SUMMARY:Olfactory bulb kindling rates were studied in two inbred strains of genetically seizure‐prone mice (DBA/2 and El) and in three non‐epileptic inbred strains [C57BL/6 (B6), ddY, and C3H/He (C3H)]. None of the DBA/2 mice had been stimulated to seizure before or during the kindling and all mice were studied at 4–6 weeks of age, before development of spontaneous or movement‐induced seizure activity in the El strain. The audiogen‐ically seizure‐susceptible DBA/2 mice required the fewest number of stimulations to reach stage 5 seizure (mean ± SE = 4.0 ± 0.6). The nonepileptic C3H mice required the most stimulations to reach stage 5 seizures (22.6 ± 1.4). Kindling rates for B6 (9.6 ± 0.6), El (14.8 ±1.1), and ddY (18.5 ± 1.0) strains were intermediate, and the kindling rate for each strain was significantly different from that of the other strains. These findings show that the seizure‐susceptible El mouse kindles more rapidly than the genetically similar but nonepileptic ddY control and suggest that an inherited seizure susceptibility accelerates the kindling rate. Nonepileptic B6 mice kindled more rapidly than El mice, however, suggesting that genetic factors other than those that influence seizure susceptibility are of primary importance in the determination of the kindling rate.RÉSUMÉLes taux de kindling du bulbe olfactif ont étéétudiés dans deux souches consanguines de souris génétiquement prédisposées aux crises: souches DBA/2 et El, ainsi que chez 3 souches consanguines non épileptiques: C57B1/6 (B6), ddY et C3H/HE (C3H). Summary:Animal experimental and human clinical investigations show that estrogens lower and progestins raise many seizure thresholds. In women, seizure frequency varies with the serum estradiol to progesterone ratio. The fluctuation of this ratio during the menstrual cycle is a major factor in catamenial epilepsy. A decline in serum antiseizure medication levels premenstrually may be another factor. Estradiol to progesterone ratios are elevated in anovulatory or inadequate luteal phase cycles. This may explain a propensity for seizure onset at the time of menarche and the exacerbation of seizures during the months or years leading up to menopause. It may also be an important factor in the association between reproductive endocrine disorders and epilepsy. Specifically, polycystic ovarian syndrome and hypogonadotro‐pic hypogonadism are significantly overrepresented among women with epilepsy. Epilepsy may promote the development of these disorders. These disorders, in turn, are characterized by inadequate luteal phase cycles that may promote the development or occurrence of seizures. SUMMARY:Panic disorder is a psychiatric diagnosis whose main feature is paroxysmal attacks of anxiety that strike suddenly without apparent provocation. Physicians explain the attacks as an ictal phenomenon in patients with known seizures because of their similarities to complex partial seizures. We report eight patients with seizures and panic disorder. Recognition of a second diagnostic entity resulted in a beneficial change in treatment in six of the eight. We did not find an increased incidence of panic disorder in our seizure clinic population as compared with the genera] population.RÉSUMÉL'attaque de panique est un diagnostic psychiatrique caractérisé surtout par la survenue soudaine, sans provocation apparente, d'une crise d'anxiété Il existe des similarités avec les crises partielles complexes et, chez les patients présentant des crises documentées, on peut être tenté d'expliquer ces attaques comme un phénomène critique. Summary:Androgen deficiency is unusually common among men with epilepsy. It may contribute to reproductive and sexual dysfunction and possibly exacerbate seizure frequency. The most important androgen is testosterone. It exists in the serum in a free form or bound to albumin or sex hormone‐binding globulin (SHBG). Free testosterone levels have correlated significantly with measures of potency and sexual interest. The possibility that measures of non‐SHBG‐bound testosterone may provide a more sensitive assessment of biologically and perhaps clinically significant androgen levels is raised for consideration. Androgen deficiency may result from increased catabolism and binding induced by antiepileptic drugs (AEDs). It is a feature of the reproductive endocrine disorders that are often associated with epilepsy: hypogonad‐otropic hypogonadism, hypergonadotropic hypogonadism, and functional hyperprolactinemia. It may be a consequence of medication‐induced elevations in serum estradiol. Estradiol exerts a potent inhibitory influence on luteinizing hormone secretion and may contribute to premature aging of the reproductive system, both at the level of the testes and the hypothalamus. Testosterone therapy may moderately benefit reproductive and sexual function. Despite its antiseizure effects in animal experiments, however, it has not been reported to improve seizures clinically. One possible explanation is that AEDs that induce enzyme synthesis may enhance the conversion of testosterone to estradiol by aromatase. Summary:Sexual dysfunction may arise more frequently in men and women with epilepsy than with other chronic illnesses, manifesting primarily as diminished sexual desire and potency. Studies using retrospective self‐report of sexual attitude and behavior find an incidence of sexual dysfunction ranging from 14–66%. Sexual dysfunction may be more common in partial than in generalized epilepsies. Sexual dysfunction in epilepsy may result from a disturbance in social or psychological factors affecting sexual responsiveness. Alternatively, epileptiform discharges may disrupt the function of structures mediating sexual behavior, particularly the limbic cortex, or alter the release of hypothalamic or pituitary hormones. Antiepileptic drugs modulate hormone release from the hypothalamic‐pituitary‐gonadal axis and may have direct inhibitory effects on sexual behavior. SUMMARY:We report three cases of reflex epilepsy with myoclonic jerks of the right arm and fingers precipitated by calculation using a Soroban, An EEG spike‐wave complex with left central prevalence was induced. Various types of stimulation were used to induce epileptic discharges, and a simultaneous mental task requiring a high degree of concentration and complicated and delicate finger movements was necessary to induce the epileptic discharges. Comparison of our cases with previously reported reflex epilepsy induced by higher mental activity led to the assumption that the neural mechanism inducing seizures in our cases is similar to that of writing epilepsy. Valproate was effective in reducing epileptic discharges, and all patients became seizure‐free.RÉSUMÉLes auteurs rapportent 3 cas d'épilepsie réflexe avec secousses myocloniques du bras droit et des doigts, provoqués par le calcul au moyen d'un soroban. Sur l'EEG, des pointes‐ondes prédominant sur la région centrale gauche ont également été induites. Différents types de stimulations ont été réalisés, et les auteurs ont constaté qu'il était indispensable qu'il y ait de façon simultanée un travail mental impliquant un degréélevé de concentration et des mouvements compliqués et délicats au niveau des doigts, pour obtenir l'induction des décharges épileptiques. SUMMARY:Seventy‐eight individuals with EEG records containing benign epileptiform transients of sleep (BETS) were identified among 7,400 records reviewed in our laboratory in a 6‐year period. The records contained no other abnormality in 51 patients (65%). Genuine epileptiform discharges were found in the records of 19 patients; 14 had a history of epilepsy. Thirty‐five patients (45%) had a proven history of epilepsy with antiepileptic drug (AED) therapy. In the records of these patients, the mean number of BETS per unit of time was significantly higher (11.88 ± 2) than in the record of the rest of the laboratory population with BETS (6.89 ± 0.9) (p<0.02). Among five conventional surface montages, ipsilateral ear referential montage (IERM) showed a significantly higher number of BETS per unit of time than did any other surface montage used in the study. Thirty‐nine records (50%) were performed after sleep deprivation (SD). When only IERM was considered, SD records showed a significantly higher number of BETS per unit of time (7.36 ± 1.1) than did non‐SD records (3.89 ± 0.69) (p<0.01). Our findings support the general consensus that individual BETS may be normal variants, but a high occurrence of BETS in the record should raise suspicion of underlying epilepsy.RÉSUMÉLes auteurs ont identifiés 78 sujets présentant des figures tran‐sitoires épileptiformes bénignes du sommeil (BETS) parmi 7400 enregistrements effectués sur une période de 6 ans. Ces anomalies étaient isolées chez 51 patients (65%). De vraies décharges épileptiques ont été constatées chez 19 patients, dont 14 présentaient des antécédents épileptiques. Au total, 35 patients (35%) avaient des antécédents prouvés d'épilepsie et de traitements antiépileptiques. Les tracés de ces patients montraient un taux de BETS par unité de temps significativement supérieur à ceux des autres sujets étudiés au laboratoire présentant des BETS (11.88 ± 2 contre 6.89 ± 9, p<0.02). Parmi 5 montages standards d'électrodes de surface, le montage avec électrode auriculaire ipsilatérale réferéntielle a permi de constater un nombre significativement plus élevé de BETS par unité de temps que tout autre montage de surface utilisé dans ce travail. Trente neuf enregistrements (50%) ont été réalises après privation de sommeil. En ne considérant que le montage par référence à l'oreille, les tracés après privation de sommeil ont mis en évidence un nombre significativement plus élevé de BETS par unité de temps que les tracés sans privation de sommeil (7.36 ±1.1 contre 3.89 ± 0.69 p<0.01). In den Ableitungen dieser Patienten lag der Durchschnitt der BETS pro Zeiteinheit signifikant höher (11.88 ± 2) als bei den übrigen Probanden mit BETS (6.89 ± 0.9) (p<0.02). In den benutzten 5 Ableiteschemata kamen die meisten BETS pro Zeiteinheit bei Ableitung gegen das ipsilaterale Ohr als Referen zelektrode (IE) zur Darstellung. 39 Ableitungen (50%) erfolgten nach Schlafdeprivation (SD). Summary:Serum prolactin (HPR) levels are influenced by waking and sleep states, as reflected by surges in serum concentrations during daytime naps and nocturnal sleep. Other physiological causes of hyperprolactinemia include sexual activity, pregnancy, and lactation. Drugs may stimulate or inhibit HPR secretion. Pathological causes for HPR secretion include destructive lesions of the hypothalamus, prolactin‐secreting neoplasms of the pituitary gland, lesions of the spinal cord, and occasionally Parkinson's disease. The most predictable postictal changes are increased serum Cortisol levels and hyperprolactinemia. Serum HPR rises after virtually all generalized tonic‐clonic seizures, most complex partial seizures, and some simple partial seizures. Absence and myoclonic seizures do not affect serum HPR levels. Repeated epileptic seizures and electroconvulsive therapy treatments produce successively less marked rises in serum HPR. The postictal elevation of serum Cortisol has a longer latency than for HPR and follows an earlier rise in serum ACTH. Other postictal hormonal changes are much more variable. Because of the normal diurnal variation in serum Cortisol levels and the relative delay in the postictal elevation of serum Cortisol, HPR is more useful as a diagnostic measure of epileptic seizures. This application of HPR requires an understanding of other factors that influence serum HPR and the use of baseline serum HPR levels for comparison. Summary:: Women with epilepsy account for approximately 0.5% of all pregnancies. Their pregnancies are high risk because of an increased frequency of maternal seizures, complications of pregnancy, and adverse pregnancy outcomes. The increase in seizure frequency is associated with a progressive decline in antiepileptic drug (AED) levels during pregnancy even with constant dosing. Fetal deaths after a generalized seizure, although rare, have been reported, and a marked decline in fetal heart rate has been demonstrated after such seizures during delivery. AEDs have been implicated in causing a twofold increase in the rate of congenital malformations, a variety of minor physical anomalies, mostly involving the midface, and a neonatal hemorrhagic disorder. The clinician caring for a pregnant woman with epilepsy is therefore faced with a dilemma and must carefully chart a middle ground providing effective seizure control while minimizing fetal exposure to AEDs

SUMMARY:Three children had both nocturnal unilateral motor seizures and daytime ipsilateral “negative myoclonus” which occurred so frequently that it resembled asterixis. Neurophysiologic studies demonstrated lateralized spike discharges that were time‐locked to postural lapse in the contralateral outstretched arm. The clinical course was characterized by good seizure control with benzodiazepines.RÉSUMÉLes auteurs rapportent trois observations d'enfants présentant l'association de crises motrices nocturnes unilatérales et de myoclonus négatif ipsilatéral diurne, les épisodes étant si fréquents que le tableau évoquait un astérixis.
