SUMMARYFive clinical cases are presented which clearly support the contention that epilepsy can arise in the mesencephalon and thalamus of man, either spontaneously or as a result of stimulation. These discharges may be associated with motor, sensory, or behavioral changes. In one instance (Case 6), general “fits” occurred in a patient with a brain tumor involving the mesencephalon‐diencephalon. These cases are discussed as they relate to the further research in epilepsy.RÉSUMÉCinq cas cliniques sont présentés qui supportent clairement le point de vue suivant lequel une épilepsie peut être engendrée dans le mésencéphale ou le thalamus de ľhomme, soit spontanément, soit comme résultat ďune stimulation. Les décharges de cette épilepsie peuvent etre associées à des manifestations motrices, sensorielles ou intéressant le comportement. SUMMARYThe genesis of independent secondary epileptic foci from artificially induced primary epileptic lesions has been demonstrated in animals which are representative of different stages of evolutionary development. These data suggest that the length of time necessary for the occurrence of an autonomously discharging secondary epileptic focus following the artificial creation of a primary epileptic lesion varies directly with the differentiation and organizational complexity of the forebrain of the animal selected as representative of the particular stage of evolutionary development.The data also indicate that secondary epileptogenesis results from an alteration of normal physiological processes and that the propensity for epileptogenesis is inherent in vertebrate neural tissue. The data presented are discussed in relation to the question of secondary epileptogenesis in humans with focal epilepsy.To the authors' knowledge this study is the first which has used the opossum and cayman as models for the study of epileptogenesis. SUMMARY1)Two cats with epileptogenic lesions of the right fornix developed seizures and status epilepticus. After a single injection of parenteral Nembutal®, seizure activity ceased and did not recur. These animals subsequently behaved essentially normally, and their EEG's showed no significant epileptic alterations.2)Histological examination revealed epileptogenic lesions in the right fornix with degeneration of the ipsilateral limbic system.3)Possible pathophysiologic mechanisms underlying the spontaneous cure of chronic epilepsy are discussed. It appears likely that intense metabolic activity in epileptic structures during status epilepticus led to hypoxic necrosis of the hemi‐limbic system.RÉSUMÉ1)Les lésions épileptogàenes du fornix droit ont entraîne chez deux chats des crises et un état de mal epileptique. SUMMARYIn “encáephale isole” preparations eye movements and depression of the electromyo‐gram of neck muscles were recorded during interclonic electrical silences and coinciding with the arrest of the seizures elicited by electrocortical stimulation.In the intact curarized animal the interclonic periods of electrical silence and arrest of seizure discharges were accompanied by a low voltage fast activity in dorsal limbic structures, pontine, and mesencephalic nuclei. At the end of the seizure, a remarkable depression of the electrically‐evoked spinal monosynaptic reflex was observed; by partial sectioning of the brain stem this inhibitory action was demonstrated to be mediated by extrapyramidal pathways.The hypothesis is proposed that paradoxical sleep and postepileptic extinction are similar phenomena in which cortical synchronism determines the putting into action of the same central process.RÉSUMÉChez les préparations àa “encéphale isolé”, des mouvements oculaires et une dépression du tonus des muscles nuquaux sont enregistrés durant les silences électriques inter‐cloniques et pendant le silence électrique qui suit la fin des crises produites par une stimulation électrocorticale.Chez ľanimal intact et curarisé, ces mêmes périodes de silence électrique inter‐clonique et post‐critique s'accompagnent ďune activité rapide et de faible intensité dans les structures limbiques dorsales, pontines et mésencéphaliques. SUMMARYThe authors studied the effects of injections of small and large doses of insulin administered to cats in which an epileptic focus had been produced by cortical application of penicillin or intracortical injection of tungsten paste. The following results were obtained.1There was no effect on the existing epileptic focus when the insulin dosage was low and caused little change in blood sugar and background activities of the EEG;2The focal epileptic activity was attenuated when the dosage was higher and gave rise to slow waves in the EEG;3The focal epileptic activity was enhanced when the dosage was still higher and produced diffuse irritative activity in the EEG. In that case the focal epileptic activity was gradually accentuated before spreading throughout the hemisphere, and then throughout both hemispheres, and giving rise to partial seizures with secondary generalization, followed by electrical silence in the hemisphere involved.These findings demonstrate that large doses of drugs which have no local epileptogenic action, can lower the convulsive threshold and produce, either a generalized seizure of sudden onset in a normal brain or a partial seizure with secondary generalization in a brain which carries an epileptic focus.RÉSUMÉLes auteurs se sont intéressés a Taction de ľinsuline injectée chez ľanimal àa faible et a forte concentration, en particulier chez le chat porteur ďun foyer épileptique provo‐qué par application corticale de pénicilline ou par injection intra‐corticale de pâte au tungstàene. SUMMARYThis study was focused on the intravenous injection of ammonium chloride in man and in animals.Both in man and in animals, rapid injection of a dose of 3–8 mg/kg body weight produced a burst of symmetrical, bilaterally synchronous slow waves without any significant change in heart rate, arterial pressure or respiration.Larger doses produced the same slow waves in the EEG, followed by a flattening of the EEG associated with clinical evidence of convulsive movements.Cats given even larger doses (up to 100 mg/kg) showed the abovementioned EEG changes followed by generalized discharges of spikes, spike‐and‐waves, multiple spikes and multiple‐spikes‐and‐waves, either isolated or in bursts, often in association with massive myoclonic jerks identical to those produced by pentetrazol injection and some‐ times leading to a grand ma1 seizure.Direct application of ammonium chloride to the cortex, however, never caused epileptic manifestations but merely produced lasting local suppression of electrogenesis.Les auteurs se sont particuliàerement intëressës A ľinjection intra‐veineuse de chlorure ďammonium chez ľhomme et chez ľanimal.The study thus disclosed two different effects of ammonium chloride: effects rem‐ iniscent of transient cerebral anoxia (small doses), and generalized epileptic mani‐ festations (doses larger than are ever used in man).RésuméĽinjection rapide ďune dose comprise entre 3 et 8 mg/kg provoque chez ľhomme et chez ľanimal une bouffée ďondes lentes bilathales, synchrones et symëtriques sans modifications importantes du rythme cardiaque, de la tension artérielle et de la respiration.Des quantités plus importantes provoquent les mêmes anomalies lentes électroencéphalographiques suivies ďun aplatissement des tracés accompagés, du point de vue clinique, de mouvements convulsifs.A des doses beaucoup plus fortes allant jusqu'àa 100 mg/kg chez ľanimal (chat) ľaplatissement observé précedemment est suivi de décharges généralisées de pointes, pointes ondes, polypointes et polypointes ondes soit isolées, soit groupées en bouffées souvent associées àa des myoclonies massives identiques àa celles produites par injection de cardiazol et susceptibles ďaboutir àa une crise grand mal.Par contre ľapplication directe du chlorure ďammonium sur le cortex n'a jamais provoqué de manifestations épileptiques mais seulement une dépression locale et durable de ľélectrogénàese.Les auteurs ont donc mis en évidence deux effets diffirénts du chlorure ďammonium: àa faible dose, des effets trés évocateurs ďune anoxie cérébrale transitoire; àa forte

SUMMARYThe effects of phenobarbitone on free amino acid changes induced by an epileptogenic focus caused by topical application of mescaline to the neocortex of the cat have been investigated.Rapid injection of phenobarbitone into the carotid artery after ligation of the common stem of the submaxillary and thyroid arteries suppressed the effect of a mescaline focus on alanine and GABA but not that on glutamic acid.When the epileptogenic focus was induced in cats previously treated with phenobarbitone by i.m. injections on 4 consecutive days the glutamic acid level no longer decreased but alanine, arginine and GABA values were still low.These alterations occurred throughout the whole neocortex.Comparison of ECoG and neurochemical findings showed a concordance between the electrical alterations and the glutamic acid level in the neocortex.The results indicate that one of the mechanisms of the action of phenobarbitone on the evolution of the focus appears to be an alteration in cortical excitability, manifested especially by preventing a decrease of the glutamic acid level.RÉSUMÉOn a étudié les effets de la phàenobarbitone sur les variations des acides aminés libres provoquées par un foyer épileptogàene du àa ľapplication locale de mescaline au néocortex du chat.Ľinjection rapide de phénobarbitone dans ľartàere carotide apràes ligature du tronc commun des artàeres sous‐maxillaire et thyrodienne ont suppriméľeffet du foyer de mescaline sur ľalanine et GAB A mais non sur ľacide glutamique.Lorsque le foyer était crée chez des chats préalablement traités, pendant 4 jours consécutifs, par la phénobarbitone en injections i.m., le taux ďacide glutamique ne s'abaissait plus, mais les chiffres ďalanine, ďarginine et de GABA restaient faibles.Ces variations se produisaient dans tout ľensemble du néocortex.La comparaison de 1'ECoG et des observations neurochimiques a fait ressortir une concordance entre les modifications électriques et le taux ďacide glutamique dans le néocortex.Les résultats paraissent indiquer que Fun des mécanismes ďaction de la phénobarbitone sur ľévolution du foyer est un changement de ľexcitabilité corticale, dont la manifestation principale est de s'o

SUMMARYTo test the hypothesis that in the isolated cortex of the cat the positive burst response (PBR) is a component of normal cortical activity and that the epileptiform afterdis‐charge (EAD) represents epileptic activity, experiments were carried out to see whether antiepileptic drugs would suppress differentially the EAD without suppressing the PBR. The results are as follows:1Diphenylhydantoin, phenobarbital, procaine and cyheptamide selectively suppressed the threshold for the production of the EAD and/or the duration of the epileptiform sustaining response (ESR) without suppressing the threshold for the PBR.2Trimethadione and compound “5” in appropriate doses suppressed only the ESR.3Thiopental suppressed the PBR without suppressing the EAD.These results suggest that the responses of the acutely isolated cortex of the cat provide a suitable test system for the screening of antiepileptic agents clinically effective in the treatment of grand mal epilepsy.4In chronic slabs the threshold for the EAD was more resistant to elevation by all drugs than was the threshold for the EAD in the acute slab.5The duration of the EAD in chronic slabs was more resistant to the effects of antiepileptic drugs than was the duration of the combined EAD/ESR in acute slabs.This suggests that the prolonged responses in chronic and acute slabs occur by different mechanisms.RÉSUMÉAnn de déterminer si dans le cortex isolé du chat, les réponses sous forme de bouffées positives (R.B.P.) sont des composantes de ľactivité corticale normale qui doivent être différenciées des post décharges épileptiques (P.D.E.), on a cherchéàa savoir si les drogues anti‐épileptiques suppriment les secondes sans modifier les premiàeres. Les résultats ont été les suivants:1La diphénylhydantoine, le phénobarbital, la procaine et la cyheptamide suppriment sélectivement le seuil pour la production de la P.D.E. ou la durée de la réponse épileptique soutenue (R.E.S.) sans supprimer le seuil pour le R.B.P.2La triméthadione et le composé No. 5, àa des doses appropriées, suppriment seulement la R.E.S.3Le thiopental supprime la R.B.P. sans supprimer la P.D.E.Ces résultats suggàe1rent que les réponses du cortex isolé de faαcon aigue constituent un test utile pour apprécier les substances anti‐épileptiques cliniquement efficaces pour traiter les crises épileptiques de grand mal.4Dans les lambeaux corticaux chroniques le seuil pour la P.D.E. résiste ďavantage àa ľélévation sous ľeffet de toutes les drogues.5La durée de la P.D.E. dans les lambeaux chroniques est plus résistante aux effets des drogues anti‐épileptiques que la durée des réponses combinées P.D.E./R.E.S. dans les lambeaux aigus.Cela suggàere que les réponses prolongees répondent
