Drug abuse, suicide, murder, accidents, and sexual problems are epidemic among teenagers. Sport-specific examinations do not address these adolescent risk factors, but the author tells how they can be uncovered in a preparticipation exam.

The basketball player in this case report had a rare longitudinal tibial pilon stress fracture, for which few diagnostic or treatment guidelines existed.

In brief The effects of resistance exercise and anabolic steroids on lipoprotein profiles were examined in female weight lifters. Steroid users had significantly depressed levels of HDL-C, HDI-2-C (p<.05), and apolipoprotein (apo) A-I compared with nonusers and controls. Nonusers had significantly higher levels of HDL-C and HDL2-C than the controls. Users had significantly less favorable ratios of total cholesterol (TC) to HDL-C, HDL2-C to HDL3-C, and apo A-I to apo B than nonusers or controls. Women who participate in resistance training apparently have significantly better lipoprotein profiles than their sedentary counterparts, but these changes do not offset the deleterious effects of steroid use.

In brief: Contusions and minor lacerations, the most common renal injuries, require conservative treatment, and the athlete must refrain from strenuous activities for 2 to 3 weeks. Most patients with hematuria have a renal contusion at most and do not need contrast studies. Underlying renal abnormalities, pyelonephritis, and stone disease are occasionally seen. Patients who have gross hematuria after strenuous jogging or marathons should not run with an empty bladder. The smoke- or tea-colored urine that occurs after athletic endeavors usually resolves without sequelae within 24 hours and requires no further evaluation.
