Summary:The comparative antiepileptic effects of the thyrotropin‐releasing hormone and its new derivative, γ‐butyrolactone‐γ‐carbonyl‐l‐histidyl‐l‐prolinamide citrate (DN‐1417), were examined in feline amygdaloid kindling preparations. When tested in kindled animals, both agents showed a transient and non‐dose‐dependent anticonvulsant effect, in addition to raising the generalized convulsive seizure‐triggering threshold for a prolonged period of time. Furthermore, DN‐1417 exerted a significant effect on postictal events: shortening the electro‐graphic silence, leaving the duration of slow activity intact, while prolonging the period of seizure refractoriness. When tested in kindling animals, DN‐1417 showed a significant but non‐dose‐dependent prophylactic effect on both primary and secondary amygdaloid sites.RESUMENEn preparaciones de amígdalas condicionadas (kindling) se han comprado los efectos antiepilépticos de la hormona liberadora de tirotropina y de su derivado el citrato γ‐butirolactona‐γ‐carbonil‐l‐histidil‐l‐prolinamida (DN‐1417). Summary:A recycling trial consisting of six amygdaloid stimuli at 60‐min intervals was applied to evaluate postictal seizure inhibition (PSI). Cats kindled unilaterally and bilaterally from the amygdala were used. The results obtained may be summarized as follows: (1) kindled generalized convulsions resulted in three consecutive postictal stages before recovery from postictal behavioral depression; (2) in bilaterally kindled cats, PSI lasted for 3–4 h, which was longer than the postictal behavioral depression; (3) no correlation was found between interictal spike frequency and decreased susceptibility to convulsion during PSI; (4) the duration of PSI was prolonged significantly after secondary site amygdaloid kindling; and (5) such changes were observed>2 weeks after the last secondary site stimulation. It is suggested that the intensity of PSI is altered by the establishment of secondary epileptogenesis in the contralateral hemisphere, which exerts a long‐lasting seizure inhibitory effect on the primary epileptogenic focus.RESUMENPara valorar la inhibición postictal de los ataques (PSI) se ha aplicado un ensayo consistente en un reciclaje de 6 estímulos de la amígdala de 60 minutos de intervalo. Se utilizaron gatos condicionados (kindled) unilateral y bilateralmente desde la amígdala. Los resultados que se obtuvieron pueden sumarizarse como siguen: 1) se observaron convulsiones generalizadas condicionadas (kindled) en 3 estados postictales consecutivos antes de que se recobrara la depresión del comportamiento postictal; 2) en los gatos condicionados (kindled) bilateralmente, la PSI duré entre 3 y 4 horas, tiempo más prolongado que la depresión postictal del comportamiento; 3) no se encontró ninguna correlación entre la frecuencia de las puntas y una susceptibilidad reducida para las convulsiones durante la PSI; 4) la duración de la inhibición postictal de los ataques se prolongó significativamente después del condicionamiento (kindling) del lugar secundario de la amígdala; y 5) estos cambios se observaron más de 2 semanas después de la última estimulación en el lugar secundario. Summary:Piperonyl butoxide, a microsomal monooxygenase inhibitor, administered intraperitoneally to mice exerts peak anti‐maximal electroshock activity and peak neurotoxicity at 5 and 7 h, respectively. The median neurotoxic dose is 1,690 mg/kg. In the maximal electroshock seizure test, the median effective dose (ED50) is 457 mg/kg and the protective index (PI) is 3.69. In the subcutaneous pentylenetetrazol test, the ED50is 443 mg/kg and the PI is 3.81. Piperonyl butoxide prevents seizure spread and elevates seizure threshold. Summary:The metabolism of sodium phenytoin (PHT) in the cat was studied by gas‐liquid chromatographic analyses of the drug and its metabolites extracted from plasma, urine, and feces during a 5‐month course of daily oral administration. Plasma PHT levels of 15–18 μg/ml were observed. Urine contained 5‐(4‐hydroxyphenyl)‐5‐phenylhydantoin, 5‐(3,4‐dihydroxy‐1,5‐cyclohexadien‐1‐yl)‐5‐phenylhydantoin, 1‐deoxy‐1‐(5,5‐diphenylhydantoin‐3‐yl)‐β‐d‐glucopyranuronate, and unmetabolized PHT. Large quantities of PHT (32–63% of the daily dose) were observed in feces. We suggest that gastrointestinal malabsorption, extensiveN‐glucuronidation, and limited hydroxylation determine the fate of orally administered PHT in the cat. Summary:The subcellular distribution of amino acids was compared in brains of genetically seizure‐susceptible (SS) and genetically seizure‐resistant (SR) rats. The total taurine content (μmol/brain) in the P2B, or synaptosomal, fraction in SS rats was only 37% of that of SR rats. Glutamate, glutamine, glycine, alanine, and γ‐aminobutyric acid (GABA) contents were unaltered. No alterations in total content were found in other subcellular fractions for the amino acids studied. SS animals that had never been stimulated to audiogenic seizure had decreased concentrations of taurine (nmol/mg protein) in the P2, P2B, and P2C fractions as compared with SR animals. These fractions contain crude synaptosomes, enriched synaptosomes, and enriched mitochondria, respectively. Phosphoethanolamine concentrations were also decreased in the P2B fractions, but concentrations of other amino acids were unaltered, as compared with SR animals. Twenty‐four hours after the intracerebroventricular injection of taurine (6 μmol) in SS animals that had never been convulsed, taurine concentrations were significantly increased in whole brain homogenate and P2and P2B fractions as compared with SS animals not given taurine. This treatment left unaltered the concentrations of glutamate, glutamine, GABA, and glycine in brain homogenate and P2fraction. Because decreases in taurine concentration were seen in animals that had not been convulsed, these alterations are intrinsic to the SS strain and are not a consequence of convulsive activity. In view of the anti‐epileptic action of taurine, and the fact that an impairment of taurine transport in the brain of SS rats has previously been demonstrated, we suggest that a defect in the biochemistry of taurine is partially responsible for the seizure susceptibility of the SS rat.RESUMENLa distribución subcelular de aminoácidos ha sido comparada entre los cerebros de ratas genéticamente suceptibles a ataques (SS) y las genéticamente resistentes (SR). Summary:The effect of asphyxia on seizures was determined in neonatal dogs. In normoxic (paralyzed and ventilated) neonatal dogs, bicuculline‐induced seizures produced significant elevations of arterial blood pressure, Po2, glucose, lactate, and epinephrine. Cerebral blood flow increased severalfold; brain glucose, adenosine triphosphate (ATP), and phosphocreatine (PCr) did not decrease significantly. In contrast, seizures during asphyxia were associated with hypoxia, hypotension, hypercarbia, and acidosis. Significant cerebral ischemia developed. Brain glucose, ATP, and PCr were significantly depleted. Complete oxygen deprivation during neonatal seizures exhausts brain energy stores, which leads to cessation of seizure activity.RESUMENSe ha estudiado el efecto de la asfixia en perros recién nacidos. En perros normóxicos (paralizados y ventilados) recien nacidos, los ataques inducidos por bicuculina produjeron elevaciones significativas de la presión arterial, pO2, glucosa, lactato, y epinefrina. Summary:Systemic kainic acid (KA) at 1 mg/kg crosses the normal blood–brain barrier (BBB) of rats to produce scattered sharp waves for ˜ 10 min on the EEG, but is without observable effect, producing no EEG or behavioral changes, at 0.5 mg/kg. When the BBB is opened in most of one hemisphere by 6,000 rads, the rats are clinically normal and have a normal EEG. When the irradiated rats are challenged with 0.5 mg/kg KA, intense, highly lateralized epileptiform bursts result; these bursts last 1–10 s and recur for 90 mins. This phenomenon is repeatable for the 1 week that the BBB is open. The results are identical when the BBB is opened with a pin lesion. Because endogenous glutamate agonists are common, these data suggest a possible pathophysiologic mechanism for human epilepsy.RESUMENEl ácido kaínico (KA) cruza la barrera hematoencefálica (BBB) de las ratas en dosis de 1 mg/kg y produce ondas agudas dispersas durante unos 10 minutos en el EEG pero sin efectos observables; no produce efectos en el EEG, o en el comportamiento, con dosis de 0,5 mg/kg. Summary:The pharmacokinetic characteristics of progabide, a new γ‐aminobutyric acid‐mimetic drug, were evaluated following single‐ and multiple‐dose administration of a progabide suspension through a chronic intraperitoneal catheter. Several issues pertaining to the bioavailability of progabide were addressed: first‐pass effect, incomplete dissolution, and dose and time dependency. Four male monkeys received five treatments: three intraperitoneal doses (50, 100, and 150 mg), one oral dose (50 mg), and one intravenous bolus dose (150 mg). Bioavailability following intraperitoneal administration was incomplete, consistent with a first‐pass effect predicted from intravenous data. There were no significant differences between the absolute bioavailabilities of the three intraperitoneal doses, which ranged between 40 and 49%. The apparent half‐life (t1/2) observed after intraperitoneal administration was significantly longer than the elimination half‐life by the intravenous route and tended to increase with dose. This behavior is consistent with dissolution rate‐limited absorption. The bioavailability of the suspension administered orally was compared with that of the intraperitoneal route, and no difference was found. However, the apparent t1/2by the oral route was significantly longer than that of the intraperitoneal route. In multiple‐dosing studies, four different dosing regimens (all intraperitoneal) were examined, including 50 mg every 2 or every 6 h, 20 mg every 2 h, and 40 mg every 4 h. In all these regimens, plasma levels exhibited an accumulation compared with single‐dose predictions. Large oscillations in plasma levels were observed when the dosing interval was 6 h and side effects were observed when the dosing interval was 2 h. A dosing regimen of 40 mg every 4 h was optimal. The pharmacokinetics of valproic acid after intraperitoneal administration were also examined. Verschiedene Modalitäten der Bioverfügbarkeit des Progabids wurden untersucht: First‐pass Wirkung, inkomplette Lösung, Dosisund Zeitabhängigkeit. Four männliche Affen erhielten 5 Behandlungen: 3 ip‐Dosen (50, 100 und 150 mg), eine orale Dosis (50 mg) und eine intravenöse (iv) Bolusdosis von 150 mg. Die Bioverfügbarkeit nach ip Gabe entsprach teilweise einem first pass‐Effekt wie er von den iv Daten vorhersagbar war. Es existierten keine signifikanten Unterschiede zwischen der absoluten Bioverfügbarkeit der 3 ip Dosen, die sich zwischen 40 und 90% bewegte. Die offensichliche Halbwertszeit (t1/2) nach ip Gabe war signifikant länger als die Eliminationshalb‐wertszeit nach i.v. Gabe; sie tendierten mit steigemder Dosis zuzunehmen. Dieses Verhalten ist zu vereinbaren mit einer durch die Löslichkeitsrate beschränkten Absorption. Die Bioverfugbarkeit der oral verabfolgten Suspension wurde mit der ip Route verglichen; es fanden sich keine Unterschiede. Jedoch war die t1/2bei oraler Verabfolgung signifikant länger als bei ip‐Gabe. In Untersuchungen mit mehreren Dosen wurden 4 verschiedene Dosierungsschemata (alle ip) geprüft; darunter waren 50 mg alle 2 oder alle 6 Stunden, 20 mg alle 2 Stunden und 40 mg alle 4 Stunden. In all diesen Versuchen zeigten Plasmaspiegel verglichen mit der Gabe einzelner Dosen eine Akkumulation. Große Schwankungen in den Plasmaspiegeln wurden beobachtet, wenn die Dosisintervalle 6 Stunden betrugen und Nebenwirkungen werden bei Dosierungsintervallen von 2 Stunden beobachtet. Optimal schien ein Dosierungsschema von 40 mg alle 4 Stunden zu sein. Die pharmakokinetischen Eigenschaften von der Valproinsäure (VPA) nach ip Gabe wurden ebenfalls geprüft. VPA zeigte komplette Bioverfügbarkeit (93% als mittlerer Wert) wenn ip verabfolgt und keine Akkumulationen bei multiplen Dosen.RESUMENLas características farmacocinéticas del progabide, una nue va droga ácido gammaaminobutírico‐mimética, han sido evaluadas después de la administración de una ünica y múltiples dosis de una suspension de progabide por un cateter intraperitoneal (i.p.) crónico. Se plantearon varios aspectos relacionados con la biodisponibilidad del progabide: efecto de primer‐paso, disolución incompleta, dependencia de la dosis y tiempo. Cuatro monos machos recibieron cinco tratamientos: tres i.p. dosis (50,100 y 150 mg), una dosis oral (50 mg) y un bolo intravenoso (i.v.) de 150 mg. La biodisponibilidad tras la administración i.p., fue incompleta, consistente con un efecto de primer‐paso como se deducía de la imformación i.v. No se observaron diferencias significativas entre las biodisponibilidades absolutas de las tres dosis i.p., que variaron entre el 40 y el 49%. La vida media aparente (t1/2) observada después de la administración i.p. fue significativamente más larga que la vida media de la eliminación por via intravenosa y mostraba una tendencia a incrementarse con la dosis. Summary:Twenty therapy‐resistant epileptic patients entered a double‐blind, randomized, two‐period, crossover trial comparing progabide (19.3–36 mg/kg/day) and placebo as add‐on drugs to standard therapy. Each period lasted 6 weeks with a gradual crossover during 4 days. Five patients were dropped because of reasons unrelated to treatment. Among the 15 patients who completed the study, seven had partial, six primary generalized, and two secondary generalized epilepsies. Preexisting antiepileptic drugs (AEDs) ranging from one to three per patient (mean 2.2 AEDs/patient) were maintained unchanged during the trial. Efficacy was assessed biweekly by means of total seizure counts, counts of each seizure type, and global clinical judgment. At the same time intervals, safety was assessed by means of reports of adverse events, clinical and neurological examination, laboratory tests, and measurement of plasma concentrations of progabide and associated AEDs. According to the clinical global judgment, eight patients were considered improved during progabide treatment. Seizures were reduced in 14 of 15 patients during the progabide as compared with the placebo period. During the progabide period, the reduction of the total seizure count was 45 and 58% in two patients and 88–97% in six patients. A significant reduction of the total seizure number was observed in the progabide period as compared with the placebo period, both in the whole patient group (p<0.01) and in the two subgroups of patients with generalized (p<0.01) and partial (p<0.05) epilepsies. Absences, major generalized, and partial seizures (including partial seizures with secondary generalization) were significantly reduced on progabide as compared with placebo (p<0.05). No drop‐outs owing to adverse effects were observed. In one case, somnolence required a reduction of the progabide dose. Somnolence (three cases), nausea or vomiting (two cases), and dysarthria (one case) were observed in six other patients on progabide but were judged to be mild. Laboratory tests did not show any treatment‐related alterations. Plasma concentrations of progabide measured before the morning dose ranged from<50 to 1,474 ng/ml. No correlation was found between plasma concentrations and the clinical outcome. In conclusion, results from this study indicate that progabide is well tolerated and possesses a promising antiepileptic activity in both partial and generalized epilepsies.RESUMENVeinte enfermos, resistentes a la terapeútica, han sido incluidos en un ensayo doble ciego, randomizado y cruzado (cros‐over) en dos periodos, para comparar progabide (19,3 a 36 mg/kg/dia) y placebo añadidos a la terapia habitual. Cada periodo duró 6 semanas con un periodo gradual de intercambio, de 4 dias. Cinco enfermos abandonaron el ensayo por razones no relacionadas con el tratamiento; de los 15 enfermos que completaron el estudio, 7 tenían epitepsía partial, 6 primaria generalizada y 2 con generalización secundaria. Las medicaciones antiepilépticas pre‐existentes, variando entre 1 y 3 por caso, (promedio de 2,2 por enfermo) no se modificaron durante el ensayo. La eficacia se determinó cada dos semanas mediante el contaje del número total de los ataques, de cada tipo de ataque y el juicio clínico global. Con la misma periodicidad, se analizo la seguridad de la medicación registrando los efectos adversos, la exploración clínica y neurológica, los test de laboratorio, y la determination de la concentraciónes plasmáticas de progabide y medicaciones antiepilépticas asociadas. Según el juicio clínico global, 8 enfermos mejoraron durante el tratamiento con progabide; los ataques se redujeron en 14 de 15 enfermos con progabide comparándolo con placebo; durante el periodo de tratamiento con progabide el número total de ataques fue de 45 a 58% en dos enfermos y de 88 a 97% en 6 casos. Se ha observado una reducción significativa del número total de ataques durante el periodo de progabide, comparandolo con placebo, tanto en el grupo total de enfermos (p<0.01) como en los dos sub‐grupos de ataques generalizados (p<0.01) y de epilepsías parciales (p<0.05). No han ocurrido abandonos debido a la medicación; en un caso la somnolencia obligó a reducir la dosis de progabide. Somnolencia (3 casos), nauseas y vómitos (2 casos), y disartria (1 caso) se observaron en otros 6 enfermos en progabide pero se consideraron como moderados. Los estudios de laboratorio no mostraron alteraciones relacionadas con el tratamiento. Las concentraciones plasmáticas de progabide, medidas antes de la dosis matinal, oscilaron entre 50 y 1474 ng/ml sin que se encontrasen correlaciones entre los niveles plasmáticos y los resultados terapeúticos. En conclusion, los resultados de este estudio indican que el progabide es bien tolerado y posee una actividad antiepiléptica prometedora sobre las epilepsías parciales y generalizadas.ZUSAMMENFASSUNGTherapieresistente epilepstische Patienten (20) wurden in einen Doppleblindversuch aufgenommen. Das Design bestand in einer zufällig zugeordneten 2‐Perioden cross‐over Untersuchung, in der Progabid (19,3 bis 36mg/kg/Tag) und Placebo als zusätzliches Medikament zur Standardtherapie verglichen wurden. Jede Periode dauerte 6 Wochen mit allmählicher Übergangsphase während 4 Tagen. Five Patienten schieden aus Gründen aus, die mit der Behandlung nicht zusammenhängen. Unter den 15 Patienten, die Studie vollendeten, hatten 7 Partial‐6 primär generalisierte und 2 sekundär generalisierte Epilepsien. Die bestehende antiepileptische Medikation rimfaβte 1 bis 3 Medikamente pro Patient: (im Mittel 2,2 antiepileptische Medikamente pro Patient). Sie wurde während der Untersuchung unverändert beibehalten. Die Wirksamkeit der Medikation wurde zweimal wöchentlich durch Anfallszählung festgestellt. Dabei wurden Anfälle jeden Typs separat gezählt und eine globale klinische Beurteilung abgegeben. In dieser Zeit wurden Nebenwirkungen notiert; klinische, neurologische Untersuchungen und Labortests fanden statt. Die Plasmaspiegel‐konzentration von Progabid und den anderen Antiepileptika wurden gemessen. Nach der globalen klinischen Einschätzung besserten sich 8 Patienten während der Progabid‐Behandlung; die Anfallsanzahl wurde bei 14 von 15 Patienten während Progabid verglichen mit der Placebo‐Periode reduziert; die Reduktion der Anzahl aller Anfälle während der Progabid‐Periode betrug 45 und 58% bei 2 Patienten und lag zwischen 88 und 97% bei 6 Patienten. Summary:Studies from several countries have reported that fewer people with epilepsy marry, but the reasons for this are not clear. In the present investigation, 178 adults with epilepsy were divided into married, separated/divorced, and never married groups. The never married group was subdivided into persons living in dependent versus independent settings. The groups did not differ with respect to seizure type, age at onset of seizures, and related variables. All were administered a complete battery of neuropsychological tests and the Minnesota Multiphasic Personality Inventory. On all tests of abilities, the married and the never married independent groups had the best scores, the separated/divorced group was intermediate, and the never married dependent group had by far the lowest scores. In the emotional area, however, trie separated/divorced group had the poorest scores and the other groups were indistinguishable from each other. It appears that marital status is more related to emotional adjustment than to mental abilities, and that independent living skills are more related to mental abilities than to emotional adjustment.RESUMENEstudios realizados en diversos paises muestran que un número menor de epilépticos se casan pero las razones permanecen oscuras. En esta investigación, 178 adultos epilépticos se dividieron en casados, separados/divorciados, y solteros. El grupo do solteros fue subdividido en personas que vivian en ambiente dependiente o ambiente independiente. Los grupos no se diferenciaban con respecto al tipo de ataques, edad de comienzo de los ataques y variables relacionadas. A todos se les administró una batería completa de tests neuropsicológicos y el MMPI. Los grupos de casados y solteros independientes obtuvieron mejores puntuaciones en todos los tests de habilidades; el grupo de separados/divorciados consiguió resultados intermedios y el de solteros dependientes obtuvieron, sin duda las puntuaciones mas bajas. Sin embargo, en el área emocional el grupo de separados/divorciados consiguió las peores puntuaciones, y los otros grupos no se distinguieron entre sí. In dieser Untersuchung wurden 178 Erwachsene mit Epilepsie einer der folgenden Gruppen Zugeordnet: verheiratet, getrennt/geschieden, niemals verheiratet gewesen. Die letzte Gruppe wurde weiter unterteilt in Personen, die in Abhängigkeit von ihrer Umgebung und solche, die unabhängig von ihr leben. Die Gruppen unterschieden sich nicht hinsichtlich des Anfallstyps, des Alters bieim Beginn der Anfälle und daruf zu beziehende Variablen. Aile wurden mit einer kompletten neuropsychologischen Testbatterie und dem MMPI untersucht. In alien den Tests zeigte die verheiratete Gruppe und die niemals verheiratet gewesene, unabhängig lebende Gruppe die besten Ergebnisse. Die getrennt/geschieden lebende Gruppe lag in der Mitte und die niemals verheiratet gewesene Gruppe hatte bei weitem die niedrigsten Scores. Auf emotionalem Gebiet jedoch schnitt die getrennt/geschiedene Gruppe am schlechtesten ab und die anderen Gruppen unterschieden sich nicht voneinander. Es scheint, daß der Status, verheiratet zu sein, mehr Beziehung zum Emotionellem als zu den geistigen Fähigkeiten besitzt und daß die Fáhigkeit, unabhängig zu leben mehr Beziehung zur mentalen Fähigkeiten als zu der
