In brief: The contention that amino acids increase secretion of human growth hormone has served as the catalyst for their increased popularity. Although growth hormone has been linked to muscle hypertrophy and its secretion is known to increase with exercise, there is no proof that artificially induced increases combined with weight training contribute to gains in strength and hypertrophy. Further, growth-hormone-induced hypertrophy, as seen in acromegaly, does not follow the same process as work-induced muscle growth. It is possible that growth-hormone-induced muscle hypertrophy lacks corresponding strength properties because of increases in noncontractile protein. Additional research is needed to determine what effect, if any, combining amino acid supplementation with weight training has on muscle strength and hypertrophy.

In brief: While weight lifting, three athletes experienced neck or shoulder/scapular pain that radiated down the arm and that was associated with segmental weakness and paresthesia. Magnetic resonance imaging and/or cervical spine radiographs showed degenerative changes of the cervical spine, and each athlete was diagnosed with acute radiculopathy. Conservative management of such patients involves restricted activity, the use of analgesics and anti-inflammatories, and the application of a cervical collar. Once the symptoms have subsided, the athlete can gradually resume activities, provided that there are no structural lesions of the cervical spine that will predispose the athlete to further injury.

In brief: Eighteen college students were tested in five stages on both a treadmill and a revolving stair ergometer. The researchers found no significant differences in peak responses between the two modes—as measured by oxygen consumption, heart rate, ventilation, respiratory exchange ratio, and work time. However, during submaximal exercise in stages I and II, the values for heart rate and oxygen consumption were higher for stair-climbing subjects than for treadmill subjects. These findings support the use of revolving stair ergometers for testing and/or training healthy young adults.

In brief: The anterior cruciate ligament is the most often seriously injured knee ligament. Because effusion and muscle guarding often accompany ACL injury, immediate field examination is advised. Several traditional tests such as the anterior drawer test and the Lachman test can be used to check the integrity of the ACL. Each has strengths and weaknesses with regard to diagnostic value and ease of use, all of which should enter into decisions about their appropriateness in ACL injuries. In addition, the author has devised a test—the femur stabilization test—to provide insight into the ACL evaluation process.

In January 1989 THE PHYSICIAN AND SPORTSMEDICINE published a list of professional organizations and resource groups that represented the growing diversity in the field of sports medicine. The list now includes government agencies, sports-related groups, and academic centers that are active in tracking sports injuries.All entries have been updated. The list is divided into three segments: 40 sports medicine organizations, 21 groups and committees that generate sports medicine information, and 17 statistics-gathering groups. Some groups appear more than once; they have been cross-referenced in each instance. Entries in all three segments have been arranged alphabetically. When appropriate, we have given the name of an administrative head along with the chief elected officer.While THE PHYSICIAN AND SPORTSMEDICINE does not endorse any of the groups listed, we do encourage readers to contact the ones that interest them.
