In briefPeople who sprain their ankles brief may sustain osteochondral fractures of the talar dome. If unrecognized, these fractures can cause chronic pain and disability. The history and physical examination may be nonspecific, but a physician who has a high index of suspicion can identify these lesions with targeted imaging procedures. Identifying the stage of the lesion will suggest a conservative or surgical treatment approach. Prognosis for either option is generally good; however, osteoarthrosis may develop.

In briefActive patients who have recurrent ankle sprains usually have a treatable underlying neuromuscular, structural, or rehabilitation-related condition. A thorough history, neurologic exam, and physical exam involving a visual survey, foot and ankle palpation, and manual stress tests will help reveal the specific diagnosis. Most patients need only conservative treatment, which usually involves strengthening and bracing. Resistant injuries may require surgery.

In briefWhat are optimal levels of protein for active people? And how does caffeine affect athletic performance? Substantial evidence indicates that endurance athletes' protein requirements are 50% to 100% higher than the average person's. Recent evidence suggests that resistance athletes have protein requirements similar to endurance athletes'; however, neither resistance nor endurance athletes benefit from excessive protein supplementation. Much evidence shows that caffeine is ergogenic in prolonged endurance exercise, but there is no evidence that it improves performance in resistance or power sports.

In briefAthletes who have heart symptoms need the same type of evaluation as nonathletes who have such symptoms, except that the evaluation generally should include testing during exercise. In history taking, be aware that an athlete may try to minimize heart symptoms in an effort to avoid restrictions on participation. Syncope and related symptoms are Important in that they may reflect an arrhythmia caused by a potentially fatal condition. Syncope or near syncope also may be caused by cat- echolamine-dependent tachycardia, which usually responds to treatment withβ-blockers.

The patient experienced worsening pain despite conservative therapies such as stretching and anti-inflammatory drugs. Radiographs indicated an osteoid osteoma, and excision completely eliminated the pain.

Many of your patients may share the anxiety publicly expressed by a few professional basketball players about playing alongside Magic Johnson.
