In brief: Effective treatment of a dislocated shoulder begins with accurate and complete diagnosis. Initial treatment includes application of ice and the use of a sling. There are several methods to reduce a dislocated shoulder. The author uses the Stimson method of reduction, in which the dislocated shoulder extends over the edge of the table and the hand hangs free above the floor with a weight attached to it for gentle reduction. Repeated I closed manipulations should be done only by an orthopedic surgeon after obtaining adequate x-rays and administering anesthesia. After the dislocation is reduced, the shoulder should be immobilized in adduction and internal rotation.

In brief: Computed tomography (CT) scans confirmed the diagnosis of hematoma of the rectus abdominis muscle in two injured football players. Surgery was not considered necessary because bleeding had subsided. Conservative treatment—rest and application of ice followed by application of heat—resolved the hematoma and soreness. Both players returned to full practice within three weeks; neither suffered a recurrence during the remainder of the season.

In brief:Football fatality data have been collected since 1931 and catastrophic injury data since 1977. There have been dramatic reductions in both areas, but for the past six years these numbers have remained steady. Some experts in the field of football injury research have stated that further reductions are not possible. The authors argue that efforts should be continued to eliminate these injuries and recommend mandatory medical examinations, rule changes, proper conditioning and equipment, having a physician on the field whenever possible, and continued research to help in this effort.

In brief: We studied the physiological and anthropometric characteristics of 20 players who were members of the US national rugby team. Forwards were compared with back-line players, and these athletes were compared with other rugby players and other elite intermittent-sport athletes. The forwards were taller and heavier than the back-line players, had greater lean body weight, a higher percent body fat, greater gross anaerobic power and capacity, but similar aerobic capacities. Rugby players overall were slightly older and leaner than other intermittent-sport athletes. Their aerobic fitness was similar to that of professional basketball, football, and baseball players but lower than that of professional soccer players and national representative ice hockey players.

In brief: Serum beta-endorphin/beta-lipotropin levels were measured in six elite kayak paddlers and 14 untrained subjects before and after an isometric knee-extension exercise regimen designed to induce profound muscle exertion and discomfort. We found no significant difference in resting beta-endorphin/beta-lipotropin levels between the two groups. Beta-endorphin/beta-lipotropin levels were not altered by the isometric exercise in either group despite a strength loss of approximately 34% and severe muscle exertion and discomfort. These data suggest that previously reported increases in beta-endorphin levels in response to other exercise regimens may not function primarily to modulate the exercise-induced muscle discomfort and pain.

In brief: Hypertrophic cardiomyopathy is the most common cause of sudden death in young conditioned athletes. The disorder is largely genetic, so families of patients need careful screening. Patients are often asymptomatic, but symptoms can include chest pain, near-syncope, palpitations, and dyspnea. A prominent S4gallop and a systolic murmur that is accentuated with standing and during Valsalva's maneuver suggest the diagnosis. Certain ECG, echocardiography, and cardiac catheterization features can help confirm the diagnosis. Athletes with hypertrophic cardiomyopathy should be restricted from vigorous exercise and treated with large doses of beta blockers. Antiarrhythmic agents are added, if necessary. Surgery is considered when large ventricular gradients are present and when symptoms do not respond to drug therapy.

In brief: This article presents two case reports of fractures of the cervical spine that occurred during a lacrosse game. Both injuries occurred to midfielders during a face-off. One player was charging into the face-off, and the other was charged during a face-off. Both had their heads down. The author suggests several ways that might prevent future cervical spine fractures in lacrosse, including a rule change that would not allow a player to be so vulnerable during a face-off or ground ball, ejecting a player for spearing or blindsiding, and alerting lacrosse coaches and sports medicine departments of the danger of spearing in lacrosse.
