In brief:Recent studies suggest that avoiding exercise is unwarranted and detrimental for asthmatics. Although exercise provokes bronchospasm in most asthmatics, the severity of exercise-induced asthma can be reduced by several factors: control of exercise duration; less intense, intermittent exercise; warm-ups; warmer, humid inspired air; aerobic fitness; and drugs. Regular vigorous activity increases fitness, enhances tolerance to attacks, and provides more social and psychological independence. The recent development of protective medications has made such activity possible for many asthmatics. Preexercise cromolyn sodium and beta adrenergic agonists are recommended for blocking or reversing attacks.

In brief:The author describes symptoms, diagnostic tests, and follow-up treatment and advice for 12 cases to illustrate cardiac rhythm problems in runners. After deciding if the disturbance is abnormal or merely a variant of normal that can occur in an athletic person, the physician should search for associated underlying cardiac disorders such as hypertrophic obstructive cardiomyopathy, mitral valve prolapse, or coronary disease that might predispose the person to an arrhythmia. Noninvasive methods, including a thorough cardiovascular physical examination, exercise testing, stress scans, ambulatory monitoring, cardiac fluoroscopy, and echocardiography, should be used whenever possible.

In brief:Torso injuries can occur in many sports, especially those involving acute deceleration or impact. They are divided into two main areas: thorax and abdominal injuries. Superficial injuries are obvious, but visceral injuries are harder to diagnose. The author discusses diagnosis and management of these injuries, and stresses a thorough examination—preferably in a quiet, unhurried environment—before resumption of competitive activity.

In brief:This report documents women's field hockey injuries at the California State University in Long Beach from 1976 to 1979. Of the 81 reported injuries, 56 were seen by a physician, but most were minor. Ankle sprains were the most common. Sixteen percent of the injuries were major. The most serious injuries were a glenohumeral subluxation of the shoulder, a cerebral concussion, a second-degree cheek contusion, and four knee injuries. The authors concluded that injuries decreased with improved team performance and that proper equipment, training, and medical coverage can limit injuries.

In brief:A hospital clinic primarily for recreational athletes was opened in New York City in 1975 and was staffed by orthopedic residents. During 4 1/2 years, 1,280 patients made a total of 2,732 visits to the clipic. Males outnumbered females by more than 2:1, and the average age was 30.2 years. The most frequently injured areas were the knee, ankle, and shoulder. Soft-tissue pathology was involved in 513 instances (53.9%); the rest (46.1%) were of a skeletal nature. Describing these problems may aid health practitioners in planning for the needs of recreational athletes.

In brief:The authors examined the effects of variations in lead selection and time of monitoring during recovery on the predictive value and sensitivity of exercise testing. Eighty-six patients received a standard Bruce protocol exercise test and follow-up angiography. The results showed that 12-lead monitoring for at least six minutes of recovery had more sensitivity for diagnosing coronary artery disease, and the greater the ST-segment depression, the greater the predictive value of the test.

In brief:Three hundred runners were surveyed about the effectiveness of orthotics in controlling the symptoms and recurrence of overuse and impact shock injuries. The most common overuse injuries in the 146 respondents were knee pain, shinsplints, plantar fasciitis and arch strain, and Achilles tendinitis. The most commonly encountered problems with the orthotics themselves were blister formation, edge irritation, heel slippage from the extra bulk in the shoe, and discomfort from the stiffness of the orthotic. The author feels that orthotics can be of significant therapeutic value in treating abnormal function and preventing its recurrence.
