SUMMARYA questionnaire survey for epilepsy was mailed to 2042 families of third‐grade students in Washington County, Md. The final 1866 responses were stratified into 8 groups. It was hypothesized that the percentage of children with epilepsy in Groups 1–3 would be significantly higher than in Groups 4–7. A sample of 200 children was selected for clinical and EEG examination to test this hypothesis and to validate the questionnaire. Of children examined in Group 1, 53% had epilepsy; in Groups 2–3, 8% had epilepsy, in Group 4, 2% had epilepsy. No cases of epilepsy were found in children from Groups 5–7.To develop a comprehensive prevalence estimate for epilepsy, 6 institutions were surveyed by telephone. A minimum prevalence rate of 14.1 per 1000 was derived by adding the cases of epilepsy in the institutionalized group to those found in the questionnaire survey, and dividing by the population at risk. A more reasonable prevalence rate of 18.6 per 1000 was derived by assuming that those not examined in each questionnaire group would have the same rate of epilepsy as those examined.A maximum prevalence rate of 20.1 per 1000 was derived by assuming that non‐respondents to the questionnaire would have the same rate of epilepsy as respondents. Prevalence rates for simple febrile convulsions and minimal brain dysfunction were similarly calculated.ABSTRACTA questionnaire was developed to ascertain the prevalence of epilepsy in a population of about 2000 third‐grade students. SUMMARYChildren with infantile spasms, 104 boys and 46 girls, were followed up; 105 received steroids. Thirty‐three children had died, nearly all of the rest were 5 yr old or more.Of children whose development was normal before the onset of spasms, (cryptogenic and immunization groups) 37% made a full recovery and attended normal schools; early therapy with steroids seemed to be of benefit in them. Other factors of importance were a short period of spasms (less than 10 mo) and absence of other types of fit.A bad prognosis – death or severe subnormality – was associated with early onset and long duration of spasms, severe retardation on presentation, the presence of other fits, abnormal development prior to the onset of spasms, and evidence of any cerebral damage, disorder or disease.RÉSUMÉOn a suivi ľévolution ďenfants ayant présenté des spasmes infantiles, 104 garcons et 46 filles. Trente‐trois enfants sont morts et la plupart des autres avaient 5 ans ou plus.Parmi les enfants dont le développement était normal ou seulement très légèrement retardé au début des spasmes (groupe sans étiologie et après vaccination) 37% ont guéri complètement et ont suivi une scolarité normale. Une thérapeutique précoce par des stéroïdes semble etre bénéfique chez eux. Dreiunddreissig Kinder starben, fast alle übrigen waren bei der Untersuchung mindestens 5 Jahre alt.Von den Kindern, deren Entwicklung bis zu Beginn der Krämpfe normal verlief (Gruppen der kryptogenetischen bzw. nach Impfungen auftretenden Krämpfe), erholten sich 37% völlig und besuchten normale Schulen; die frühzeitige Therapie mit Steroidal scheint bei ihnen erfolgreich gewesen zu sein. Wichtig für dieses Ergebnis waren eine kurze Krankheitsdauer (weniger als 10 Monate) und das Fehlen anderer Anfallsformen.Zeichen für eine schlechte Prognose – Tod oder schwere cerebrale Schädigung mit Intelligenzminderung – waren: früher Beginn der Krämpfe, lange Krankheitsdauer, eine pathologische Entwicklung bzw. schwere Rückstandigkeit bei Beginn der Behandlung, das Vorhandensein anderer Anfallsformen und Zeichen jeder cerebralen Schädigung oder Erkrankung.RESUMENHa sido seguida la evolución de 104 niños y 46 niñas con espasmos infantiles; 33 han fallecido, mientras que casi todos los restantes tienen actualmente 5 años de edad o más.En los niños cuyo desarrollo era normal o sólo ligeramente retrasado al inicio del síndrome (grupo criptogenético y de immunización), el 37% han llegado a una completa recuperateón y acuden a escuelas normales; en ellos una terapéutica precoz con esteroides ha dado excelentes resultados. SUMMARYReport about experiences with clonazepam in the treatment of 24 infants with infantile spasms and 13 children with Lennox‐Gastaut syndrome. Control of spasms for a varying period of time was obtained in 13 infants with West syndrome and in 6 children with Lennox‐Gastaut syndrome, although nearly one‐third of the patients had failed to respond to previous treatment with nitrazepam. Among the side effects of clonazepam hypersecretion in the respiratory tract is the most limiting factor in infants.RÉSUMÉRapport des résultats concernant le traitement par le clonazepam de 24 enfants présentant des spasmes en flexion et de 13 enfants avec la syndrome de Lennox‐Gastaut; un tiers de ces 37 enfants avait démontré une résistance préalable au nitrazepam. La disparition des crises pour une période variable fut obtenue chez 13 nourrissons avec spasmes en flexion et chez 6 enfants avec syndrome de Lennox‐Gastaut. Parmi les effets secondaires ľencombrement des voies respiratoires chez les nourrissons fut le facteur le plus important.ZUSAMMENFASSUNGEs wird über die Erfahrungen mit Clonazepam in der Behandlung von 24 Säuglingen mit Blitz‐Nick‐Salaam‐Krämpfen und von 13 Kindern mit Lennox‐Gastaut Syndrom berichtet. Bei 13 Säuglingen mit BNS‐Krämpfen und bei 6 Kindern mit Lennox‐Gastaut Syndrom konnte eine Anfallsfreiheit von unterschiedlicher Dauer erzielt werden, wobei ein Drittel dieser Patienten früher auf Nitrazepam nicht angesprochen hatte. Die wichtigste Nebenerscheinung des Clonazepam war die Verschleimung der Atemwege bei 8 Säuglingen.Las crisis desaparecieron durante un periódo variable de tiempo en 13 de los enfermos con espasmos infantiles, y en 6 de la niños afectos de síndrome de Lennox. SUMMARYA rapid and inexpensive method to determine the degree of protein binding of diphenylhydantoin (and other anticonvulsants) is described; it is applicable to screening large samples of patients. The method was used in 25 selected subjects to test the hypothesis that degree of intoxication with diphenylhydantoin is related more to the serum concentration of free than of total diphenylhydantoin. The results support the hypothesis. It is suggested that the method could be used more widely and that it would reduce the variance between total serum concentration and clinical response for those anticonvulsants that are highly bound to serum proteins.RÉSUMÉOn décrit une méthode rapide et peu chère pour déterminer le degré de diphénylhydantoine liée aux protéïnes (et ďautres anticonvulsivants). Elle est applicable àľétude de larges échantillons de patients. La méthode a été utilisée sur 25 sujets sélectionnés pour tester ľhypothèse que le degréďintoxication aux hydantoïnes est plus en relation avec la concentration sérique de diphénylhydantoïne librequede la diphénylhydantoïne totale. SUMMARYA clinical trial with dipropylacetate (DPA), a new anti‐epileptic drug (Dépakine®, Ergenyl®) is reported in 116 patients. Primarily generalized epilepsies ‐ petit mal of different types and centrencephalic grand mal ‐ were mainly benefited. Less favourable results were obtained in epilepsy of focal origin. The side effects were usually minimal, consisting of gastric disturbances. Dipropylacetate increased the effect of other anti‐epileptics, especially barbiturates, primidone and benzodiazepines. Some patients were more alert and their behaviour improved on DPA.RÉSUMÉOn rapporte ľétude clinique de ľeffet du dipropylacétate (DPA) (Dépakine®, Ergenyl®), une nouvelle drogue anti‐épileptique chez 116 sujets. Ce fut surtout bénéfique dans les épilepsies généralisées primaires‐petit mal de différents types et grand mal centrencéphalique. On obtient de moins bons résultats dans ľépilepsie ďorigine focale. Les effets secondaires étaient généralement minimes, consistant en des troubles gastriques. Le dipropylacétate augmentait ľeffet des autres drogues anti‐épileptiques, particulièrement des barbituriques, du primidone et des benzodiazépines. Quelques patients avaient une meilleure conduite et étaient plus réveillés avec le DPA.ZUSAMMENFASSUNGDas Antikonvulsivum Dipropylacetat (Dépakine®, Ergenyl®) wurde bei 116 Patienten erprobt. Die bevorzugte Indikation sind primär generalisierte Epilepsien (Petit mal verschiedenen Typs und sogenanntes zentrenzephales Grand mal). Bei Epilepsien fokaler Genese sind die Erfolge weniger gut. Myoklonische Syndrome bei degenera‐tiven Hirnerkrankungen können günstig beeinflusst werden. Die Nebenwirkungen des Präparates sind insgesamt gering. Dipropylacetat verstarkt die Wirkung anderer Antiepileptica, insbesondere der Barbiturate, des Primidone und der Benzodiazepine. In machen Fällen entfaltet das Präparat eine günstige Wirkung auf das psychische Verhalten der Patienten.RESUMENEstudio clinico en 116 pacientes con dipropilacetato (DPA), nueva droga antiepiléptica (Depakine®, Ergenyl®). Se han benificiado principalmente de los efectos de esta medicateón las epilepsias generalizadas primarias (diferentes formas de petit mal y gran mal centroencefálico). Se han obtenido resultados menos favorables en la epilepsia de origin focal. La intolerancia ha sido generalmente mínima consistiendo en transtornos gastricos. El dipropilacetato aumenta el efecto de otros antiepilépticos especialmente de los barbitúricos, la Primidona y las benzodiacepinaz.

SUMMARYThe effect of clonazepam (RO‐4023) on the threshold to bemegride‐induced convulsions and on the focal electroencephalographical and clinical seizures after application of cobalt powder to the left sensorimotor cortex was investigated in 10 alert cats. A significant increase in the bemegride threshold as well as a signicant decrease in focal epileptic activity was observed. Clonazepam had an effect on generalized seizures comparable to that of diazepam but in dose ten times smaller and with less side‐effects.Clonazepam was much superior to diazepam in suppressing focal seizures and focal spiking.RESUMEOn a étudié chez 10 chats éveillés Peffet du clonazépam (RO‐4023) sur le seuil des convulsions produites par la mégimide et sur les crises focales électriques et cliniques obtenues par application de poudre de cobalt sur le cortex sensorimoteur gauche. On a observé de façon significative une augmentation du seuil à la mégimide et une diminution ďimportance de ľactivitéépileptique focale. Ľeffet du clonazépam sur les crises généralisées est comparable à celui du diazépam mais à des doses 10 fois plus petites et sans effets secondaires.Le clonazépam est bien plus efficace que le diazépam dans la suppression des crises focales et des pointes focales.ZUSAMMENFASSUNGDie Wirkung von Clonazepam (RO‐4023) auf die Auslösbarkeitsschwelle von Krampfanfällen durch Bemegrid und auf fokale elektroencephalographische und klinische Krampfaktivität nach Applikation von Cobaltpulver auf den sensorimoto‐rischen Cortex wurde an 10 Katzen im Wachzustand überprüft. Eine signifikante Erhöhung der Schwelle der Krampfauslösung durch Bemegrid und eine signifikante Abnahme fokaler epileptischer Aktivität wurden beobachtet. ABSTRACTThe effects of some anticonvulsant drugs on the development and persistence of cobalt‐induced epileptic foci have been studied in rats. Phenobarbitone produced bilateral polyspikes in control animals and apparently increased the frequency and duration of spike discharges in cobalt rats. Diphenylhydantoin had no discernible effect on the electrocorticogram in either control or cobalt animals. Ethosuximide had no effect in control animals but suppressed spike discharges in cobalt rats for 30–40 min after injection. Chronic administration of ethosuximide slightly delayed but did not prevent the development of the epileptic foci.SUMMARY1The effects of phenobarbitone, diphenylhydantoin and ethosuximide on the development and persistence of cobalt‐induced epileptic foci have been studied in rats.2Phenobarbitone produced bilateral polyspikes in control animals at doses of 5 mg/kg i.p. and 25–100 mg/kg orally. The same doses apparently increased the frequency and duration of spike discharges in cobalt‐treated rats. Drug‐induced spikes could not be distinguished from these produced by cobalt implantation and both types of discharge were accompanied by a motor disturbance.3Diphenylhydantoin (50 mg/kg i.p.) had no effect in control animals although it suppressed spike discharges in cobalt‐treated rats for a short period (<5 min) after injection. Chronic administration with the same dose daily did not prevent the development of the epileptic foci or their persistence.4Ethosuximide had no effect in control animals but suppressed spike discharges in cobalt‐treated rats in doses of 50–400 mg/kg i.p. This effect lasted 30–40 min. Chronic administration (100 mg/kg i.p. daily or 200 mg/kg orally daily) slightly delayed but did not prevent the development of the epileptic foci.RÉSUMÉChez les rats, on a étudié les effets de quelques drogues antiépileptiques sur le développement et la persistance de foyer épileptique produit par le cobalt. Ethosuximide hatte keine Wirkung auf Kontrolltiere aber es unter‐driickte bei Cobaltratten die spike‐Entladungen für 30–40 Min. nach der Injektion. Die chronische Anwendung von Ethosuximide verzogerte etwas die Entwicklung des epileptischen Fokus aber sie verhinderte sie nicht.RESUMENSe estudian la acción de ciertos medicamentos anticonvulsivantes en la evolución y persistencia del foco epiléptico inducido por cobalto en les ratas. La fenobarbitona que provoca polipuntas bilaterales en los animales de control, aparentemente aumenta la duratión y frecuencia de las descargas de puntas en las ratas tratadas con cobalto. La difenilhidantoina no da lugar a efectos diferentes en el electro‐corticograma entre los animales de control y los tratados con cobalto. La ectosuc‐cinamida no tiene acción en los animales de control, pero en cambio suprime la descarga de puntas en las ratas con cobalto durante 30–40 minutos después de su inyección. SUMMARYIn baboons from Senegal(Papio papio), which show photically‐induced epilepsy, we have recorded averaged responses to flash stimulation at the retina, the lateral geniculate body and the visual cortex. The effects of 3 drugs (D‐lysergic acid diethylamide, tranylcypromine and L‐5‐hydroxytryptophan) which block the paroxysmal electrographic and myoclonic responses to intermittent photic stimulation, have been compared with those of 2 monoamine precursors, L‐tryptophan and L‐2,4‐dihydroxyphenylalanine.D‐Lysergic acid diethylamide, 60μg/kg i.v., induced no modifications in the electroretinogram (ERG), but reduced the amplitude of the early positive wave at the lateral geniculate by 67 %. At the cortex, wave II diminished and wave III increased in amplitude.Tranylcypromine, 10–25 mg/kg i.v., reduced the amplitude of wavebof the ERG. Both early positive waves at the lateral geniculate were diminished in amplitude.L‐5‐Hydroxytryptophan, 20–30 mg/kg intraperitoneally, also diminished wavebof the ERG. At the lateral geniculate the second early positive wave was diminished after 30 min.L‐Tryptophan, 400 mg/kg i.p., produced a slight decrease in wavebof the ERG, and of the second positive wave at the lateral geniculate.L‐2,4‐Dihydroxyphenylalanine did not reduce visual responses at any level.Although tranylcypromine and 5‐HTP share with LSD 25 the capacity to block photically‐induced epileptic responses, their actions on evoked responses at the lateral geniculate are somewhat different, and at the cortex very different, from those of LSD 25.RÉSUMÉLes expériences ont été réalisées sur des Babouins du Sénégal(Papio papio)qui présentent un syndrome photomyoclonique. Les réponses évoquées par la stimulation lumineuse sont enregistrées au niveau de la rétine, du corps genouillé latéral (CGL) et du cortex visuel. Les effets de trois drogues (diéthylamide de ľacide D‐lysergique ou LSD 25; tranylcypromine et L‐5‐hydroxytryptophane) qui bloquent les décharges paroxystiques électrographiques et cliniques induites par la stimulation lumineuse intermittente (SLI) sont compareés à ceux de deux précurseurs des monoamines (L‐tryptophane et L‐2,4‐dihydroxyphenylalanine).Le LSD 25 (60μg/kg i.v.) ne provoque aucune modification de ľélectrorétino‐gramme (ERG); mais réduit de 67%ľamplitude de ľonde positive précoce de la réponse enregistrée dans le CGL. SUMMARYThe effect of diphenylhydantoin, an anticonvulsant, on the specific activity alterations of enzymes related to function, membrane stability and energy processes in the chronic isolated cerebral cortex of the cat as a function of time after undercutting is described. Data are also presented demonstrating the action of diphenylhydantoin on the development of hyperexcitability in terms of duration of afterdischarge monitored by surface‐recording post stimulus.RÉSUMÉĽactivité enzymatique était mesurée sur le cortex isolé de chats jusqu'au vingthuitième jour et chez des chats de contrôle. La diphénylhydantoïne diminuait le taux de monoamine oxidase qui résultait de la section sous piale mais elle n'affectait pas ľactivité des autres enzymes (déhydrogénase succinique, Na/K‐dependant ATPase, choline acétylase et acétylcholine esterase). Dans le cortex normal et dans le cortex isolé hyperexcitable, la diphénylhydantoïne diminuait la durée de la post‐décharge.ZUSAMMENFASSUNGDie Enzymaktivität wurde im unterschnittenen Cortex von Katzen bis zu 28 Tagen nach dem Eingriff und bei Kontrollen gemessen. Diphenylhydantoin vermindert den Anstieg der Monoaminooxidase als Folge der Unterschneidung. Es hat jedoch keine Wirkung auf die Veränderung der Aktivität anderer Enzyme (Succinyldehydro‐genase, Natrium‐Kalium abhängige ATPase, Cholinacetylase und Acetylcholinesterase). Sowohl im normalen wie im hyperexcitablen unterschnittenen Cortex vermindert Diphenylhydantoin die Dauer der Nachentladung.RESUMENSe mide la activadad encimática en el cortex crónicamente aislado del gato a lo largo de 28 diás. La difenilhidantoína disminuye el aumento de la monoaminoxidasa que se produce al aislar el cortex, pero no modifica los cambios de actividad de otras encimas (dehidrogenasa succínica, Na/K‐ATPasa, acetilasa colínica y esterasa acetilcolínica). SUMMARYThe epileptic model, chronic undercut cerebral cortex of monkey, was studied with regard to enzyme activity and to hyperexcitability monitored physiologically. The enzyme activities evaluated included transmitter‐related enzymes, monoamine oxidase, tyrosine hydroxylase, acetylcholine esterase and choline acetylase; a membrane‐related enzyme, NaK‐dependent ATPase; and an energy‐process‐related enzyme, succinic dehydrogenase. Only monoamine oxidase was significantly increased in undercut cortex over control values. Hyperexcitability in the form of spontaneous spiking and prolonged afterdischarge characterized the undercut cortex.RÉSUMÉOn a étudié un modèle épileptique de cortex cérébral isolé, chronique, chez le singe par rapport àľactivité enzymatique et au degréďhyperexcitabilitéélectrique.Les activités enzymatiques étudiées comprennent les “transmitter‐related” enzymes, la monoamine oxydase, la tyrosine hydroxylase, ľacétylcholine esterase; une enzyme de la membrane, la Na/K‐dependante ATPase, et les enzymes de processus énergétiques et la déhydrogénase succinique; seule la monoamine oxydase était augmentée de façon significative par rapport aux valeurs de contrôle; le cortex isolé par section sous piale (undercut cortex) montre une hyperexcitabilité caractérisée par des pointes spontanées et par des post‐décharges plus longues.ZUSAMMENFASSUNGAn einem Epilepsiemodell, dem chronisch unterschnittenen Cortex von Affen, wurden die Enzymaktivität und die Hyperexzitabilität auf physiologische, durch einen Monitor applizierte Reize geprüft. Die untersuchten Aktivitäten betreffen Enzyme, die mit der Reizübertragung zusammenhängen: Monoaminooxydase, Tyrosinhydroxylase, Acetylcholinesterase und Cholinacetylase, ein die Membran‐funktion betreffendes Enzym: Natrium‐Kalium abhängige ATPase, ein in den Energieprozess eingreifendes Enzym: Succinyldehydrogenase. Nur die Werte der Monoaminooxidase im unterschnittenen Cortex überstiegen signifikant die Kontroll‐werte. Der unterschnittene Cortex zeichnet sich durch eine Hyperexzitabilität in Form spontaner spike‐Entladungen und verlängerter Nachentladungen aus.RESUMENSe estudia la actividad encimatica y la hiperexcitabilidad provocada fisiologicamente en un “modelo” epiléptico constituído por una preparatión crónica de cortex cerebral aislado del mono. Se evalúan las actividades encimáticas correspondientes a encimas transmisoras, monoaminoxidasa, hidrolasa tirosínica, esterasa acetilcolínica; así como el ATPase dependiente de sodio y potasio, y la deshidrogenasa succinica.
