摘要:
In brief: Gout is an acute inflammatory arthritis caused by precipitation of uric acid crystals in the synovial space. It is most common in men over age 40. Because attacks of gout may follow trauma and usually involve a single joint, they must be differentiated from acute injury and infection. Intense pain, marked erythema, and joint effusion are common to all of these inflammatory processes. The definitive diagnosis is made by demonstration of uric acid crystals in synovial fluid by joint aspiration. Treatment consists of nonsteroidal anti-inflammatory drugs, rest, and elevation. Underlying medical conditions such as lymphoma and chronic renal disease must be ruled out. Prophylactic therapy is aimed at reducing hyperuricemia.

摘要:
In brief: One in three teenage girls is on a diet—often a dangerous crash diet. When they enter adolescence, some girls stop playing childhood games and become more sedentary, which can lead to obesity. In addition, adolescent girls are self-conscious about their appearance and want to maintain a slender figure. Exercise is an effective alternative to fad diets. A weight-loss goal of 1 to 2 lb per week is recommended for the mid- to late-adolescent. Weight stabilization is recommended for early adolescents.

摘要:
In brief: This study investigated the effects of a ten-week cardiac rehabilitation program on selected coronary heart disease (CHD) and blood clotting risk factors in 14 cardiac patients. Comparisons of before and after program values showed a significant reduction in body weight, diastolic blood pressure, and triglyceride concentration. Mileage covered per exercise session increased significantly. Serum cholesterol, systolic blood pressure, and resting heart rate values showed moderate but nonsignificant positive changes. Results of this study suggest that ten weeks of cardiac rehabilitation can enhance certain CHD risk factors while reducing others. However, a cardiac patient may require years of rehabilitation with careful attention to diet and medication to effectively reduce risk factors.

摘要:
In brief: Holter monitors were used to make ECG recordings on 48 men for six-hour periods on successive days—one day with and one without spontaneous exercise. The subjects were divided into four groups based on frequency of PVCs. There were no significant differences between the four groups' ages or heart rates during the control, exercise, and recovery periods. In subjects with PVCs there was a significant increase in the incidence of PVCs per hour and PVCs per 1,000 heart beats during exercise compared with the control period but no significant difference between exercise periods and the first hour of recovery. More complex arrhythmias occurred during exercise than during the control or recovery periods.

摘要:
In brief: To clarify the risks involved in soccer, 4,018 children and adolescents on 253 teams were studied. One hundred seventy-six injuries occurred, a rate of 4.38%. The rate decreased as the teams got younger. Competitive teams had higher rates than recreational teams in each age category, and all but two of the injuries that required surgery occurred in the competitive leagues. The ankle was the most commonly injured joint, followed by the knee. These results indicate that soccer is a comparatively safe sport.

摘要:
In brief: A 20-year-old wrestler sustained an injury to the right orbital area during practice. He did not lose consciousness, but he had headache on exertion and ecchymosis. Postconcussion headache was diagnosed one week after injury. Four weeks later he was admitted to the hospital with a severe, bitemporal, throbbing headache. A computed tomography scan showed a large left subdural hematoma. Surgery relieved his symptoms, and he was told not to participate in contact sports. This case demonstrates the need for careful and continued observation of all athletes who sustain concussions.

摘要:
In brief: Professional soccer players may head the ball an average of five times a game, which adds up to 5,250 impacts to the head in a 15-year career. The impact to the head may be as high as 200 kp. Players complain of headache, pain in the neck, dizziness, irritability, insomnia, hearing disturbances, weakened memory, and abnormal alcohol reactions. Since these symptoms are subjective, additional studies are needed to understand the possible negative effects of heading the ball. Players should try to decrease the number of headings, especially on balls kicked from a short distance, and they should use proper heading techniques and light soccer balls.
