SUMMARYDose‐response studies were performed on freely moving rats bearing chronically implanted electrodes. The effects of intracortical injections of penicillin, bicuculline, and conjugated estrogens (Premarin®) were determined on EEG activity and motor performance. Bicuculline in doses of 0.025 and 0.05% regularly produced spike and afterdischarge (AD) patterns of rapid onset (<10 sec), brief duration (<20 min), and marked intensity. Lower doses had little or no effect. Premarin 1% and penicillin 100 international units produced spike and AD patterns of slower onset (5 min), longer duration (up to 2 hr), and mild intensity. Smaller doses generally produced no AD activity, whereas larger doses produced epileptic patterns severe enough to preclude their use in operant studies. Penicillin and Premarin both produced a significant decrease in operant‐response rate in animals trained to bar press on a FR 20 schedule. SUMMARYSurface and depth EEG seizure patterns were compared in 34 patients with intractable temporal lobe epilepsy in whom depth EEG electrodes had been chronically implanted in order to localize epileptogenic sites with a view to surgery. EEG records accompanied by clinical seizures, auras, no behavioral changes, as well as records for which no behavioral observations had been made, were judged with respect to the manner in which seizure activity originating unilaterally in the depth of one of the temporal lobes spread to the surface. For each EEG record, the onset of seizure activity in depth was classified as being focal or regional in form, and seizure activity was judged as: (1) not spreading to the surface, (2) spreading bilaterally and synchronously to the surface, (3) spreading initially to the surface ipsilateral to the depth site(s) in which the electrographic seizure first appeared, or (4) spreading initially to the surface contralateral to the depth site(s) in which the seizure activity initially occurred. EEG seizure activity was found to be less likely to propagate to the surface for those records that were either unaccompanied by behavior changes or accompanied only by auras than for those records accompanied by clinical seizures. In records accompanied by clinical seizures, seizure activity commonly propagated to the surface in a bilateral and synchronous fashion and was also found to spread initially to the ipsilateral but not to the contralateral surface. Anatomical and electrophysiological data accounting for the occurrence of ipsilateral spread were discussed. Ihnen waren EEG‐Tiefenelektroden chronisch implantiert worden, um die epileptogenen Herde in Hinblick auf einen chirurgischen Eingriff zu lokalisieren. Die EEG‐Aufzeichnungen während klinisheer Anfälle, Aura, ohne Verhaltensänderung ebenso wie Aufzeichnungen für die keine Verhaltensbeobachtung gemacht wurden, wurden ausgewertet, um zu erfahren, auf welche Weise die Anfallsaktivität, die ihren Ursprung einseitig in der Tiefe eines der Temporallappen hat, sich auf die Oberfläche ausbreitet. Für jede EEG‐Kurve wurde der Beginn der Anfallsaktivität in der Tiefe als fokal oder regional klassifiziert. Die Anfallsaktivität wurde bewertet als: (1) ohne Ausbreitung zur Oberfläche, (2) mit bilateraler synchroner Ausbreitung zur Oberfläche; (3) mit initialer Ausbreitung zu der Oberfläche der Seite, auf der in der Tiefe der elektrographische Anfall zuerst beobachtet wurde, oder (4) Ausbreitung initial zur Oberfläche der Gegenseite, in deren Tiefe die Anfallsaktivität initial auftrat. SUMMARYThe authors polygraphically studied the nocturnal sleep of 20 neurologically normal children with typical centrotemporal spikes. The children were divided into two groups: (A) 10 children with centrotemporal spikes and benign epilepsy; and (B) 10 children with centrotemporal spikes without epilepsy.The mean age when the average period of sleep record was performed was the same for the two groups, 8.5 years. The cyclic organization of sleep and the percentages of the different stages were normal in all 20 subjects. The number of spikes was counted in all the subjects during wakefulness and during different stages of sleep. All subjects of both groups showed an important increase in the frequency of spikes going from drowsiness until slow sleep; on the other hand, the activity remained high during REM sleep only for subjects of Group A. Other differences between the two groups concerned the appearance during sleep of generalized spike waves and independent Rolandic spikes. The authors conclude with the affirmation that the polygraphic study of nocturnal sleep in children with Rolandic spikes can be an additional parameter for differential diagnosis between children with and those without epilepsy.RÉSUMÉLes auteurs ont étudie en polygraphie le sommeil de nuit complet de 20 enfants avec pointes Rolandiques sur l'EEG, sans troubles neurologiques. Ces enfants ont été répartis en deux groups. Groupe A: 10 enfants avec pointes Rolandiques et épilepsie benigne de l'enface; Groupe B: 10 enfants avec pointes Rolandiques sans épilepsie. L'âge moyen auquel a été pratiqué le traces de sommeil est homogène pour les deux groupes (8.5 ans). Les 20 sommeils ont montré une organisation cyclique et un pourcentage en phases normaux pour l'âge. Chez tous les sujets des deux groupes les décharges focales ont été quantifiées au cours de la veille et des différentes phases du sommeil, y compris la PMO. Chez les 20 sujets il existait une très importante activation des décharges Rolandiques, de l'endormissement au sommeil lent; par contre l'activation persistait importante en PMO, seulement chez les sujets avec épilepsie. SUMMARYA double‐blind study of the antiepileptic effect and side effects of carbamazepine (CARB) and diphenylhydantoin (DPH) was undertaken in 38 patients with psychomotor epilepsy and without grand mal epilepsy except for a single previous seizure. The patients were treated with CARB and DPH only, each in periods of 16 weeks and with a crossover of 4 weeks. The initial dosage of 6 mg/kg DPH or 15 mg/kg CARB was corrected according to the serum values aiming at therapeutic intervals of 8–16 mg/1 DPH and 6–10 mg/1 CARB. The trial had to be discontinued in 12 patients. The effect of the two drugs in preventing psychomotor seizures was the same. Some patients, however, had considerably fewer seizures while on CARB; others had fewer seizures on DPH. It seems advisable, therefore, to try both drugs separately before proceeding to combined medication. During CARB treatment the selected therapeutic interval was more easily reached and maintained than during DPH. During the latter treatment, one‐third of the monthly serum value determinations were below the level in spite of dosage corrections. Side effects were equally mild and occurred as often during DPH as during CARB treatment.RESUMENSe ha realizado un estudio doble ciego para valorar los efectos antiepilépticos y los signos tóxicos de la carbamazepina (CARB) y de la difenilhidantoina (DPH) en 38 enfermos con epilepsía psicomotora y sin epilepsía gran mal con la excepción de una sola convulsión previa.Los enfermos fueron tratados exclusivamente con CARB y DPH durante periodos de 16 semanas cada uno con 4 semanas de tratamiento cruzado.La dosis inicial de 6 mg/Kg de DPH o 15 mg/Kg de CARB fué corregida según los nivelés séricos y tendiendo a alcanzar intervales terapéuticos de 8 a 16 mg/L de DPH y 6 a 10 mg/L de CARB. Este estudio tuvo que ser interrumpido en 12 enfermos.El efecto de ambas medicaciones fué el mismo en lo que se refiere a la prevención de los ataques. Sin embargo algunos enfermos tuvieron muchos menos episodios con la CARB y otros con la DPH. Die Initialdosis von 6 mg/kg DPH oder 15 mg/kg CARB wurde entsprechend den Serumspiegeln angepasst, um therapeutische Bereiche von 8–16 mg/1 DPH und 6–10 mg/1 CARB zu erzielen. Bei 12 Patienten musste der Versuch abgebrochen werden. Beide Medikamente waren hinsichtlich der Vermeidung psychomotorischer Anfälle gleichermassen wirksam. SUMMARYRats were cerebellectomized 72–96 hr prior to evaluation (1) during maximum electroshock seizures and (2) for their capacity to respond to pentylenetetrazol‐induced clonic seizures. Cerebellectomized rats failed to exhibit tonic hindlimb extension, an endpoint characteristic of maximal electroshock seizures. The dose of pentylenetetrazol required to produce clonic seizures or death was not different in cerebellectomized and sham‐operated controls. The anticonvulsant efficacy of diazepam, when assessed as a pentylenetetrazol antagonist, was not influenced by removal of the cerebellum. These data indicate that whereas cerebellar influences may suppress seizure activity which is largely focal, seizures of more diffuse origin are not markedly influenced by cerebellar activity. It is, therefore, essential that the role of the cerebellum in suppressing seizures be characterized for each kind of experimentally induced seizure process.RESUMENVarias ratas fueron cerebelectomizadas 72–96 horas antes de la evaluatión de las convulsiones al electroshock máximo y de su capacidad para responder a la inductión de convulsiones clónicas con el pentilentetrazol.Las ratas cerebelectomizadas no demostraron extensión tónica de las patas traseras, dato fundamental y característico de las convulsiones al electroshock máximo. La dosis de pentilentetrazol necesaria para producir convulsiones clónicas o la muerte no fué distinta en los animales cerebelectomizados o en los operados‐control. La eficacia anticonvulsiva del diazepan, determinada como antagonista del pentilentetrazol, no se modificó al extirpar el cerebelo. Estos datos indican que mientras que el cerebelo puede influenciar suprimiendo la actividad convulsivógena fundamentalmente focal, los ataques de orígen más difuso no se modifican sustancialmente por la actividad cerebelosa. SUMMARYIn a vanety of laboratory models of experimental epilepsy, imipramine exerts a biphasic action on the CNS as manifested by antiepileptic properties at low doses and convulsant effects at higher doses. In mice, imipramine (17.5–25 mg/kg, i.p.) blocks maximal electroshock seizures while exerting little or no effect on pentylenetetrazol‐induced seizures. In cats, imipramine (2.5–15 mg/kg, i.v.) reduces penicillin and estrogen‐induced epileptiform discharge, shortens afterdischarge duration, and elevates afterdischarge threshold. Higher doses in mice induce neurotoxicity, including clonic seizures. In cats, doses above 20 mg/kg intensify chemically and electrically induced seizures and induce spontaneous epileptiform episodes. Such a biphasic action of imipramine may limit the drug's clinical utility as an antiepileptic agent and may provide an interesting tool for studies of catecholamines and brain excitability.RESUMENEn diversos modelos de epilepsía experimental la imipramina ejerce una acción bifásica sobre el SNC que se manifiesta por propiedades antiepilépticas a dosis reducidas y por efectos convulsivos a dosis más elevadas. En ratones la imipramina (17,5–25 mg/kg i.v.) bloquea las convulsiones del electroshock máximo mientras que produce poco o ningún efecto sobre los ataques inducidos por el pentilentetrazol. En gatos la imipramina (2,5–15 mg/kg i.v.) reduce las descargas inducidas por la penicilina y los estrógenos, acorta la duración de las post‐descargas y éleva el umbral de las post‐descargas. En ratones, dosis más elevadas, induce cierta neurotoxicidad incluyendo convulsiones clónicas. En gatos, dosis por encima de 20 mg/kg intensifican los ataques inducidos quimica o eléctricamente y desencadenan episodios epileptiformes espontáneos. SUMMARYAmygdaloid stimulations were applied to rats at gradually increasing current intensities once every 60 sec until an afterdischarge (AD) was recorded through the stimulation electrode. These initial AD “thresholds” (x = 75 μA) were reduced over a 4‐day period by a series of 12 subthreshold stimulations. In contrast, stimulations administered on the same regimen but at a higher intensity (400 μA) had raised the AD threshold. The decreases in the AD threshold were relatively permanent, whereas the elevated thresholds were subsequently reduced to control levels by a series of subthreshold stimulations or by a 7‐day stimulation‐free period. Two days following the completion of Experiment 1, all of the rats were injected with a low dose of pentylenetetrazol. The incidence of pentylenetetrazol‐induced epileptic symptoms was much higher in the experimental animals than in the control subjects previously subjected only to the threshold estimation procedure.RESUMENEstimulaciones de la amfgdala fueron aplicadas a ratas de manera gradual, aumentando la intensidad de las corrientes cada 60 seg. hasta que, a través del electrodo de estimulación, se consiguió una post‐descarga (AD). SUMMARYUsing data obtained by the National Center for Health Statistics through household interviews, an estimate of 8,100,000 cases of head injury was determined for the civilian population of the United States in 1974. Excluding contusions and lacerations of the scalp, face, and neck, there remained 1,900,000 with concussion, skull fractures, intracranial hemorrhage, cerebral laceration, or other intracranial injury. Extrapolating from the military experience with craniocerebral trauma, admittedly a rough approximation, it is estimated that in 30% of this latter group one or more seizures will develop. Both the severity of the injury and the predisposition of the injured are thought to play a part in the occurrence of seizures, with the predisposition playing the dominant role in the persistence of seizures. The following gradation of posttraumatic epilepsy is predicted for the 1,900,000 with the greater implication of brain damage: 1,340,000 will never have a seizure; 560,000 will develop one or more attacks, most of which will begin in 1974, 1975, or 1976. In 280,000 of these, the attacks will be transient and of little consequence. In an equal number, the attacks will require medical attention, and in some 140,000 the seizures will be intractable to therapy. This latter group represents 7% of the 1,900,000 cases.Attention is directed to the practical problems in the control of craniocerebral trauma and of posttraumatic epilepsy.RESUMENUtilizando datos obtenidos por el Centro Nacional de Estadística para la Salud, conseguida mediante consultas a domicilio, se han reunido unos 8,100,000 casos de traumatismos craneoencefálicos en la población civil de los Estados Unidos en 1974. Una vez excuidas las laceraciones del cuero cabelludo, de la cara y cuello, quedaron 1,900,000 de conmociones, fracturas de cráneo, hemorragia intracraneal, laceración cerebral u otros tipos de lesiónes intracraneal es. Extrapolando de la experiencia militar con respecte a los traumatismos craneoencefálicos, aún admitiendo que sea una estimación grosera, se calcula que un 30% de este ultimo grupo habrá desarrollado una o más convulsiones. Se estima que tanto la severidad del trauma como la predisposición personal juegan un papel en la produccion de los ataques siendo la predisposición el factor más importante que détermina la persistencia de los ataques. Se puede predecir la siguiente gradación de epilepsía post‐traumática para los 1,900,000 casos con mayor implicación de sufrimiento cerebral: 1,340,000 nunca desarrollaran ataques; 560,000 tendrán uno o más ataques y la mayoría empezarán en 1974, 75 o 76. En 280,000 de estos últimos los ataques serán transitorios y sin consecuencias importantes. En el mismo número de casos los ataques necesitarán atención médica y en 140,000 las canvulsiones serán resistentes al tratamiento. Este último grupo representa el 7% de los 1,900,000 casos.Se dirige la atención hacia los problemas prácticos en el control de los traumatismos craneoencefálicos y de la epilepsía post‐traumática.ZUSAMMENFASSUNGUnter Zuhilfenahme von Daten des National Center for Health Statistics, die durch Haush‐altsbefragung gewonnen waren, wurde eine Schätzung von 8 100 000 Fällen von Schädeltrauma in der Zivilbevölkerung der Vereinigten Staaten 1974 ermöglicht. Unter Ausschluss von Contusionen und Verletzungen der Kopfschwarte, des Gesichts und des Halses blieben 1,900,000 Fälle mit Commotio, Schädelbrüchen, intracranieller Blutung, Hirnzerreissung oder anderer intracranieller Verletzung. SUMMARYParenterally injected penicillin in rats induces a peculiar pattern of multifocal epilepsy. The effective amount is higher than that required in cats. The epileptiform activity initially appears on the cortical mantle of one hemisphere; after a variable delay, contralateral cortical spikes arise in a completely asynchronous way. Spontaneous independent firing is observed, with a further delay in subcortical structures. Although cortical spiking shows a tendency to become bilaterally synchronous, such synchrony has been only occasionally observed between the various subcortical structures. Formation of generalized spike‐and‐wave complexes, as reported in cats, or other features reminiscent of the human petit mal did not occur. This study stresses the differences in pathophysiological responses to the same epileptogenic model according to species variation.RESUMENLa penicilina inyectada parenteralmente en ratas induce un patrón peculiar de epilepsía multifocal. La cantidad efectiva es mayor de la que se requiere en los gatos. La actividad epileptiforme aparece inicialmente en el manto cortical de un hemisferío; tras un retraso variable aparecen puntas corticales contralaterales de forma completamente asincrónica. Con mayor retraso se observan descargas independientes y espontáneas en las estructuras subcorticales. Mientras que las puntas corticales muestran tendencia hacia la sincronía bilateral, esta sincronía sólo aparece ocasionalmente entre las diversas estructuras subcorticales. Nunca se observarón complejos punta‐onda generalizados, como se ha publicado en gatos, ni otros datos que recuerden al Petet Mal humano. Spontane unabhängige Entladungen werden nach weiterem Intervall in subcorticalen Strukturen beobachtet. Während die corticalen Entladungen die Tendenz zur bilateralen Synchronizität zeigen, wird solche Synchronie zwischen verschiedenen subcorticalen Strukturen nur gelegentlich beobachtet.
