In briefA 72-year-old man and a 47-year-old woman presented with subdural hematomas more than a month after repeated head injuries incurred while skiing. Neither had lost consciousness. The man had remained asymptomatic for 3 weeks, then had developed a bifrontal headache of increasing intensity and frequency. CT scan revealed a large left subdural hematoma. The woman had experienced intermittent symptoms, including headache, light-headedness, paresthesias, and lower-extremity weakness, for 4½months before MRI revealed bilateral hematomas. Acute hemorrhage into a chronic subdural hematoma may explain the delayed onset of symptoms in these patients.

In briefA 17-year-old male high school football player sustained two mild concussions in 7 days. Five weeks later, he ran 50 yd in a game and fell to his knees complaining of severe headache, nausea, facial pain, and difficulty breathing. An immediate on-field physical examination was normal, but the boy progressively became lethargic, stuporous, then comatose. CT scan revealed a right subdural hematoma, perhaps a long-term complication of the two mild concussions. Rules requiring neurocognitive testing after head injury and a physician's written clearance to return to play may be needed to prevent life-threatening injury or permanent neuropsychological sequelae in head-injured athletes.

In briefCalcaneal apophysitis, a condition commonly seen in active adolescents, results in pain in the posterior aspect of the os calcis, or heel. Radiographic findings sometimes noted at the calcaneal apophysis, including fragmentation and increased density of the apophysis, were once thought to be evidence of osteochondrosis. These findings are now recognized as a normal stage of apophysis development and appear to be unrelated to the symptoms. Teatment, which generally produces excellent results, includes restriction of physical activity, nonsteroidal anti-inflammatory medication, icing, a heel lift, and a stretching program.

In briefA recent case of posterolateral rotatory knee instability in a recreational athlete provides an opportunity to review diagnosis and treatment of this often unrecognized condition. Two simple clinical tests, the external rotational recurvatum test and the posterolateral drawer test, can identify this injury to the arcuate ligament complex. Treatment consists of bracing and physical therapy and, in some cases, surgery.

In briefTibial stress fractures often occur in Patients who participate in running and jumping sports. With rest, most stress fractures heal without incident, though midshaft fractures are more resistant to healing. Bone scans are usually required to confirm the diagnosis. Relative rest, strength training, proper footwear, and treating underlying risk factors are the mainstays of treatment. A new treatment option is a pneumatic brace that the patient can wear when he or she returns to play. Physicians should be prepared to counsel competitive athletes about the risks of returning to play too soon.
