Summary:The regional distribution of phenytoin in the brains of experimental animals was studied using the indirect fluorescent antibody technique. Using antibodies directed against phenytoin, we observed a specific pattern of staining the Purkinje and granule cell layers of the cerebellum. An identical pattern of staining was seen in phenytoin‐treated and untreated animals. Several control procedures and studies indicated that the observed pattern was not caused by nonspecific staining or autofluorescence. The distinctive staining pattern obtained using this technique can be attributed to the high molecular specificity of antigen‐antibody reactions and a discrete binding site for phenytoin in the vicinity of cerebellar Purkinje and granule cells.RESUMENUtilizando la técnica de anticuerpos con fluorescencia indirecta se ha estudiado la distribución regional de la fenitoina en cerebros de animales experimentales. Usando anticuerpos anti‐fenitoina, hemos observado en un partón específico de teñido en las células de Purkinje y en la capa de células de gránulos del cerebelo. Un partón de teñido idéntico también se encontró en los animales que no habían recibido la droga. Varios estudios de control indican que el partón observado no fué producido por teñido inespecífico o autofluorescencia. Verschiedene Kontroll‐prozeduren und Untersuchungen zeigten, daß das beobachtete Muster nicht durch unspezifische Anfärbung oder Autofluoreszenz hervorgerufen wurde. Summary:The hypothesis was tested that epileptiform alterations of brain excitability may be elicited by a conditioning stimulus (CS) in a classical conditioning paradigm. Fifteen male albino rats were chronically implanted with electrodes in the amygdala and over the visual cortex. A train of six stroboscope flashes served as the CS and the unconditioned stimulus was the amygdaloid electrical stimulation, delivered together with the sixth flash in the course of the kindling procedure. Rats were randomly assigned to two groups. Rats in Group 1 were exposed to one or two kindling sessions per day, and those in Group 2 were exposed to 10 such sessions every day. All rats in Group 1 showed fully mature seizures within 15 ± 5.5 sessions, whereas none of those in Group 2 displayed kindled seizures. There was no evidence of amygdaloid after discharges or enhanced spiking as a response to the CS. A gradual selective suppression of the visual evoked potentials (VEP) secondary components, i.e., slow negative wave and sensory after discharge, paralleled kindling in Group 1. This effect was weak in Group 2. There was no change in the primary response of VEPs in either group. No recovery of VEPs followed the extinction of conditioning.RESUMENLa hipótesis de que las alteraciones epileptiformes de la excitabilidad cerebral pueden ser dectectadas mediante estímulos condicionantes (CS) se ha analizado utilizando un modelo clásico de condiciona‐miento. En 15 ratas albino machos se implantaron electrodos crónicos en la amígdala y sobre la corteza cerebral. Como CS se utilizeó un tren de seis destellos estroboscópicos mientras que la estimulación eléctrica de la amígdala sirvió de estímulo no condicionante, aplicándose simultáneamente a los seis destellos durante el proceso de la activación condicionada. Las ratas fueron divididas aleatoriamente en dos grupos. Fünfzehn mannliche Albinoratten wurden mit chronisch implantierten Elektroden im Amygdalon und über dem visuellen Cortex versehen. Eine Serie von 6 stroboskopischen Blitzen diente als CS während der unkonditionierte Stimulus (US) in der elektrischen Stimulierung des Amygdalons bestand, die zusammen mit dem 6. Blitz im Laufe der Kindlingprozedur verabfolgt wurde. Die Ratten wurden zufällig einer von zwei Gruppen zugeordnet. Ratten der Gruppe 1 wurden einem oder zwei Kindlingsitzungen pro Tag ausgesetzt, während diejenigen der Gruppe 2 10 derartigen Sitzungen pro Tag ausgesetzt wurden. Alle Ratten der Gruppe 1 zeigten voll ausgeprägte Anfälle innerhalb 15 ± 5,5 Sitzungen aber keines der Tiere der Gruppe 2 zeigte Anfälle durch Kindeln. Summary:Cimetidine, 300 mg p.o. four times a day, was administered for 5 days to nine epileptics who were stabilized while receiving phenytoin. Five patients had statistically significant increases in phenytoin serum levels, including two who became clinically toxic. One patient had a statistically significant decrease in phenytoin serum concentration. A relationship was not found between cimetidine levels and change in phenytoin serum levels. Cimetidine can cause significant changes in phenytoin serum levels which may be manifested clinically.RESUMENDurante 5 días se han administrado 300 mg cuatro veces diarias de cimetidina por vía oral a nueve epi‐lépticos estabilizados con un tratamiento de fenitoina. Cinco enfermos mostraron incrementos estadística‐mente significativos de niveles séricos de fenitoina in‐cluyendo dos que se intoxicaron clínicamente. Summary:Although several studies have reported on the risk of “early seizures” (seizures occurring within 7 days following a head injury), the reported proportions of patients experiencing these seizures vary from 1.4 to 15%. This wide divergence may be due to problems with methodology such as case selection and definitions of head injury and early seizures. In a series of 702 patients admitted with a head injury to Cook County Hospital (CCH), Chicago, Illinois, 29 (4.1%) had early seizures. This proportion is twice as high as one previously reported in a comparable series. Summary:Since 1963 a continuous registration of patients with epileptic seizures has been carried out in Greater Aarhus, Denmark. This registration has been attempted by means of medical records dating back to 1940. On the basis of the established prevalence and the calculated cumulated incidence, we have tried to describe the natural history of the different types of epilepsy. The prevalence of all types of epileptic seizures (including febrile convulsions) has been found to be 2,441/100,000, whereas for patients with the diagnosis of epilepsy the rate is 1,274/100,000. The study describes the natural history of the different types of epilepsy. In addition, the risk‐increasing effect of a febrile convulsive seizure on the development of epilepsy is analyzed.RESUMENDesde 1.963 se ha llevado a cabo un registro contínuo de enfermos con ataques epilépticos en la ciudad danesa de Greater Aarhus. Este registro se ha basado en protocolos médicos fechados desde 1.940. Summary:Fetal mousecerebralcortex in culture was exposed chronically to diazepam, and benzodiazepine (BDZ) receptor activity was assayed on the intact cell. Results have indicated: (1) minimal evidence of generalized drug toxicity, determined by choline acetyltransferase activity, tetanus labeling, high‐affinity uptakes of ‐γ‐aminobutyric acid and β‐alanine, protein, and light microscopy; (2) a significant dose‐dependent reduction (down‐regulation) of BDZ receptor activity that occurs after chronic exposure to clinically relevant concentrations of the drug; (3) a disproportionate suppression of the neuronal portion of the receptor after both acute and chronic exposure; and (4) recovery patterns of the BDZ receptor that differ substantially after acute versus chronic treatment. The latter finding may partially explain the observed clinical manifestations of drug tolerance and reduced anticonvulsant efficacy after prolonged use of this class of drugs.ZUSAMMENFASSUNGKulturen des cerebralen Kortex von Mäusefeten wurden im chronischen Versuch mit Diazepam behandelt; die Aktivität des Benzodiazepin‐Rezeptors (BDZ) wurde in der intakten Zelle bestimmt. Summary:A follow‐up study was made on 304 children (164 boys, 140 girls) with convulsive disorders, excluding occasional convulsions, in the first year of life. All patients except 45 who died were followed until 6 years of age or older. At the final follow‐up, the subjects were divided into six groups according to the degree of mental and physical development (groups I‐VI). Seizures were regarded as absent if the patient had been seizure free for more than 3 years. At the final follow‐up, seizures had ceased in 57.7%, and 43.4% had normal mental and physical development (group I). As to the initial diagnosis, the percentage of group I at the final follow‐up was 81.8% with febrile convulsions and 37.6% with epilepsy. In patients without seizures it was 69.7% with febrile convulsions and 55.8% with epilepsy. Some 80.6% of patients with unclassified generalized motor seizures, 11.5% of those with infantile spasms, 2.9% of those with secondary generalized epilepsy other than infantile spasms, 46.4% of those with partial seizures, and 25.0% of those with hemiconvulsive seizures were finally placed in group I. The percentage of patients without seizures was 81.4, 33.0, 34.4, 57.7, and 100%, respectively. As has been suggested, among the first‐year epilepsies, a subgroup with a more favorable prognosis may exist. Further studies regarding the etiology, ictal EEGs, and effectiveness of treatment and long‐term prognosis of these cryptogenic benign infantile convulsions are needed to provide a firm basis for understanding convulsive disorders in the first

Summary:The ability to measure serum levels of anticonvulsants has been a significant advance in the treatment of epilepsy. This technique enables practitioners to monitor a patient's plasma concentration, to detect potential toxicity, and to assess compliance with the prescribed regimen. A retrospective study of 164 adults with epilepsy was conducted to evaluate how serum anticonvulsant determinations were used by physicians in their treatment of epilepsy. Results indicate that the availability of test results did not improve the degree of seizure control, nor did it diminish patient reports of toxicity. In 17% of therapeutic decisions, prescribers did not appear to use the blood levels appropriately in their therapeutic decision‐making process. When physicians did appropriately utilize information from serum levels, the degree of seizure control improved significantly compared with when the prescribers did not use this information. The cost of determining serum levels of anticonvulsants is justified only if the information is appropriately utilized.RESUMENLa posibilidad de medir los niveles séricos de anticonvulsivanles ha demostrado ser un avance significative en el tratamiento de la epilepsyía. Esta técnica permite al clínico monitorizar la concentración plasmática de los pacientes para detectar la potencial toxicidad y verificar el cumplimiento del régimen prescrito. Para evaluar cómo las determinaciones séricas de anticonvulsives han sido utilizadas por los médicos en el tratamiento de la epilepsyía se ha realizado un estudio retrospectivo de 164 adultos con epilepsyía. Los resultados indican que la disponibilidad de los resultados no majoró el grado de control de los ataques ni disminuyó las quejas de toxicidad. En el 17% de las decisiones terapeúticas, los médicos no parecen haber usado los niveles séricos de modo apropiado en su proceso de tomar decisiones terapeúticas. Cuando los médicos utilizan correctamente esta información el grado de control de los ataques mejora significativamente compareándolo con la falta de utilización de estos datos. Eine retrospektive Unter‐suchung an 164 Erwachsenen mit Epilepsie wurde mit dem Ziel durchgeführt, zu erkennen, wie die Serum‐bestimmungen der Antikonvulsiva von den Ärzten in ihrer Behandlung der Epilepsie berücksichtigt werden. Summary:Seventy‐four patients with intractable seizures were followed up 6 to 57 months (mean 25 months) after intensive monitoring. At discharge from the hospital, 59% of the patients had at least a 50% reduction in seizure frequency and 63% had decreased antiepileptic drug toxicity. The findings at follow‐up compared with those at admission showed reduced seizure frequency in 55% of the patients, diminished medication toxicity in 54%, and improved social adjustment in 38%. A change in the seizure diagnosis was the best predictor of a favorable outcome. Patients with impaired mental status were not less likely to improve. The relationship between seizure type and outcome was not significant, although patients with complex partial seizures tended to be less likely to improve. Intensive monitoring can lead to significant long‐lasting improvement of patients with severe epilepsy.RESUMENDurante un periodo de 6 a 57 meses (25 de promedio) han sido seguidos 74 enfermos con ataques incontrolables sometidos a intensa monitorización. Al ser dados de alta, 59% mostraron. por lo menos, un 50% de reducción de la frecuencia de los ataques y en un 63% se habían reducido los signos de intoxicación medicamentosa. Summary:A patient with partial complex seizures evaluated for surgery with chronic depth electrode recordings demonstrated falsely localizing ictal onsets during anticonvulsant drug withdrawal. When phenytoin was being reduced, more seizures appeared to originate from the left temporal lobe than from the right. Right anterior temporal lobectomy, performed on the basis of other findings, revealed a small unsuspected tumor in the resected specimen, and the patient has remained seizure free for 3 years. The seizures that originated from the left temporal lobe were different from the patient's habitual attacks and appeared to be the result of anticonvulsant withdrawal and, perhaps, electrode irritation. Four other patients who received anterior temporal lobectomies at UCLA between 1977 and 1980 had at least one stereotaxic EEG (SEEG)‐recorded contralateral seizure onset, and all have benefited from surgery. Although multifocal SEEG‐recorded ictal onsets should be considered a poor prognostic sign, distant sites that give rise to atypical seizures during drug withdrawal may not generate spontaneous seizures postoperatively. Consequently, this finding should not be used as a sole criterion against the recommendation of surgical therapy.RESUMENUn paciente con ataques parciales complejos que fueron evaluados para posible cirugía con registros mediante electrodos profundos demostraron comienzos ictales de falsa localizacidn durante la retirada de la medicación anticonvulsiva. Cuando se reduceía la fenitoina, más ataques parecían originarse en el lóbulo temporal izquierdo que en el derecho. La lobectomía temporal anterior del lado derecho, realizada en base a otros hallazgos, reveló, insospechadamente, un tumor en el fragmento resecado y el enfermo ha permanecido sin ataques durante tres años. Los ataques que se originaban en el lóbulo temporal izquierdo eran distintos de los ataques habituates y parecían ser el resultado de la retirada de los anticonvulsivantes y, quizás, de la irritación de los electrodos. Cuatro de las pacientes sometidos a una lobectomía en UCLA entre 1.977 y 1.980 tuvieron, por lo menos, un ataque de comienzo contralateral registrado en el SEEG y todos se beneficiaron de la cirugía. A pesar de que los comienzos multifocals segün el SEEG deberían considerarse como signo de pobre pronóstico, lugares distantes que producen ataques atípicos durante la retirada de las drogas pueden no generar. post‐operativamente, ataques espontáneos. Por consiguiente este hallazgo no debe usarse como criterio único contra la recomendación de terapía quirúrgica.ZUSAMMENFASSUNGEin Patient mit komplexen Partialanfällen, bei dem wegen eines chirurgischen Eingriffs über chronisch implantierte Tiefenelektroden abgeleitet wurde, zeigte eine falsche Lokalisation des Anfallsbeginns während des Entzugs der Antikonvulsiva. Als das Phenytoin reduziert wurde, schienen mehr Anfälle ihren Ursprung vom linken Temporallappen zu nehmen als vom rechten. Eine rechtsseitige vordere temporale Lobek‐tomie, die aufgrund anderer Befunde durchgeführt wurde, zeigte einen kleinen unerwarteten Tumor im resezierten Gewebe und der Patient blieb 3 Jahre lang anfallsfrei. Die Anfälle, die ihren Ursprung im linken Temporallappen hatten, unterschieden sich von den üblichen Attacken des Patienten und schienen das Ergebnis des Entzugs der Antikonvulsiva zu sein und vielleicht auf einer Irritation durch die Elektrode zu beruhen. Vierandere Patienten, die zwischen 1977 und 1980 in der UCLA am vorderen Temporallappen lo‐bektomiert wurden, hatten wenigstens ein SEEG mit kontralateralem Anfallsbeginn und alien wurde durch den chirurgischen Eingriff geholfen. Obwohl multi‐fokale SEEG‐Aufzeichnungen des Anfallsbeginns als schwacher prognostischer Hinweis betrachtet werden sollten, kann angenommen werden, daß entfernte Herde. die Anlass für atypische Anfälle während Entzugs der Medikamente bilden könnten, nicht Anlass für spontane Anfälle postoperativ geben müssen. Damit sollte ein solcher Befund nicht als einziges Kriterium gegen die
