In brief:The authors surveyed a random sample of 50% of Wisconsin high schools in each of the three classes to see how they were handling (1) organization of medical coverage, (2) first aid and initial care of sports injuries, and (3) follow-up care and reconditioning of sports injuries. The data showed that the medical coverage of the athletes was poorly organized and supervised. To help alleviate this problem, the University Hospital Sports Medicine and Fitness Center developed an outreach program in which 19 schools contract for (1) twice-weekly visits by an athletic trainer; (2) educational programs for coaches, athletes, and parents; and (3) trainer coverage at competitive events. This program provides the expertise and services of a certified trainer to schools that would otherwise go without.

In brief:The University of Wisconsin Hospital Sports Medicine and Fitness Center gives a mandatory battery of tests to everyone who wants to use the facility. It includes a sub-maximal stress test, muscular strength tests, flexibility tests, and an assessment of body composition. Prospective clients also complete a health-risk profile and activity questionnaire. An exercise prescription that takes into account the likes and dislikes of the individual is formed from the information received in these tests. The clinic staff uses positive feedback and frequent evaluations to keep the individual interested in making exercise a part of the daily routine.

In brief:Seventeen track team runners were evaluated after complaining of wheezing, coughing, rhinitis, or shortness of breath after practicing in a facility under construction. Investigation revealed levels of ethyl acetate and toluene low enough to meet federal guidelines but apparently sufficient to cause symptoms in the athletes. Therefore, current standards for regulating potentially toxic organic chemicals may be inadequate when aerobic exercise is performed in the polluted environment.

In brief:The marathon is one of the greatest tests of human endurance. This review article describes the physiological demands and responses of the respiratory, cardiovascular, and muscular systems to marathon running, focusing on the chain of oxygen transport needed to fulfill the aerobic requirements of marathon running. During the race, runners use about 75% of VO2max. Increased levels of ventilation (>80 liters·min−1) have been observed during the marathon. This can lead to decrements in forced vital capacity and respiratory muscle fatigue. Prolonged muscle activity during the run can cause muscle injury such as inflammation and necrosis of muscle fibers. Techniques for augmenting performance include endurance and interval training, diet manipulation, and racing strategy.

In brief:All the injuries and illnesses that resulted in days lost from football participation were tabulated for a 12-year period. Each occurrence (2,186) and the number of days lost (11,730) were ranked in 11 categories. Knee injuries (495) were the most numerous and resulted in the most lost days (4,426). The ankle category ranked second in both groups. Illness did not contribute significantly in either group. Games produced 33% of all injuries; more than 70% of head injuries occurred during games but only 14.3% of strains occurred during games. Time-loss knee injuries occurred in 18% of the players; 93 surgical procedures were performed, and there were 13 cases of repeat knee surgery.
