In brief:The subspecialty of sports medicine is not reserved for the physician, who is just one member of the team. Sports scientists, psychologists, dentists, optometrists, athletic trainers, physical therapists, orthotists and prosthetists, podiatrists, nutritionists, and others play an integral part on the sports medicine team. However, the primary care physician can be the key to optimal performance of the sports medicine team—by being familiar with the talent and capability of specialists, communicating effectively, and knowing his or her own limitations.

In brief:It is accepted that circuit weight-training programs are effective for developing and maintaining muscular strength, but it is less clear that these programs are aerobic and will develop and maintain cardiorespiratory fitness. The authors evaluated the Nautilus Express Circuit (NEC) in terms of the American College of Sports Medicine recommendations for developing and maintaining cardiorespiratory fitness. They studied energy cost (oxygen uptake) and heart rate responses in ten men and eight women during the 20-minute weight-training program. Although heart rate responses were maintained within recommended ranges, VO2responses were not. The authors concluded that the NEC elicited only minimal aerobic exercise intensity.

In brief:Laryngeal trauma resulting in structural damage and/or laryngospasm with compromised airway is one of the true emergencies faced by team physicians and trainers. These care givers should be familiar with the anatomy and signs and symptoms of laryngeal injury and be prepared to deal with them. Most laryngeal injuries respond well to straightening the airway and firmly moving the chin forward with pressure behind angles of the jaw to relieve laryngospasm. Reassuring the patient is important. When airway obstruction is persistent, artificial airways may most easily be established by cricothyreotomy or needle jet cricothyreotomy. All laryngeal injuries should have immediate laryngoscopy to fully evaluate the degree of injury.

In brief:To compare infrared thermography, radionuclide bone scanning, and routine x-ray in diagnosis, a prospective blind study was conducted on 17 athletes (ten men, seven women) who had exertional leg pain compatible with stress fracture. The athletes with‘hot’patterns were unable to resume their original activity, while those with‘cold’patterns were able to resume activity. Additionally, specific soft-tissue syndromes were identified. The data indicate that thermography may be an accurate, noninvasive new way to distinguish stress fracture from other causes of shinsplint syndromes in runners. Applied repeatedly, this technique could be used to follow the course of bone healing, permitting the earliest safe return to activity.
