In the Salivary Gland Register (University of Hamburg), 9883 cases were recorded from 1965 to 1984. Among these cases there were 3,326 neoplasms, of which 3,017 were epithelial in origin. In this latter group, 80 neoplasms arose in children and adolescents. Among these 80 persons, there was a slight predominance of females; 57 tumors (71 %) were localized in the parotid gland, and only 6 (8 %) in the submandibular gland. The incidence of the tumor types was different in children and adolescents as compared to the incidence in adults: monomorphic adenomas were virtually absent in children (only 1 patient), and benign tumors were also less common (65%). The incidence of mucoepidermoid tumors was remarkable (12 cases, 15 %). Acinic cell tumors occurred more frequently (5 cases, 6 %). The neoplasms have been analyzed histologically. Two very rare tumors, a monomorphic salivary duct adenoma and an embryonal carcinoma, were also studied by electron microscopy and immunohistochemistry.

An unusual cause of life-threatening stridor in infancy due to a congenital lateral saccular cyst of the larynx in association with an upper respiratory infection is presented. The literature is reviewed. The histopathogenesis, classification, clinical manifestations, and therapeutical priciples of congenital laryngeal cysts are discussed. As almost 50 % of the reported cases of congenital laryngeal cysts were diagnosed at autopsy, the gravity and importance of early recognition of these lesions must be stressed to the pediatrician and otolaryngologist.

The first case of malignant schwannoma of the submandibular gland in a 23-year-old man with neurofibromatosis is reported. Light microscopy showed typical features of neurogenic sarcoma and an immunocytochemical study confirmed the schwannian differentiation of the neoplasm. The patient is still well without evidence of metastases or local recurrence of the tumor 80 months after the excision of the neoplasm.

The article describes a family in which a father and both sons were treated for carcinoma of the larynx. Reports of 21 similar families were collected from the literature. Certain factors occurring in familial cases are discussed (alteration of the sex ratio, early onset of the disease, same histological type and same localization of the cancer in relatives), which could indicate a genetic susceptibility in these families.

The ultrastructural morphology of Kaposi’s sarcoma of the oral mucosa in patients with acquired immune deficiency syndrome (AIDS) was examined electron-microscopically. Neoplastic spindle cells and vascular endothelial tumor cells are frequently involved in the phagocytosis of extra vasated erythrocytes. Pericytes, fibroblasts, macrophages and other phagocytic cells of the tumor stroma also participate in this process. Erythrocytes are also phagocytized by some of the keratinocytes belonging to the intact surface epithelium covering the Kaposi’s sarcoma lesion. Both the phenomenon of erythrophagocytosis and the presence of tubuloreticular structures in the cytoplasm of Kaposi’s sarcoma cells in patients with AIDS seem to be prominent ultrastructural markers and thus of diagnostic

A case of chondrosarcoma originating within the postethmoidal and sphenoidal sinuses with intracranial invasion will be reported. The patient, a 54-year-old female, presented severe headaches and diminished visual acuity of the right eye. Angiographic findings showed both the left carotid artery to be completely occluded and the right inner carotid artery to be narrowed at the site of the carotid bifurcation. Computerized tomography scan findings showed a tumor, located mainly in the frontal base and temporal fossa, that had invaded the cranium through the skull base. True gigantism with overproduction of growth hormone (GH) and prolactin (PRL) was diagnosed in a 13-year-old boy. The clinical history indicated that the tumour had caused an oversecretion of GH since the age of 4–5 years. At diagnosis, the sella turcica was markedly enlarged. No infiltrative growth was noted at surgery. Endocrine investigations showed elevated GH and PRL secretion. Light and electron microscopy of tumour tissue revealed densely packed pleomorphic cells of both GH and PRL type. In addition, oncocyte like cells were observed. Organ culture of pieces of tumour tissue demonstrated continued secretion of GH and PRL into the medium for more than 5 days in vitro. Addition of bromocriptine to the medium caused a rapid decline in PRL secretion while GH secretion remained the same. X-ray irradiation in vitro also caused a decrease in PRL secretion. These effects of bromocriptine and X-ray on hormone secretion in vitro mirrored the corresponding effect of treatment, when the patient showed signs of tumour recurrence after pituitary surgery. It is concluded that also in childhood, the in vitro response of tumour tissue to various treatments may be explored as a possible way to predict the efficacy of pharmacological or irradiation treatment of pituitary tumour
