Summary:Progabide (SL 76002) was studied in a randomized double‐blind crossover trial using 20 outpatients suffering from partial complex seizures. Progabide was added to the concomitant antiepileptic treatment in a fixed dosage schedule. The design included an open therapy control unit. No significant difference was established between the number of partial seizures during treatment with progabide and placebo. A trend was observed for lower seizure frequency of secondary generalized seizures during treatment with progabide. Only mild and transient side effects were observed. There was no difference between the side effects of progabide and placebo.RÉSUMÉLe progabide (SL 76002) a étéétudié selon un protocole croisé en double aveugle chez 20 malades ex‐ternes souffrant de crises partielles complexes. Le progabide a été ajouté au traitement antiépileptique en cours à posologie fixe. Le plan D'études prévoyait une surveillance de la thérapeutique par un médecin informé du code. Aucune différence significative n'a pu êtréétablie entre le nombre de crises partielles pendant le traitement par progabide et pendant le traitement par placebo. On a observé sous progabide une tendance à la diminution de la fréquence des crises secondairement généralisées. Les effets collatéraux ont été minimes, transitoires et identiques avec le progabide et le placebo.RESUMENMediante un ensayo “cross‐over” randomicado y doble ciego que incluy6 20 enfermos ambulatorios que padecfan ataques parciales complejos se ha estudiado la accion del progabide. Es bestanden keine signifikanten Unterschiede zwischen der Anzahl der Partialanfälle unter Behandlung mit Progabid und Placebo. Summary:Of 363 infants with infantile spasms (IS) admitted into our hospital during an 18‐year period, 286 were closely followed for at least 6 years and form the basis of this report. These patients were divided into symptomatic (165 cases) or cryptogenic (121 cases) groups. The cryptogenic group consisted of patients exhibiting no etiological clues and in whom development until the onset of the disorder was normal, even if some had mild impurities in their previous biography. We did not use the “doubtful subgroup” proposed by others for infants with no known etiology but with definite previous psychomotor delay. Close monitoring of ictal phenomena confirmed the latter variability and strengthened the view that IS should not be classified as a distinct seizure pattern. No statistically significant clues for etiology or prognosis were offered by the analysis of prevailing seizure patterns or frequency. Etiologies in the symptomatic group were subsumed within pre‐, peri‐, and post‐natal periods. Asphyctic events predominated in this series. Dysgenetic, infectious, metabolic, and hemorrhagic factors were almost equally represented, except for the first, which was found somewhat more frequently. That postimmunization reactions may be one cause of IS was not considered sufficiently documented. A gender predominance (♀: ♂= 2:1) occurred within the symptomatic group only. No significant genetic trends could be revealed after excluding those disorders that themselves carry clear genetic implications. Age of onset was earlier than in most reports because we corrected for true concep‐tional age. When this was done, onset of IS below 3 months carried a more unfavorable prognosis (p<0.001). As expected, the presence of preceding neurological impairment resulted in worse outcome. Infants who had typical hypsarrhythmic EEG at the onset of IS fared better than those with atypical EEG (p<0.02). While bacterial infections usually aggravated the condition, viral ones seemed to temporarily ameliorate it. On a short‐term follow‐up (10 months from onset treatment), the hypsarrhythmic pattern may disappear in both groups regardless of therapies, but on a long‐term basis (6 years), the EEG evolution showed no statistically clear correlations with early therapies. The evolution into other seizure patterns also appeared independent of therapies. In our series less than a quarter eventually developed the Lennox‐Gastaut syndrome. Controlled therapeutic protocols were adopted in an effort to resolve ongoing controversy about optimal treatment. Both groups were maintained, thoughout the therapeutic trial, on either antiepileptic drugs, on oral steroids alone, or on nonsynthetic ACTH alone, the latter followed in one subgroup by oral steroids also. Dosages were based on body surface or weight. Whenever cross‐over or a change in the initial therapeutic regimen became necessary, the case was eliminated from the study. Clinical and laboratory tests were obtained at prefixed periods of time. For the short‐term assessment both cohorts were included. No statistical difference emerged between the controlled therapeutic regimens, each of the latter causing amelioration or not, except for a trend favoring the ACTH treatment. To establish long‐term effects (6 years) it was felt that only the more homogenous cryptogenic group would allow valid conclusions. The latter indicated that cases treated with nonsynthetic ACTH fared better than those treated with antiepileptic drugs or with oral steroids alone. This more favorable outcome included a lower incidence of later epilepsy (p<0.05) and better psychomotor status (p<0.025 to p<0.05). Evolution of EEG and of motor‐sensory deficits did not appear to be significantly influenced. Babies started on ACTH within a month from onset of IS fared better than those in whom ACTH therapy was delayed (p<0.01). These results also suggest that the effects of nonsynthetic ACTH may in part be independent of Cortisol release, through some direct influence upon the CNS.RÉSUMÉParmi 363 nourrissons hospitalisés pour des spasmes infantiles (IS) sur une période de 18 ans, 286 suivis pendant au moins 6 ans forment le matériel de cette étude. lis ont été divisés en 165 cas symptomatiques et 121 cas cryptogénétiques. Les IS ne peuvent pas être considers comme un “pattern” critique univoque puisqu'une observation minutieuse des crises montre leur polymorphisme, qui n'a D'ailleurs pas de signification étiologique ou pronostique, non plus que la fréquence des crises. Dans le groupe symptoma‐tique L'étiologie la plus fréquente est D'ordre anoxique. Viennent ensuite L'étiologie dysgénétique, puis, avec une fréquence à peu prés égale, les étiologies infectieuses, métaboliques, hémorragiques. En dehors des affections génétiques connues, aucun facteur génétique particulier n'est apparu. L'étiologie post‐vaccinale n'apparaissait pas comme suffisamment documentée. Une prédominance de garçons (2 pour une fille) n'a été trouvé que dans le groupe symptomatique. L'âge de début (âge conceptionnel vrai) avant 3 mois et L'exis‐tence D'anomalies neurologiques avant le début des spasmes entratnaient un pronostic plus defavorable (p<0.001 pour L'âge de début). Use hypsarythmie typique sur les tracés EEG au debut avait un meilleur prognostic qu'un tracé atypique (p<0.02). Dans les 10 mois suivant le début du traitement, quel qu'il soit, L'hypsarythmie pouvait disparaftre dans les 2 groupes. Mais à long terme (6 ans) L'évolution EEG et L'évolution vers D'autres types de crises apparaissaient indépendantes des traitements utilisés au début. Moins D' 1/4 des cas ont présenté ensuite un syndrome de Lennox‐Gastaut. Des protocoles thérapeutiques con‐trôlés ont été adoptés pour essayer de trancher la controverse à propos du traitement. Médicaments antiépileptiques, corticoídes oraux, ACTH non synthétique, seule ou associée aux cortioídes oraux, ont ete donnes, a des doses calculées selon la surface corporelle ou le poids, sous surveillance clinique et biologique régulière. L'évolution des résultats à court terme n'a fait apparaítre aucune difference statistique entre les différents traitements contrôlés, en dehors D'une tendance favorable pour L'ACTH. L'évaluation des effets a long terme (6 ans) a été faite seulement sur le groupe cryptogénétique. Les cas traités par ACTH seule ont eu une évolution meilleure que les autres: incidence plus faible D'épilepsie ultérieure (p<0.05) et meilleur développement psychomoteur (de p<0.025 a p<0.05) sans différence significative des tracés EEG ni des déficits sensori‐moteurs. Un traitement précoce (délai<1 mois) donnait de meilleurs resultats qu'un traitement tardif (p<0.01). Les effets de L'ACTH pourraient étre partiellement indépendants de la secrétion de Cortisol, agissant directement sur le SNC.RESUMENEn un period de 18 años 363 infantes fueron admitidos en Nuestro Hospital con Espasmos Infantiles (EI). 286 de ellos, seguidos por lo menos por 6 años, formen la base de este trabajo. Los niños fueron divididos en dos grupos: sintomático (165 casos) y criptogenico (121 casos). Como criptogenico fueron definidos aquellos casos en quienes no se encontró una etiologia, cuyo desarrollo psycomotor fue normal hasta el cominzo de la enfermedad, incluyendo casos con alqunas abnormalidades en su historia personal previa. En este trabajo no se uso el termino “subgrupo dudoso” propuesto por otros autores en referenda a infantes con etiologia desconocida y retraso psychomotor previo al comienzo de las convulsiones. La observación cuidadosa de los phenomenes convulsivos refuerza el concepto de que los EI no deben clasificarse como un patron epiléptico diferente. Ni el analisis del tipo de convulsiones ni de la frecuencia de las mismas tuvo relación con el pronostico no con el diagnóstico. Las causas etiológicas en el grupo sintomático fueron divididas en pre‐peri y post‐natales. Asphixia fue la causa más frequente. Sequida por disgenesia cerebral. Infecciones, desordenes meta‐bólicos y hemor Predominancia masculina (2:1) se encontró en el grupo sintomático solamente una vez excluidos aquellos desordenes que presentan una implicación genética clara, no influencia genetica fue demostrada.La edad de comienzo fue mas temprana que en otros reportes porque se hizo una correccion por edad con‐ceptcional. Comienzo de los E.I. en infantes menos de 3 meses tiene un pronóstico peor (p<0.001). La presencia de daflo neurologico previo, como era de es‐perar, tambien tiene un pron6stico peor. Niños con tipica hipsarrhythmia al comienzo de los E.I. tuvieron mejor pronostico que aquellos con EEG atipico. (p<0.02). Mientras que infecciones bacterianas usu‐almente agravaron la enfermedad, las infecciones vir‐ales parecen produci una mejorfa temporal. Cuando el sequimiento electroencefalógrafico fue corto (10 meses desde el comienzo del tratamiento) el patron hypsarrhythmico puede desaparecer en los dos grupos independiente de las terapia empleada mientras que cuando el seguimiento fur prolongado (6 años) la evolución electroencefalógrafica no mostro correlacion con los tratamientos comenzados en edad, temprana. La evolución en otros patrones convulsivos tampoco mostró correlatión con el tratamiento. En esta serie menos de la cuarta parte desarrolló el sin‐drome de Lennox‐Gastaut. Con el objeto de resolver diferencias de opinion acerca del tratamiento más optimo, los pacientes fueron tratados sequiendo pro‐tocolos therapeuticos. Los dos grupos fueron man‐tenidos, a través de la prueba terapeutica, en medication antiepiléptica, en esteroides orales o en ACTH no‐sintetico. En este ultimo grupo, un subgrupo de pacientes recibieron esteroides orales. Las doses fueron basadas en el peso o la superficie corporal. Los casos en los cuales hubo cambios en le terapeutica original fueron eliminados del estudio. Examenes clinicos y de laboratorie fueron realizados en fechas preestablecidas. En la evaluacibn del efecto terapeutico a corto plazo los dos grupos los pacientes fueron incluidos. No hubo diferencia significativa entre los diferentes esquemas terapeuticos aunque hubo una tendencfa en favor del ACTH. Para evaluar los efectos a larzo plazo (6 años) solamente el grupo criptogenico fue estudiado. En este grupo, los casos tratados con ACTH nosintetico tuvieron mejor pronostico que aquellos tratados solamente con drogas antiepilepticas o con esteroides orales. El pronóstico favorable incluye una menor incidencia de epilepsia (p<0.05) y un desarrollo psychomotor mejor (p<0.025 to p<0.05). La evolucion de EEG asf como deficits sensori‐motores no mostraron ninguna diferencia. Los infantes en quienes el tratamiento con ACTH comenzó en el primer mes del comienzo de las convulsiones tuvieron mejor pronosticó que aquellos en quienes el tratamiento comenzó maTs tarde (p<0.01) Estos re‐sultados sueieren que el efecto del ACTH no‐sintético puede, en parte, ser independiente de las secrecion de Cortisol, y tener effecto directo sobre el sistema ner‐vioso central.ZUSAMMENFASSUNGVon 363 Kindern mit BNS‐KrSmpfen, die wahrend 18 Jahren in dieses Krankenhaus uberwiesen wurden, konnten 286 mindestens 6 Jahre engmaschig über‐wacht werden. 165 Patienten gehörten der symp‐tomatischen, 121 Patienten der kryptogenen Gruppe an. Unter kryptogen verstehen wir Patienten mit BNS‐Krämpfen ohne ätiologische Rückschlüsse, mit normaler Entwicklung bis zum Beginn der Erkran‐kung. Wir vermieden, eine Gruppe mit fraglicher Ätiologie zu bilden, wie sie von anderen für Kinder vorgeschlagen wird, bei denen die Ätiologie unbekannt ist, die aber eine eindeutige psychomotorische Entwicklungsverzögerung zeigen. Es war nicht möglich, durch die Analyse des vorherrschenden Anfallsmusters oder die Anfallsfrequenz statistisch signifikante Rückschlusse auf die Ätiologie und die Prognose zu Ziehen. Das Anfallsbild war sehr variabel und unterstreicht die Ansicht, daß BNS‐Krämpfe nicht alseinwohldefiniertes Anfallsbild angesehen werden sol‐lten. In der symptomatischen Gruppe wurde zwischen einer präperi‐ und postnatalen Ätiologie unterschieden. Asphyktische Zustände herrschten in dieser Serie vor. Infektiöse, metabolische und hämor‐rhagische Faktoren waren fast gleichermaßen reprasentiert, eine Dyskinesie wurde etwas häufiger gefunden. Die Annahme, daß BNS‐Krämpfe eine Folge von Impfungen sein könnten, konnte wegen un‐genügender Dokumentation nicht weiter verfolgt werden. Nur in der symptomatischen Gruppe fand sich eine Knabenwendigkeit (Jungen:Mädchen = 2:1). Nachdem Erkrankungen als Ursachen der BNS‐Krämpfe ausgeschlossen waren, die genetisch bedingt sind, fand sich kein Hinweis auf genetische Faktoren fur die Entstehung der BNS‐Krämpfe. Das Alter bei Krankheitsbeginn liegt etwas früher als in den meisten Berichten, da wir auf das Konzeptionsalter zurück‐rechneten. Nach dieser Korrektur zeigten BNS‐Krämpfe mit einem Beginn in den ersten 3 Leben‐smonaten eine ungünstigere Prognose (p<0,001). Wie zu erwarten, war die Prognose schlechter bei neurologischer Vorschädigung. Kinder mit typis‐cher Hypsarrhythmie im EEG bei Beginn der BNS‐Krämpfe schnitten besser ab als solche mit atypis‐chem EEG (p<0,02). Wahrend bakterielle Infek‐tionen den Zustand gewOhnlich verschlimmerten, schienen virale ihn vortlüergehend zu verbessern. Bei kurzzeitiger Verlaufsbeobachtung (10 Monate nach Behandlungsbeginn) kann die Hypsarrhythmie in beiden Gruppen unabhangig von der Therapie ver‐schwinden; bei längerer Beobachtung (6Jahre) zeigt die EEG‐Entwicklung keine statistisch signifikante Korrelation zur Frühbehandlung. Auch der Übergang in andere Anfallsmuster schien unabhängig von der Therapie zu sein. In unserer Serie entwickelte weniger als ein Viertel ein Lennox‐Gastaut‐Syndrom. Wir verwendeten kontrollierte therapeutische Protokolle um zu versuchen, die Kontroverse über die optimale Behandlung zu schlichten. Beide Gruppen erhielten wahrend des Versuchs entweder Antiepileptika oder orale Steroide alleine oder nur ein nicht synthetisches ACTH. Eine Untergruppe der letzteren erhielt anschlieBend auch orale Steroide. Die Dosierung richtete sich nach der Körperoberfläche oder dem Gewicht. Ein notwendiger Wechsel des initialen Therapie‐plans führte zur Eliminierung aus der Studie. Klinische und Laboruntersuchungen erfolgten in den festgesetzten Intervallen. Für die Kurzzeitbehandlung wurden beide Kohorten berücksichtigt. Es ergaben sich keine statistischen Differenzen hinsichtlich der kontrollierten Therapieplane, beide führten zur Besserung oder nicht, ein Trend schien das ACTH zu favorisieren. Fur die Langzeitbeurteilung (6 Jahre) wurde die mehr homogene, kryptogene Gruppe alleine verwendet. Es zeigte sich, daß Patienten mit nicht synthetischem ACTH besser abschnitten als solche mit Antiepileptika oder oralen Steroiden alleine. Dieses günstige Ergebnis bezog sich auf eine geringere Häufigkeit späterer epileptischer Anfalle (p<0,05) und eine günstigere psychomotorische Entwicklung (p<0,025 bis p<0,05). Die EEG‐Entwicklung und das Aufholen sensomotorischer Defizite schienen nicht signifikant beeinflusst. Säuglinge, die ACTH innerhalb eines Monats von Krankheitsbeginn erhielten, schnitten besser ab als die, in denen die ACTH‐Therapie verzogert begann (p<0,01). Diese Ergebnisse lassen vermuten,

Summary:Infantile spasms begin in a very active phase of brain development. The new neuropathological techniques have shed light on changes in the fine structure of brain in children with infantile spasms. An understanding of the action of adrenocorticotropic hormone on the biochemistry and development of the brain provides, in an indirect way, a partial explanation of the biochemical events in this syndrome; most of these actions are well documented in animal experiments. Some anticonvulsants are also effective in treating infantile spasms, and modern research has explained many actions of these agents as well. The principal mechanism causing retardation in most children is unknown. Some future aspects of the research are reviewed.RÉSUMÉLes spasmes infantiles commencent dans une phase tres active du développement cérébral. Chez des en‐fants atteints de spasmes infantiles les nouvelles techniques neuropathologiques ont mis en évidence des modifications de la structure ce>6brale fine. La con‐naissance des modes D'action de L'ACTH sur la biochimie et le développement du cerveau fournit une voie indirecte D'explication partielle des événements biochimiques accompagnant les spasmes infantiles. La plupart de ces modes D'action sont bien documentés chez L'animal. Certains anticonvulsivants sont également efficaces dans la traitement des spasmes infan‐tiles. La recherche moderne a expliqué beaucoup de leurs modes D'actions sur le cerveau humain. Le mécanisme principal responsable du retard observe chez la plupart des enfants est inconnu. Certains aspects futurologiques de la recherche sont envisagés.RESUMENLos espasmos infantiles comienzan en una fase de gran actividad en el desarrollo cerebral. Las técnicas neuropatológicas realizadas en niños con espasmos infantiles solo han mostrado mínimos cambios en la fina estructura cerebral. El conocimiento actual de la acción del ACTH sobre la bioquímica y el desarrollo cerebral proporciona, de un modo indirecto, una ex‐plicación parcial de los acontecimientos bioquímicos en los espasmos infantiles. La mayoría de las acciones están bien documentadas en experimentos animales. Algunos anticonvulsivantes también son eficaces en su tratamiento. La investigación moderna explica muchas de sus acciones en el cerebro humano. El mecanismo principal que causa retraso en la mayor parte de los niños es desconocido. Die Kenntnis von Wirkungen von ACTH auf die biochemischen Prozesse im Gehirn und auf Hirnentwicklung hat Einiges von biochemischem Geschehen in infantilen Spasmen zu verstehen ermöglicht. Die meisten Wirkungen hat man gut in Tierversuchen dokumentiert. Einige antikonvulsive Medikamente sind auch wirksam in der Behandlung der infantilen Spasmen. Summary:Seizures produced by intracerebral injection of zinc sulfate in rabbits are a new chronic model of experimental epilepsy. The main features of this model are: the animals are easily controlled, the electrocorticogram is conveniently recorded, the endpoints are definite, and the rate of seizure is higher than with other methods. The commonly used antiepileptic drugs, such as phenobarbital (30 mg/kg), diphenylhydantoin (30 mg/kg), nitrazepam (3 mg/kg), and sodium valproate (300 mg/kg), have therapeutic effects in treating this experimental epilepsy, when they are given intravenously. But they can not protect the rabbits from death, except phenobarbital.RÉSUMÉLa production de crises par injection intracérébrale de sulfate de zinc chez le lapin est un nouveau modèle D'épilepsie expérimentale chronique. Les caractèristiques principales de ce modèle sont les suivantes: les animaux peuvent être aisément contrôlés; L'ECoG est facilement enregistré le début et la fin des épisodes sont bien définis; la fréquence des crises est plus é1evée que dans les autres méthodes. Les antiépileptiques courants tels que phenobarbital (30 mg/kg), diphenylhydantoine (30 mg/kg), nitrazepam (3 mg/kg) et valproate de sodium (300 mg/kg) ont des effets thérapeutiques positifs sur cette épilepsie expérimentale lorsqu'ils sont administrés par voie in‐traveineuse. Summary:Piperine and its derivatives are effective anticonvulsant drugs that antagonize convulsions induced by physical and chemical methods. Their major anticonvulsant activity as shown in animal tests lies in modification of the maximal electroshock seizure pattern. They also have sedative‐hypnotic, tranquilizing, and muscle‐relaxing actions and can intensify the depressive action of other depressants, when used in combination. Antiepilepsirine, one of the derivatives of piperine, is used as an antiepileptic drug in treating different types of epilepsy. It has been proved effective and is being widely used in China. The anticonvulsant action of 7446, 7448, and 7903 is more potent than that of antiepilepsirine. The chemical structure of piperine and its derivatives is different from that of prototype antiepileptic drugs, and, therefore, these may become a new group of antiepileptic drugs.RÉSUMÉLa piperine et ses dérivés sont des drogues anticonvulsives efficaces. Summary:In order to evaluate immediate recall and learning in epileptic patients, four tests were chosen: Wechsler's memory span for digits, graphic reproduction of geometric figures from the Wechsler Memory Scale, learning of a list of words from the Rey Memory Scale, and recognition of these words. These tests were performed on 200 epileptic patients over the age of 15 years, without defined cerebral lesions, and with a normal or subnormal social adjustment. Memory impairment was analyzed with respect to the following variables: seizure frequency, seizure type, duration of the disorder, and anticonvulsant medication. Patient data were compared with values obtained for a population of 100 normal subjects matched for age and level of education. Comparison between epileptics and controls clearly demonstrated a statistically significant memory impairment in the group of patients. However, the reasons for these poor performances are not clear, and none of the studied parameters (type of epilepsy, frequency of seizures, duration of the disease, and medication), if considered alone, accounts for this impairment.RÉSUMÉQuatre tests ont été choisis pour explorer la m6m‐oire immédiate et L'apprentissage à court terme chez les épileptiques: chiffres et figures géométriques du Wechsler Mémoire, liste de mots du test de mémoire de Rey (apprentissage et recognition). Les tests ont été passés à 200 épileptiques, àgés de plus de 15 ans, sans lésioncérébraleeAvidence et ayant une insertion sociale normale ou subnormale. Le résultat des tests a été analysé en fonction du type des crises et de leur fréquence, de L'électroencéphalogramme intercritiqué, de L'anciennete de la maladie et du traitement en cours. II a été comparé aux résultats obtenus chez 100 témoins, appariés en fonction de L'âge et du niveau culturel. Les comparaisons entre les épileptiques et les témoins ont montré une altération statistiquement significative de la mémoire chez les premiers. Toute‐fois les mécanismes sous‐tendant ces troubles mnésiques ne sont pas simples et aucun des paramèt‐res considérés (type D'épilepsie, etc.) n'en rend comp‐te à lui seul.RESUMENPara valorar la evocatión inmediata y el aprendizaje en epilépticos se han elegido cuatro pruebas: la retentión de díUgitos de Wechsler, la reproductión gráfica de figuras geométricas, el aprendizaje de una lista de palabras de la escala de memoria Rey y el recono‐cimiento de dichas palabras. Estas pruebas fueron aplicadas a 200 epilépticos mayores de 15 años sin lesiones cerebrales definidas y con un acoplamiento social normal o subnormal. La alteratión de la memoria se analizó con respecto a las variables siguientes: frecuencia de ataques, tipo, duratión del proceso y medicación anticonvulsiva. La informatión sobre los pacientes fué comparada con los valores obtenidos para una poblacioan normal de 100 sujetos ajustada a edad y nivel de educatión. La comparación entre epilepticos y controles demostró claramente una alteration de la memoria estadísticamente significativa en el grupo de pacientes. Sin embargo, no aparecieron claras las razones de estos pobres resultados y ninguno de los parámentros, si se consideraban aisladamente (tipo de epilepsía, frecuencia de ataques, duratión de la enfermedad y medicatión), podía ser responsable de esta alteratión.ZUSAMMENFASSUNGUm das unmittelbare Erinnern und das Lernen bei epileptischen Patienten zu prilfen, wurden 4 Tests aus‐gewählt: Der Wechsler Gedächtnistest für Zahlen und die graphische Reproduktion geometrischer Figuren. des Lernen einer Liste von Wörtern aus der Rey Gedáchtnisscala und das Wiedererkennen dieser Worte. Diese Tests wurden bei 200 epileptischen Patienten über 15 Jahren durchgefürht, die keine de‐finierte cerebrale Läsion hatten und normal oder annähernd normal sozial integriert waren. Die Stöning des Gedächtnis wurde im Hinblick auf die folgenden Variablen analysiert: Anfallsfrequenz, Anfallstyp. Dauer der Störung und antikonvulsive Medikation. Die Patientendaten wurden mit solchen verglichen, die von 100 Personen einer normalen Bevölkerung, nach Alter und Ausbildung angeglichen, gewonnen wurden. Summary:Study of the developing central nervous system can lead to better understanding of the mature central nervous system. The thalamocortical augmenting response is a complex neurophysiological response considered to be related to the occurrence of some forms of epileptic activity. Using a paradigm previously developed in adult cats, we assessed the development of the thalamocortical augmenting response in kittens and found that the relative proportion of thalamocortical activity occurring at high frequencies of thalamic stimulation increased with increasing age. Anticonvulsants effective against petit mal seizures also increase the relative proportion of thalamocortical activity following high frequencies of thalamic stimulation. Developmental changes in the thalamocortical augmenting response can be related to the age‐dependent decreases in the prevalence of petit mal seizures.RESUMENEl estudio del sistema nervioso central en desarrollo puede conducir a una mejor comprensión del sistema nervioso central maduro. La respuesta incrementa‐dora tálamo‐cortical es neurofisiógicamente compleja y se acostumbra a relacionarla con alguna forma de actividad epitéptica. Usando un modelo previamente desarrollado en gatos adultos hemos de‐terminado el desarrollo del aumento de la respuesta tálamo‐cortical en gatos jóvenes encontrando que la proporciön relativa de la actividad tálamo‐cortical que ocurría en la estimulación talámica a altas frecuencias, aumenta con la edad. Los anticonvulsivos que son eficaces en el tratamiento del petit‐mal tambten in‐crementan la proporción relativa de la actividad tálamocortical que sigue la estimulación talámica a altas frecuencias. Summary:A coinciding temporal sequence of electrophysiological and biochemical correlates of developing generalized penicillin epilepsy in cats may indicate a “cause and effect” relationship between the two phenomena. After intramuscular injection of penicillin, in the pre‐epileptic state prior to the onset of spike‐and‐wave discharge, the cortical content of glutamic acid decreases. This change occurs when an increased amplitude of visual evoked potentials in association cortex heralds the approach of spike‐and‐wave activity. The decrease of glutamic acid and that of aspartic acid occur in parallel with an almost stoichiometric increase of glutamine, γ‐aminobutyric acid (GABA), or both, while taurine levels in the pre‐epileptic state remain near normal. As the preepileptic progresses to the epileptic state, characterized by generalized 4–5 cycles/s spike‐and‐wave discharges, a failure of the glial capture mechanisms for taurine and glutamate appears to occur, since both amino acids are lost from the tissue and glutamine levels fall while GABA levels are maintained or become elevated but increasingly at the expense of aspartic acid. A pRÉSUMÉd increase in interstitial glutamic acid concentration possibly in combination with subsequent failure of GABA inhibition appears the most plausible explanation for the increasing hyperexcitability during the development of feline generalized penicillin epilepsy.ZUSAMMENFASSUNGDie gleichzeitige A'nderung elektrophysiologischer und biochemischer Korrelate bei der sich entwic‐kelnden, generalisierten Penicillinepilepsie der Katze bedeutet möglicherweise ein Ursache‐Wirkungs‐verhältnis zwischen beiden Phänomenen. Nach i.m. Injektion von Penicillin im präepileptischen Zustand, vor dem Einsetzen von spike wave Entladungen, nimmt der kortikale Gehalt der Glutaminsäure ab. Diese Veränderung erfolgt zu einem Zeitpunkt, da eine erhöhte Amplitude der visuell evozierten Potential im Assoziationskortex das Auftreten von spike wave Aktivität ankündigt. Die Abnahme der Glutaminsäure und der Asparaginsäure läuft parallel mit einer fast stöchiometrischen Zunahme des Gluta‐mins und/oder der GABA, während die Taurinspiegel im präepileptischen Zustand praktisch normal bleiben. Wenn der präepileptische Zustand, durch general‐isierte 4 bis 5/s‐ spike Entladungen gekennzeichnet, in den epileptischen übergeht, scheint es zu einem Versagen des glialen Aufnahmemechanismus für Taurin und Glutamat zu kommen, da beide Aminosäuren aus dem Gewebe verloren gehen; der Glu‐taminspiegel fällt, während der GABA‐Spiegel un‐verändert bleibt oder steigt, was in zunehmendem Maße auf Kosten der Asparaginsäure passiert. Summary:A 24‐year‐old man with epilepsia generalisata secundaria had a frequency of 5.70 seizures daily and an EEG epileptic activity of 5–15 s/min. Auditory stimulation was administered contingent on epileptogenic activity during daily experimental periods. In comparison to control base‐line periods this first resulted in a reduction of seizure duration and thereafter in a reduction in seizure frequency. In a control experiment in which auditory stimulation was provided randomly there was no similar reduction of seizure duration or frequency. At the termination of the experiment the patient's seizure frequency had dropped to 2.66 per day.RESUMENUn hombre de 24 años de edad con epilepsía gen‐eralizada secundaria tenía una frecuencia de 5.70 ataques diarios y una actividad epiléptica electroencefalo‐gráfica de 5 a 15 segundos por minuto. Se llevó a cabo una estimulación auditiva, contingente con actividad epiléptica, durante periodos experimentales diarios. En comparación con periodos basales de control se observó una reducción de la duración de los ataques y, posteriormente, una disminución de su frecuencia. En un experimento control en el que la estimulación auditiva se realizó de modo randomizado no se observó una reducción semejante de la frecuencia o duración de los ataques.
