results in a successful return to sports.

In briefFour athletes, aged 16 to 20, died shortly after they collapsed while playing basketball. Preparticipation screening had revealed no cardiac abnormality in three of them. The fourth, who had been advised not to participate in sports, had been treated for hyperlipidemia at age 7 and for exercise-related syncope at age 15. Physicians should make a concerted effort to identify those athletes with risk factors (eg, exertional syncope, chest pain, or dyspnea) through careful history taking and physical examination.

In briefAlthough magnetic resonance imaging of the knee is performed primarily to assess meniscal and soft-tissue pathology, traumatic osseous lesions also may be detected with this method. These lesions include occult intraosseous fracture (a new diagnostic entity), stress fracture, and osteochondral fracture. The authors discuss specific features of magnetic resonance images that can help the physician make an accurate diagnosis.

In briefSoft-tissue responses to injuries result in impaired neuromuscular function. Therapeutic heat and cold can minimize this injury response and maximize functional recovery. Before determining which type of treatment is most appropriate at a given stage, the team physician must understand not only the expected physiologic response to each modality but also the athlete's individual response to a given mode. For example, although cryotherapy has broad applicability to sports injuries, many athletes tolerate cold poorly. The author discusses both the art and the science of treating injuries with therapeutic heat and cold.
