In brief: Some experts believe that exercise testing in women elicits an unacceptably high false-positive rate. The authors studied the results of exercise testing in 192 apparently healthy women (mean age, 38.2 years) to assess the false-positive rate in ST-segment response. Based on the tests and a five-year follow-up, only seven women (3.7%) showed false-positive results. The authors suggest that varying methods of testing and interpretation may account for the high rate of false positivity in some testing centers.

In brief: Over a two-year period, we reviewed all acute boxing injuries among professional boxers statewide (484 the first year, 422 the second year). During the study period, the boxers fought 3,110 rounds and incurred 376 injuries (262 craniocerebral injuries, 114 other injuries), ie, they incurred 1.2 injuries per 10 rounds fought (0.8 craniocerebral, 0.4 others). Only four boxers required immediate neurological evaluation at a hospital after a fight; one of the four died as a result of bilateral subdural hematomas. Facial lacerations were the most common other type of injury (66 cases), followed by hand and eye injuries (8 cases each). The authors suggest that severe, acute neurological injuries are rare in professional boxing when strict medical supervision is present. However, they caution that their findings should not be used to draw inferences about the development of chronic neurological injuries among professional boxers.

In brief: Nerve injuries in athletes may be serious and may delay or prevent an athlete's return to his or her sport. Over a two-year period, the authors evaluated the condition of 65 patients who had entrapments of a nerve or nerve root, documented with electromyography. They describe four case histories: Two patients had radial nerve entrapments, one caused by baseball pitching and the other by kayaking; one football player had combined suprascapular neuropathy and upper trunk brachial plexopathy; and one patient had carpal tunnel syndrome of a median nerve secondary to rowing. Sports-related peripheral nerve lesions of the lower extremity were not seen during the study period. Based on a literature review, the nerve injuries discussed represent the spectrum of nerve entrapments likely to be seen in US clinics. The authors conclude that peripheral nerve lesions should be considered in the differential diagnosis of sports injuries, particularly at the shoulder, elbow, and wrist.

In brief: Primary care physicians provide most of the sports medicine care rendered in the United States through their office practices and through their roles as team physicians for local schools. With appropriate training and experience, their breadth of medical expertise probably makes them the physicians most qualified to function as“sports physicians”(although few do so on a full-time basis). One such physician describes his part-time sports medicine experience, including the multiple roles he plays as team physician, the way sports medicine is integrated into his family practice, and how it affects his professional life and peer relationships.

In brief: Cross-sectional studies of physical performance generally show a linear decline in maximum aerobic power (V O2max) with increasing age (about 1% to 2% per year). In the study described in this article, 15 men were serially followed for 20 years. They performed exercises consisting of walking, running, swimming, and cycling for an average of 3.6 days per week and requiring an energy expenditure of 2,104 kcal per week. Over the 20 years, directly measured V O2max declined 12% (from 44.4 to 38.9 ml·kg−1·min−1, or 0.27 ml·kg−1·min−1per year). Essentially no differences were seen in resting heart rate or arterial blood pressure. The authors conclude that physical training forestalls the decline in V O2max until at least age 65.

In brief: Many cyclists refuse to wear helmets because they produce discomfort and drag. To determine the effects of wearing a helmet on thermal balance and rating of perceived exertion while cycling in the heat, six male competitive cyclists aged 19 to 32 rode a stationary bicycle attached to a road-racing simulator in an environmentally controlled chamber for two hours at 70% V O2max. Measurements were taken of rectal and skin temperatures, V O2, heart rate, sweat rate, and rating of perceived exertion. The results showed that (under the experimental conditions used) wearing a helmet while cycling in the heat does not alter thermal balance or cardiovascular strain compared with not wearing a helmet.
