Light and electron microscopical observations of adenoids are described. A marked feature was the presence of extensive folding of the reticulum cells in sub-epithelial areas and germinal centres. This was taken to indicate processing of antigen, possibly virus, derived by passage through the epithelial surface. Antibody formation is probably carried out in a sub-epithelial position by plasma cells which are mainly located in that situation and which are in close contact with the reticulum cells with infolded membranes on the one hand and epithelial cells on the other. The latter relationship is associated with the production of ‘secretory IgA’ in this area. Changes in the upper respiratory tract after naso-tracheal intubation were studied in a group of 95 patients who had been intubated for a prolonged period of time because of respiratory insuffiency. Length of intubation varied from 1 to 31 days. Patients were followed by frequent inspection of the upper respiratory tract until all objective changes had disappeared. Mucosal lesions of the oro- and hypopharynx apparently played only a minor role. In the nasal cavity mucosal necrosis of varying extent and at times even granulation was found, but all changes healed spontaneously. The laryngeal changes also disappeared spontaneously. These changes consisted primarily of deposits on the vocal cords and the posterior subglottis together with erosion of the arytenoid cartilage. In 10% of the cases laryngeal granulation polyps were seen which disappeared spontaneously. After extubation stenosis in the subglottis or trachea was seen in 3 cases and was apparently in part caused by intubation. On the basis of this study it is concluded that changes and late complications after prolonged naso-tracheal intubation are so minor that intubation can be continued for several days without serious risk.

Twelve patients with chronic respiratory paralysis after poliomyelitis were given an otolaryngological examination, including tracheoscopy and bacterial and fungal culture of the tracheal mucus. Pathogenic organisms were cultured from the slime in 10 patients, gram-negative rods in 9 cases. Three patients had a positive fungal culture of the tracheal mucus. Tracheoscopy revealed a diffuse tracheitis in all but 1 patient. Four patients had granulation in the tracheal wall close to the lower end of the cannula, and in 2 patients a bleeding tendency was observed at the same site.

A report of 123 middle ear operations using a cyanoacrylate adhesive is presented. The results of healing and restoration of the hearing function are equal or better than those achieved by standard methods of tympanoplasty. Good results can be obtained only when the quantity used is kept as small as possible. Check-ups 1 and 2 years after the operation show, that in the majority of cases successful results were lasting. A tendency to further improvement of hearing was noticeable. The use of tissue adhesive in tympanoplasty facilitates particularly the reconstruction of the chain of hearing ossicles. Findings after implantation by revision or planned second operations prove, that the acrylate polymerisate does not impede the healing process of the affected mucosa in the middle ear if sufficiently small quantities are applied.

Squirrel monkeys were injected intraperitoneally with L-Thyroxine-3’,5’-Iodine-125 solution in 50-µCi/kg doses. Middle and inner ear tissues and end organs, VIIIth and VIIth nerves, auditory cortex, insular-temporal cortex, brain stem, whole blood, serum, and cerebrospinal fluid were studied with a ‘well-type’ scintillation counter to determine concentration of the L-thyroxine-I125. L-Thyroxine-I125 concentration was higher in the VIIIth and VIIth cranial nerves and vestibular and cochlear end organs, than in the tympanic membrane, middle ear ossicles and otic capsule. The concentration in the VIIIth and VIIth cranial nerves was also higher than that in the central nervous system tissues: primary auditory cortex, insular-temporal cortex, and brain stem which included cochlear and vestibular nuclei. The term acoustic accident was literally translated into German through a false interpretation as ‘akustischer Unfall’. But in fact we understand by this term a sudden disturbance of the inner ear which occurs during a short exposure to noise where the noise lies below the acoustic trauma. We describe a case where the patient – after being exposed to a noise of 106 dB only during some seconds while turning his head – became deaf primarily on one side. Thereafter he showed a decrease in the middle frequency range. We establish the hypothesis that a vascular disturbance of the inner ear should be made responsible for the hearing loss and not the exposure to noise
