In brief: To study the effects of diuretics on performance and neuromuscular function, the authors examined 46 male athletes before and after a 48-hour weight loss regimen. One group took a diuretic (furosemide, 1.7 mg-kg1), one group exercised, and one group served as a control. During maximal cycle ergometry the diuretic group had marked decreases in VO2, O2pulse, work load, ventilation, and tidal volume compared with the control group.Electromyographical examination showed striking differences in nerve reactivity after diuretic use. Therefore, taking diuretics to achieve a rapid weight loss may be hazardous. Athletes can lose a similar amount of weight through exercise, which has fewer side effects.

In brief: The authors conducted a retrospective survey of 43 male and 14 female competitive elite board sailors in Hawaii. They were young, healthy individuals, mostly between 20 and 29 years of age. The most commonly injured areas were the skin, feet, shins, back, and forearms. The most serious injuries were mainly associated with advanced maneuvers such as wave jumping and wave sailing. The increased use of custom short sailboards with foot straps for wave jumping and wave sailing may lead to the new clinical entities of“wave-jumper's and wave-sailor's knee and ankle derangements.”

In brief: Varicose veins and deep venous thrombosis represent two extremes in diseases of the venous circulatory system; the former occurs mainly in the superficial veins and the latter in the deep veins. In sports-related injury they may appear to be devastating and actually be benign, or they can look harmless and ultimately turn into a life-threatening condition. Three forms of venous disease are discussed in this article: varicose veins of the lower extremities, thrombophlebitis and its complications, and varicose veins of the spermatic cord (varicocele). Treatment methods include exercise, elastic stockings, elevation, warm compresses, and antiinflammatory and anticoagulant medications.

In brief: It is generally recommended that rectal rather than oral temperature be used to diagnose heat injury in runners. However, because of lack of privacy after a race, oral temperature is often used. In this study oral and rectal temperatures of seven women and 16 men were taken simultaneously three minutes, 15 minutes, and 30 minutes after a race in a tropical climate. The mean rectal temperature at three minutes was an elevated 103.5 F, while the mean oral temperature was a normal 98 F. This study shows the importance of using rectal rather than oral temperature for accurate assessment and diagnosis of heat injury.

In brief: Despite warnings of possible risk to liver, prostate, and kidney function, the use of anabolic steroids has become widespread among athletes. This study examined the influence of oral and injected anabolic steroids on serum HDL-C levels in nine strength-trained men. The mean HDL-C concentration in these subjects was 17.0±2.3 mg/100 ml, which was significantly lower (p≤.05) than the means for untrained (46±1.6 mg/100 ml) and strength-trained (44.6±1.3 mg/100 ml) men who were not using these drugs. In light of the relationships reported between low levels of HDL-C and the incidence of coronary artery disease, the administration of these drugs to otherwise healthy men appears to be ethically and clinically inadvisable.

In brief: High-density lipoprotein cholesterol (HDL-C) levels were measured in five elite weight-trained athletes who were intermittently taking anabolic-androgenic steroids and in 28 age- and sex-matched control subjects who had never taken anabolic-androgenic steroids Athletes had significantly lower levels of HDL C while taking steroids than while abstaining (p<.01). Athletes also had significantly lower HDL-C levels while using steroids than the control subjects (p.05).

In brief: The purpose of this study was to investigate the effects of using a sunscreen during exercise in both hot, dry and hot, humid environments. Sixteen men exercised for 39 minutes with sunscreen cream applied to their exposed body surface area. Results indicated that the mean skin temperature was significantly increased due to the use of sunscreen during the hot, dry exposure. This increase did not occur during the hot, humid exposure. Therefore, we concluded that using a sunscreen while exercising at low humidity retards sweat evaporation.

In brief: The purpose of this study was to determine the feasibility and physiological effects of mild steady-state exercise during hemodialysis treatment. Three men and three women pedaled a Monark bicycle ergometer at 150 kgm min1for five minutes during the first hour of hemodialysis treatment. Heart rate, blood pressure, pH, Hco3, and respiratory and blood gases were analyzed before hemodialysis treatment, at rest just before exercise, during exercise, and at selected times during the first hour of recovery. The authors concluded that all the physiological responses during hemodialysis treatment were typical of a deconditioned person responding to low-level exercise. The rise in blood pressure and arterial oxygen tension may have positive implications for hemodialysis treatment. Further investigation is warranted into the value of using exercise during hemodialysis treatment as a means of training.

In brief: Data suggest that an employee fitness program in a large corporation is not only an attractive fringe benefit but a profitable investment as well. Fitness programs have resulted in decreased absenteeism, reduced health care costs, and increased productivity. Developing and marketing a fitness program requires months of well-planned activity that includes forming a fitness committee, hiring a program coordinator, planning and equipping the proposed exercise facility, getting the support of top management, and generating publicity. Cost is a big consideration, but the savings seem to outweigh the expense of a successful corporate employee fitness program.
