A case of metastatic fibrosarcoma of the temporal bone was studied histopathologically. The primary site of this tumor was the posterior neck region. The initial major symptom of the patient was a left facial nerve paralysis. Although the patient underwent a suboccipital posterior craniectomy, radio-, and chemotherapy, she eventually died from multiple metastases. Histological examination of the temporal bones showed an extensive tumor invasion in the external auditory canal, middle ear cavity, middle ear muscle, tympanic membrane, mastoid cavity, petrous apex, facial nerve, and superior division (distal segment) of the vestibular nerve (left ear). The route from the stylomastoid foramen to the superior division of the vestibular nerve via the facial nerve canal could be one of the possible passages of tumor invasion.

A 60-year-old man affected by nonhealing midline granuloma (NHmG) is described. The histopathological features, the clinical course and the radiosensitivity of the lesion clearly differentiate this pathological condition from malignant lymphomas and from Wegener’s granulomatosis (WG). The serum retinol levels of 19 patients with T1 or T2 tumours of the larynx were compared with those of non-malignant smoking and non-malignant non-smoking controls matched for age, sex and weight. The cancer group had a significantly lower level of serum retinol than either control group between which there was no significant difference. It is unlikely that the lower serum retinol levels were due to either tumour or cigarette smoke.

Cyclic nucleotides (cAMP, cGMP) are suggested to participate in the regulation of cell growth and differentiation. Guanylate cyclase is the enzyme which catalyzes the synthesis ofcGMP. The basal guanylate cyclase activity was slightly higher in well-differentiated squamous cell carcinomas than in normal mucosa, but was two-fold higher in papillomas. Poorly differentiated squamous cell carcinoma did not show any increased basal activity. Stimulation with nitroprusside (NP) resulted in a 20% increased activity for normal mucosa and a 30% increase for poorly differentiated carcinomas, whereas enzyme prepared from well-differentiated squamous carcinomas and papillomas showed a two-fold increase.

Chondrosarcoma of the larynx is a rare neoplasm. The characteristic features of this tumor are described, and a case in a 69-year-old woman is reported. This case demonstrates the difficulties of histological classification. On the first biopsy specimen a benign chondroma was diagnosed. However, the microscopical features of the excised tumor were those of a chondrosarcoma. It is generally accepted that radiotherapy is of no value; surgical treatment by laryngectomy is advised. Some authors recommend a conservative attitude towards these tumors, which are of low malignancy, the growth slow and the risk of local or distant metastases slight.

Spindle cell squamous carcinoma is a highly malignant neoplasm, characterized by a biphasic appearance, due to the presence of a diffuse proliferative pattern of pleomorphic spindle cells with extremely rare foci of conventional squamous cell carcinoma. Epithelial markers in eight cases of spindle-cell squamous carcinoma of the head and neck region were studied by immunocytochemistry. Epithelial membrane antigen and cytokeratin resulted strongly positive in the spindle cells of the sarcoma-like areas demonstrating the epithelial nature of the neoplasm. In 6 of our patients who later died from widespread metastases, the neoplasm had poor prognosis with a mean survival rate of 14.7 months.

The effect of powdered ginger root (Zingiber officinale) upon vertigo and nystagmus following caloric stimulation of the vestibular system was studied in 8 healthy volunteers in a double-blind crossover placebo trial. The results reported are based upon 48 vertigo scores and 48 electronystagmograms. Ginger root reduced the induced vertigo significantly better than did placebo. There was no statistically significant action upon the duration or the maximum slow phase velocity of nystagmus.

In 33 Ménière patients (42 ears) the therapeutic effects of Dyazide® (50 mg triamterene and 25 mg hydrochlorothiazide) were investigated in a cross-over placebo-controlled study. The hearing was measured by tone and speech audiometry whereas tinnitus and vertigo were recorded using fixed scales. In this study, Dyazide was found to have no significant effect on hearing or tinnitus. However, the vestibular complaints decreased significantly during the Dyazide treatment. Ears with a positive glycerol or furosemide test and ears with a fluctuating hearing loss did not show significant differences in hearing thresholds between the two periods in the trial. Two cases of rupture of the round window membrane verified by operation are presented. Both cases arose in connection with myringotomy and were presumably caused by a violent force acting directly on the tympanic membrane and the ossicles. This would result in a strong movement of the stapes and an acute change in the perilymph pressure, thereby causing an indirect injury to the round window. Symptoms and treatment are briefly discussed.

The occurrence of epistaxis in 121 habitual nose-bleeders has been analysed together with the importance of different etiological factors. Heredity for recurrent nosebleeds was noted by 42% of the bleeders, which was significantly more than in the control group (p < 0.001). The start of the nose-bleeds was mostly spontaneous. Both common cold and stress or tiredness were frequently experienced before the occurrence of the nosebleeds. The blood pressure distribution of the habitual nose-bleeders did not differ from that of the population samples used for comparison. Diseases reported by the habitual bleeders were few, and the blood routine tests revealed only few diseases that could be related to the recurrent nose-bleeds.
