Valproic acid (VPA) was released for general use in the United States in 1978. Since that time the use of VPA has steadily grown, and considerable literature on valproate from the United States has resulted. Despite this fact, however, very few studies that were either well controlled or compared the efficacy of VPA with that of another standard anticonvulsant drug have been published from this country.

Any review on adverse effects alone without due reference to the therapeutic benefits of a valuable drug is in danger of exaggerating the side effects. Nevertheless, it is reasonable to expect that some of the signs and symptoms seen in patients with epilepsy may not be due to the epilepsy but rather to the adverse effects of antiepileptic drugs (Schmidt and Seldon, 1982). Awareness of the common and the less well‐known adverse effects will increase the safety of a drug, such as valproate, that is taken chronically by many patients worldwide. From a clinical perspective, any unwanted reaction may be seen as an adverse effect. Adverse effects can usually be classified in a small number of rather rare but sometimes fatal and mostly unpredictable drug‐induced diseases or in more frequent, mostly mild, dose‐related, and reversible side effects (Schmidt, 1983). The adverse effects of valproate are difficult to classify in that scheme, because dose dependency is rare and the mechanisms of most adverse reactions are not well understood. It may therefore be useful to start with an overview of the common adverse effects observed in therapeutic trials, followed by a review of the individual organ systems involved in the adverse effects of valproate, including rather rare drug‐induced diseases. The purpose of this review is to provide an overview of the adverse effects of valproate, except for its hepatotoxic and teratogenic actions, which will be covered by Prof. Jeavons (

Summary:Effects of long‐term antiepileptic therapy on the hypothalamic ‐pituitary axis were evaluated from the basal and stimulated plasma levels of growth hormone (GH) and prolactin (PRL) and from circadian adrenocorticotropic hormone (ACTH)/cortisol rhythms. Data for patients with well‐controlled epilepsy of mild‐to‐moderate severity were compared with those for normal healthy volunteers. Analysis of the effects of each antiepileptic drug (AED) and of combined AEDs revealed minor abnormalities of stimulated GH secretion in all treated patients. In epileptic men, all individual AEDs (except valproate) and AED polytherapy increased both basal and stimulated plasma levels of PRL. In epileptic women, this effect was more variable and less marked, probably because of early depletion of PRL reserves. Each AED and combined AEDs did not significantly change circadian ACTH/cortisol rhythms in epileptic patients. The effects observed seem not to be related to epilepsyper se.Clinical implications, pathways, and neurotransmitters involved and possible mechanisms of the neuroendocrine effects of long‐term AED therapy are discussed.RESUMENLos efectos de la terapia antiepiléptica de larga duración, sobre el eje hipotalámico‐hipofisario, han sido evaluados mediante los niveles, basales y estimulados, en plasma de la hormona de crecimiento (GH) y de la prolactina (PRL) y mediante los ritmos circadianos de ACTH/cortisol. La información de pacientes con epilepsyía de severidad leve o moderada, bien controlada, fué comparada con la de voluntarios normales o sanos. El análisis de los efectos de droga antiepiléptica única (AED) y de AED combinadas reveló anormalidades menores de la secretión estimulada de la GH en todos los enfermos tratados. En los epilépticos varones todos los AED individuales (excepto el valproate) y los AED de la politerapia incrementaron los niveles basales y estimulados de PRL. En mujeres epi16pticas este efecto fue mas variable y menos pronunciado, probablemente debido a una depletion precoz de las reservas de PRL. Cada AED individual y AED combinada no no modificaron significativamente los ritmos circadianos ACTH/cortisol en los enfermos epilépticos. Los efectos observados no parecen estar relacionados con la epilepsyía “per se”. Die Ergebnisse von Patienten mit gut kontrollierter Epilepsie geringen oder mäßigen Schweregrads wurden mit denen normaler gesunder Freiwilliger verglichen. Die Analyse der Wirkungen jedes antiepileptischen Medika‐ments (AED) und kombinierter AED zeigte bei alien behandelten Patienten geringere Abweichungen der GH Sekretion nach Stimulation. Beim männlichen Epileptiker erhöhten alle individuellen AED (mit Ausnahme von Valproat) und AED Poly‐Therapie sowohl die Basalwerte als die stimulierten Plasmaspiegel von PRL. Bei epileptischen Frauen war diese Wirkung weniger ausges‐prochen und variabler, wahrscheinlich deswegen, weil die PRL Reserven früher erschopft waren. Keine antiepileptische Monooder Kombinationstherapie anderte die circadianen ACTH‐Cortisol‐Rhythmen bei epileptischen Patienten in signifikanter Weise. SUMMARYThe clinical and histological findings in 88 patients in whom hepatotoxicity with valproate has been reported were reviewed.The characteristic lesion is microvesicular steatosis. Hepatotoxicity appears to be an idiosyncratic reaction and is most likely to appear within 6 months from the start of therapy, especially in young patients on comedication. The condition may be reversible if the drug is immediately withdrawn in patients who show acute gastrointestinal symptoms, drowsiness or lethargy, jaundice, or change in seizure pattern. Clinical monitoring is more important than laboratory monitoring.The outcome of pregnancies in 344 women who received valproate in the first trimester was reviewed. There were 225 normal babies and 68 abnormal babies. The role of valproate in the aetiology of neural tube defects remains uncertain, but mothers treated with valproate should receive prenatal counselling.

Summary:Male rats were exposed to lead from parturition through weaning. When subjected to electrical hippocampal kindling as adults, although there were no alterations in the kindling rates, lead‐treated animals were found to differ from controls in several other respects. In contrast to controls, lead‐treated animals did not show an increase in afterdischarge (AD) duration with kindling, and the power spectrum of the AD did not shift to a higher peak frequency. There was a clear dose‐related effect of lead on the spectra of the kindled AD, which in lead‐treated groups was characterized by greater power in the low‐frequency bands. These data indicate that long‐lasting dysfunction can occur following a brief neonatal lead exposure. Power spectral techniques may be useful for future studies in neurotoxicology and epilepsy.RESUMELes rats masculins étaient exposés au plomb depuis la parturition et jusqu'à la periode de sevrer et durante toute. Los animales con plomo no mostraron un incremento en la duratión de la AD con el “kindling” y el espectro de potenciales de la AD no se desplazó hacia más altas frecuencias. Se observó un claro efecto relacionado con la dosis del plomo sobre el espectro de la AD derivada del “kindling”, la cual, en los grupos tratados con plomo se caracterizó por un potencial más elevado y bandas de frecuencia reducidas. Im Gegensatz zu Kontrollen zeigten bleiexponierte Tiere keine Zunahme der AD‐Dauer beim Kindling und das Leistungsspektrum der AD war nicht zu einer höheren peak‐Frequenz verschoben. Es bestand eine eindeutige dosis‐bezogene Wirkung des Bleis auf die Spektren der AD nach Kindling, die bei den bleiexponierten Gruppen durch höhere Leistung in den niedrigen Frequenzbändern charakterisiert waren. In the treatment of absence seizures, doctors have to choose mainly between two drugs: ethosuximide (ES) and sodium valproate (VPA). The situation is somewhat unusual in the field of epilepsy. There are two drugs of about equal efficiency (Simon and Penry, 1975) for about the same seizure types, but they are absolutely unrelated chemically; as similar as they are in their desired action, as much they differ in their possible side effects. With VPA, loss of hair, weight gain, tremor, thrombocytopenia, and rare but fatal instances of liver failure have been reported, and, more recently, there is suspicion of a teratogenic effect (Robert and Guibaud, 1982). With ES, no fatalities seem to have been reported, and singultus is one of the few common adverse effects. In quite a few patients, however, psychotic episodes have been observed, which were reviewed by Roger et al. (1968). As these occur at extremely different drug levels (Wolf, 1977) but are usually accompanied by complete or quasicomplete seizure control, they have been considered examples of “forced normalization” (Wolf and Trimble, in press)—an intriguing condition, the pathogenesis of which is still unexplained. This has not been observed with absence control by VPA (Wolf, 1976) even in patients that have had forced normalization with ES. Quantitative investigations of this difference have, however, not been reported, although they seem of considerable interest, as any significant difference between the drugs in this respect would be of high theoretical and practical impor

SUMMARYThe literature concerning the effects of sodium valproate on cognitive function was reviewed, including both volunteer and patient studies. Two were reported from the National Hospital—one a double‐blind crossover investigation against placebo in volunteers, and the other in patients on monotherapy. Summary:Feline generalized penicillin‐induced epilepsy is a reliable experimental model of epileptic photosensitivity. Binocular photic stimulation at 4–8 flashes/s consistently triggers generalized, bilaterally synchronous spike‐and‐wave discharges in cats submitted to long‐term intramuscular penicillin administration at low dosage (50,000–150,000 IU/kg). The photically induced epileptic activity is first and mainly recorded from the cerebral cortex as compared with the lateral geniculate (GL) body, therefore supporting a cortical onset of epileptic photosensitivity. At this low penicillin dosage, only minimal spontaneous spike‐and‐wave activity is recorded. Bilateral GL stimulation is more effective than binocular photic stimulation in triggering spike‐and‐wave discharges, whereas unilateral GL stimulation is grossly ineffective. This observation implies a role of bilateral specific visual thalamocortical volleys in the genesis of epileptic photosensitivity. Five of 12 randomly selected cats receiving long‐term penicillin exhibited spontaneous epileptic photosensitivity, a finding that suggests an underlying genetic predisposition of the feline species to this condition.RESUMENLa epilepsyía penicilínica de los felinos (FGPE) es un modelo experimental fiable de fotosensibilidad epiléptica. La estimulación fótica binocular de 4–8 destellos/seg. consistentemente dis‐para descargas generalizadas, bilaterales y síncronas de puntaonda en gatos crónicos sometidos a administración intramuscular de penicilina a dosis bajas (50.000–150.000 IU/K). La actividad epiléptica inducida fóticamente se registra primero y fundamentalmente en la corteza cerebral en comparación con el cuerpo geniculado lateral, lo que apoya el comienzo cortical de la fotosensibilidad epiléptica. Con estas bajas dosis de penicilina sólo se registra una mínima actividad espontánea de punta‐onda. Die optisch induzierte epileptische Aktivität wird früher und vorwiegend vom cerebralen Kortex abgeleitet, verglichen mit dem corpus geniculatum lateraie: dieser Befund deutet auf den kortikalen Ursprung der epileptischen Photosensibilität. Bei diesen niedrigen Penicillin‐Dosen wird nur minimale spontane spike wave Aktivität abgeleitet. Die bilaterale GL Stimulation ist wirkungsvoiler als die binoculare Photostimulation hinsichtlich des Triggerns von spike wave Entladungen während die unilaterale GL‐Stimulation weitgehend unwirksam ist. Sodium valproate (Na VPA) is more often prescribed in European countries than valproic acid (VPA) in the United States for two reasons: Na VPA has been available for clinical use for a longer time, since it was first introduced in France in 1967 and VPA was not authorized for use in the United States until 1978; and in the United States indications for VPA (syrup and capsules) and very recently enteric‐coated tablets of divalproex sodium include only absence seizures accompanied or not by other seizure types. In the majority of European countries, Na VPA (liquid forms and enteric‐coated tablets) may be prescribed in all types of epilepsy.Before outlining the indications and drug regimens of Na VPA/VPA in epilepsy, we shall consider how this drug is presently used in a realistic situation and not in a highly specialized cen

Summary:Some authors include somatomotor manifestations (SMM) among the clinical features of temporal lobe seizures in man; however, data are limited and conflicting. This study describes 101 seizures recorded during stereo‐EEG explorations in 50 patients, selected on the basis of the demonstration of onset in temporal lobe structures (stereo‐EEG) and the presence of lateralized SMM and/or secondary generalization (SG). SMM and SG were present in about 20% of our population of patients with temporal seizures explored with stereo‐EEG, and were only rarely observed during the first 10 s (early SMM) of the seizures (10 s) event in the ictal symptomatology. The face and the upper limb were by far the most frequently involved segments. Thirty‐eight percent of seizures ended up in SG. The associated ictal symptomatology was rather poor and differed from our previous findings in temporal lobe seizures: oroalimentary automatisms were relatively rare, whereas an impairment of consciousness was observed in more than 50% of seizures. The critical electrical discharge had a long duration and always involved extratemporal structures in one or both hemispheres. The involvement of the central region could be demonstrated in all patients with deep electrodes in that region. In conclusion, SMM are not characteristic of temporal lobe seizures, and their presence indicates spreading of the critical discharge beyond the temporal lobe.RÉSUMÉLes manifestations “somatomotrices” sont considérées par certains auteurs comme faisant partie de la sémiologie des crises du lobe temporal, cependant les données à ce sujet sont relativement rares et contradictoires. Cette étude porte sur 101 crises enregistrees chez 50 malades, au cours ď explorations stéréo‐EEG. Toutes ces crises débutent dans les structures temporales (stéréo‐EEG) et elles sont caractérisées, entre autre, par des manifestations somatomotrices (MSM) et/ou par une généralisation secondaire (GS). Nous avons pu observer des MSM et/ ou une GS chez environ 20% des malades ayant présenté des crises à origine temporale au cours de la stéréo‐EEG. Les MSM surviennent très rarement (90%) les MSM surviennent tardivement au cours de la crise, la face et le membre supérieur étant les segments le plus fréquemment atteints. 38% des crises se terminent par une generalisation secondaire. Les autres signes cliniques critiques sont relativement peu nombreux et différent considérablement du tableau clinique que nous avons pu observer au cours des crises temporales: les automatismes oroalimentaires sont relativement rares, alors qu'une rupture du contact survient dans plus de 50% des crises. La décharge critique, de longue durée, affecte toujours des structures extratemporales dans un ou les deux hémisphères. La mise en jeu de la région centrale a été démontrée chez tous les malades ches lesquels cette région a été explorée. En conclusion, les manifestations somatomotrices ne sont pas caractéristiques des crises temporales; leur existence indique que la décharge critique n'est pas limitée seulement au lobe temporal.RESUMENAlgunos autores incluyen las manifestaciones somato‐motoras (SMM) entre las characterísticas clínicas de los ataques del lóbulo temporal en humanos; sin embargo la informatión es limitada y conflictiva. Este estudio describe 101 ataques registrados durante exploraciones mediante estereo‐EEG en 50 pacientes y han sido seleccionados basándose en la demostración de su comienzo en estructuras del lóbulo temporal (estereo‐EEG) y en la presencia de manifestaciones somatomotoras lateralizadas y/o generalizatión secundaria (SG). Se han identificado las SMM y SG en alrededor del 20% de nuestros enfermos con crisis temporales registradas mediante el estereo‐EEG y fueron solo ocasionalmente observadas durante los primeros 10 segundos (SMM precoces) de los ataques (10 segundos) en la sintomatología ictal. La cara y la extremidad superior fueron, con creces, los segmentos más afectados y el 38% de los ataques finalizaron con una SG. La sintomatología ictal asociada era escasa y diferente de la encontrada en nuestros estudios previos de ataques temporales: los automatismos oroalimen‐tarios fueron relativamente raros mientras que alteraciones de conciencia se observaron en más del 50% de los ataques. La descarga eléctrica de la crisis tenía una larga duratión y siempre afectaba estructuras extra‐temporales en uno u otro hemisferio. La participateón de la región central podía demostrarse en todos los casos con electrodos profundos en esa región. Diese Untersuchung beschreibt 101 Anfälle, die bei Stereo‐EEG‐Ableitungen von 50 Patienten gewonnen wurden. Sie wurden ausgewählt, um den Anfallsbeginn in den Temporallappenstrukturen (Stereo‐EEG), die Gegenwart lateralisierter SMM und/oder eine sekun‐däre Generalisation (SG) nachzuweisen. SMM und SG waren bei etwa 20% der Patienten mit Temporallappenanfällen vorhanden, die stereoelektroenzephalographisch untersucht wurden. Sie wurden nur selten während der ersten 10 Sekunden (frühe SMM) des Anfalls beobachtet (unter 10%). Anfalle mit friihen SMM wurden im allgemeinen durch elektrische Stimulation oder chemische Aktivierung induziert und erfolgten nur ausnahmsweise spontan. In alien Fällen breiteten sich die ele‐kritschen Entladungen in einem frühen Stadium auch auf extra‐temporale Strukturen wie das Rolandische Operculum, den Parietallappen und die kontralateralen temporalen Strukturen aus. In den meisten Fällen (mehr als 90%) repräsentieren SMM ein spätes Ereignis der Anfallssymptomatologie (später als 10 Sekunden). Gesicht und obere Extremitäten waren bie weitem die am häufigsten befallenen Segmente. 38% der Anfälle endigten in einer sekundären Generalisation. Die iktale Begleitsymptomatologie war ziemlich dürftig und unterschied sich von unseren früheren Ergebnissen bei Temporallappenanfällen: Oro‐alimentäre Automatismen waren relativ selten, während eine Beeinträchtigung des Bewußtseins bei mehr als 50% der Anfälle beobachtet wurde.
