Summary:U18666A, an inhibitor of desmosterol reductase (a terminal enzyme in cholesterol synthesis), has been found to produce chronic epileptiform activity in laboratory animals. Since desmosterol might substitute for cholesterol in neuronal membranes without detriment, the present study was undertaken to examine the possibility that this drug‐induced epilepsy was related to changes in other brain lipids. Chronic treatment of the rat with U18666A, beginning at one day of age, resulted in pronounced decreases in the concentration of phospholipids and increases in gangliosides of brain microsomal, synaptosomal, and crude myelin fractions. Since total sterol levels were not changed, the ratio of sterols to phospholipids also increased. If drug treatment was stopped at 4 weeks of age, brain lipids of all subcellular fractions examined returned to normal levels by 8 weeks, and no epileptiform activity was detected. However, following 8 weeks of continuous treatment, epileptiform activity was present, and the changes in brain lipids were focused in the myelin fraction. Phospholipid levels and the sterol:phospholipid ratio of microsomes and synaptosomes, in contrast to myelin, were near normal; however, gangliosides were still clearly elevated in all fractions. A reported ability to induce epileptiform activity in rats by treatment with antiserum to brain gangliosides could indicate a special significance of the altered myelin and synaptic gangliosides to the U18666A‐induced epilepsy. We suggest that some epileptiform conditions could be directly related to alterations in the lipid composition of critical neuronal structures.RÉSUMÉL'U 18666A, inhibiteur de la reductase du desmosterol (enzyme terminal de la synthèse du cholestérol) est capable de produire une activitéépileptiforme chronique chez les animaux de laboratoire. Le desmosterol pouvant se substituer au cholestérol dans les membranes neuronales, le but de cette étude est de rechercher si cette épilepsie induite par la drogue peut modifier d'autres lipides cérébraux. Chez le rat, un traitement chronique avec l'U 18666A débutant à lâge de un jour entraîne une diminution marquée de la concentration des phospholipides et une élévation des gangliosides dans les fractions microsomale, synaptosomale et myélinique. Les taux de sterol total nétant pas modifiés, le rapport sterols/phospholipides augmente aussi. Si, à láge de quatre semaines, le traitement est arrété, les taux des lipides cérébraux de toutes les fractions subcellulaires examinées se normalisent en huit semaines et aucune activitéépileptiforme n'est détectée. Cependant, à la suite d'un traitement continu de 8 semaines, on note une activitéépileptiforme et une modification des lipides cérébraux principalement dans la fraction myélinique. Les taux des phospholipides et le rapport sterol/phospholipide des microsomes et des synaptosomes contrairement à ceux de la myéline, sont sensiblement normaux; cependant, les gangliosides sont encore nettement élevés dans toutes les fractions. La possibilité des antisérums de gangliosides cérébraux d'induire une activitéépileptiforme chez le rat souligne l'importance de la modification des gangliosides synaptiques et myéliniques lors de lépilepsie induite par l'U 18666A. Die chronische Behandlung der Ratten mit U18666A, wurde im Alter von 1 Tag begonnen und führte zu einer ausgeprägten Abnahme in der Konzentration der Phospholipide und einer Zunahme der Ganglioside der mikrosomalen, synaptosomalen und rohen Myelinfraktionen des Gehirns. Da die Sterol‐Gesamtspiegel unverändert blieben, wuchs das Verhältnis Sterol zu Phospholipide. Wenn die Medikamente im Alter von 4 Wochen abgesetzt wurden, waren normale Spiegel der Hirnlipide und aller subzellären Fraktionen nach weiteren 4 Wochen zu messen und keine epileptiforme Aktivität konnte entdeckt werden. Nach 8 Wochen kontinuierlicher Behandlung jedoch fand man epileptiforme Aktivität und die Veränderungen der Hirnlipide betrafen die Mye‐linfraktionen. Die Phospholipidspiegel und das Verhältnis Sterol zu Phospholipiden der Mikrosomen und Synaptosomen waren im Gegensatz zum Spiegel des Myelin fast normal; dennoch blieben die Ganglioside in allen Fraktionen eindeutig erhöht. Die in der Literatur mitgeteilte Fähigkeit, epileptiforme Aktivität bei Ratten durch Behandlung mit Antiserum gegen Hirnganglioside hervorzurufen, spricht für die besonderen Beziehungen des veränderten Myelins und der synaptosomalen Ganglioside zur U18666A‐induzierten Epilepsie. Summary:The concentration, half‐life, and distribution in brain of U18666A, a drug that can drastically alter cerebral lipids and induce a chronic epileptiform state, was determined following both acute and chronic drug administration. U18666A specifically labeled with tritium was prepared by custom synthesis. Brain levels of 1 times 10‐6M and higher were reached soon after giving an acute 10‐mg/kg dose (i.p. or s.c.) of U18666A containing 7‐3H‐U18666A of known specific activity. A steady state concentration of 1 to 2 times 10‐6M was reached with chronic injection of 10 mg/kg every 4th day, a treatment schedule that results in altered brain lipids and induction of epilepsy if begun soon after birth. The disappearance of U18666A from both brain and serum was described by two similar biexponential processes, a brief rapid clearance(t1/2= 10 h) and a sustained and much slower one (t1/2= 65 h). Brain levels of the drug were about 10 times higher than serum at all times examined. Few differences were seen in the regional distribution of radiolabeled drug in brain as determined by both direct analysis and by autoradiographic examination; but the drug did concentrate in lipid‐rich subcellular fractions. For example, the synaptosome and myelin fractions each contained about 25–35% of both the total3H‐labeled drug and total lipid in whole brain. The lipid composition of these fractions was drastically altered in treated animals. In conclusion, the chronic epileptiform state induced by U18666A does not appear to involve localization of the drug in a specific brain region or particular cell type. Rather, the condition could involve localization of the drug in lipid‐rich membranes and marked changes in the composition of these membranes.RÉSUMÉLa concentration, la demie vie et la distribution cérébrale de I'U 18666A, substance pouvant sérieusement altérer les lipides cérébraux et induire un état épileptiforme chronique, ont été déterminées après administration aiguë et répétée de la drogue. L'U 18666A marqué spécifiquement au tritium est préparé par les procédés habituels de synthèse. Des taux cérébraux de 1 times 10‐6M et des taux plus élevés sont atteints tout de suite après I'administration d'une dose aiguë de 10 mg/kg (i.p. ou s.c.) de U 18666A contenant une activité spécifique connue de 7‐3H‐U18666A. Une concentration stable de 1 à 2 times 10‐6M est obtenue avec une injection chronique de 10 mg/kg tous les 4 jours, traitement qui, s'il est entrepris tout de suite aprés la naissance, entraîne une modification des lipides cérébrale et l'induction d'une épilepsie. Lélimination cérébrale et sérique de l'U 18666A s'effectue suivant deux processus similaires biexponentiels, une clairance courte et rapide (t1/2= 10hr) et une longue et plus lente (t1/2= 65 hr). Dans tous les cas, les concentrations cérébrales de la drogue sont environ 10 fois plus élevées que celles du sérum. On observe au niveau du cerveau peu de différences dans la distribution régionale de la substance marquée aussi bien après analyse directe qu'aprés autoradiographie; cependant on note une concentration plus élevée dans les fractions subcellulaires riches en lipides. Ainsi, dans tout le cerveau les fractions myéliniques et synaptosomales contiennent chacune environ 25 à 35% aussi bien de la drogue marquée que des lipides totaux. La composition lipidique de ces fractions est nettement modifiéed chez les animaux traités. En conclusion Iétat épileptiforme chronique induit par I'U 18666A ne semble pas impliquer une localisation de la drogue dans une région spécifique cérébrale ou un type particulier de cellule. Cet état pourrait plutôt impliquer une localisation de la drogue dans les membranes riches en lipides et une modification de la composition de ces membranes.ZUSAMMENFASSUNGBestimmung der Konzentration, Halbwertszeit und Verteilung von U18666A im Gehirn, einem Medikament, das die cerebralen Lipide drastisch verändern und einen chronischen epileptiformen Zustand hervorrufen kann, im akuten und chronischen Versuch. Speziell mit Tritium markiertes U18666A wurde durch übliche Synthese hergestellt. Hirnspiegel von 1 times 10‐6Mund höher wurden bald nach Verabreichung von 10 mg/kg (i.p. oder s.c.) U18666A erreicht, die 7‐3H‐U18666A in bekannter spezifische Aktivität enthilt. Eine steady state Konzentration von 1 bis 2 times 10‐6Mwurde durch chronische Injektion von 10 mg/kg jeden 4. Tag erreicht; dieses Behandlungsschema führt zu einer Veränderung der Hirnlipide und einer Induktion der Epilepsie, wenn es kurz nach der Geburt begonnen wird. In der regionalen Verteilung des radiomarkierten Medikaments im Gehirn (Bestimmung sowohl durch direkte Analyse als auch durch Autoradiographie) wurden wenig Veränderungen gesehen. Allerdings zeigte das Medikament eine Konzentration in den lipidreichen subzellulären Fraktionen. So enthielten z.B. die Synaptosomen und Myelinfraktionen je 25 bis 35% sowohl des gesamten H3‐markierten Medikaments als auch der gesamten Hirnlipide. Die Lipidzusammensetzung dieser Fraktion war bei den behandelten Tieren drastisch verändert. Wir folgern, daβ der chronische epileptiforme Zustand, durch U18666A hervorgerufen, nicht die Lokalisation des Medikaments in spezifischen Hirnregionen oder spezifischen Zelltypen zu beeinflussen scheint. Summary:Fifty patients with epilepsy were randomly divided into three groups and given, respectively, oral information about the nature, purpose, appearances, functions, and unwanted effects of their medications at an initial interview; the same information supplemented by its presentation in written form for the patient to take home; and the same information by telephone contact only. Compliance with anticonvulsant therapy was assessed by interview and by drug levels. The amount of knowledge retained and the drug levels were measured again 4 weeks later. While no increase in serum levels could be detected over the mean values in the first interview, no reduction in levels could be documented either, although the drug information sheets had listed both the minor and the more serious unwanted effects of the drugs. The patients' information scores improved significantly in all three groups, but the combination of data presented at interview both orally and in written form was markedly superior to the other methods.RÉSUMÉ50 Patients épileptiques ont éteA divisés arbitrairement en 3 groupes à qui on a donné les informations suivantes: dans le ler groupe, une information orale sur la nature, le but, les aspects, les fonctions et les effets indésirables de leurs médicaments lors d'un en‐tretien initial; dans le 2ème groupe la méme information orale et une information écrite à emporter chez eux; dans le 3éme groupe l'information a %eAté donnée seulement par téléphone. L'adésion au traitement anticonvulsivant a été testée par un entretien et par une mesure du taux des médicaments dans le sang. La quantité d'informations retenues par les patients et les taux des médicaments ont étéévalués de nouveau 4 semaines plus tard. Bien qu'aucune augmentation des taux sériques n'ait pu être détectée par rapport aux valeurs moyennes obtenues lors du premier entretien, aucune diminution des taux sériques n'a pu être mise en évidence non plus, alors que les feuilles d'information mentionnaient aussi bien les effets indésirables mineurs que les plus sérieux des drogues. Les scores d'information des patients s'ameélioraient de façon significative dans les 3 groupes mais la combinaison de l'information donnée oralement et de l'information éc‐rite

Summary:Filter paper strips soaked in3H‐glycine solution were applied to acoustic cortex of cats, anaesthetized with Nembutal and pretreated with epileptogenic agents (Metrazol. G‐penicillin, and 3‐amino‐pyridine) and cy‐cloheximide. The untreated contralateral hemisphere served as control. After I h incubation, both cortical samples were excised simultaneously and fixed in Bouin solution for autoradiography. Incorporation was blocked by cyclo‐heximide. There was no glycine incorporation on the penicillin‐treated side, while pyramidal cells were intensively labelled in layers II‐V of the mirror focus. 3‐Aminopyridine produced the same result. Metrazol as convulsant proved to be far weaker than the previous two. The intensity of incorporation was significantly more intensive in the mirror focus than in the primary one. Penicillin and 3‐aminopyridine, while provoking cortical seizures, seem to inhibit glycine incorporation into a neuron‐specific, function‐dependent protein contained by the labelled cells in the autoradiogram.RÉSUMÉDes bandes de papier filtre imprégnées d'une solution de3H‐glycine furent déposées sur le cortex auditif de chats prétraités par des agents épileptogènes (Métrazol, Pénicilline‐G, 3‐aminopyridine), puis par la cycloheximide. L'hémisphère contralatéral, non prétraité. servait de témoin. Les animaux étaient anesthésiés au Nembutal. Après une heure d'exposition, les deux territoires corticaux furent exciseés simultaneément, puis fixés dans une solution de Bouin en vue d'une étude radioautographique ultérieure. L'incorporation fut bloquée par la cycloheximide. Aucune incorporation de glycine ne s'est produite du cödté traité par la Pénicilline, tandis que dans le foyer en miroir un intense marquage est apparu dans les cellules pyramidales au niveau des couches II‐V. Des résultats identiques furent obtenus avec la 3‐aminopyridine. Le Métrazol se pésente comme un agent convulsivant beaucoup moins efficace que les deux autres. L'incor‐poration était significativement plus intense dans le foyer en miroir que dans le foyer primaire. Bien que susceptibles d'induire des crises corticales, la Pénicilline et la 3‐aminopyridine semblent inhiber l'incorporation de la glycine à une protéine neuronale spécifique, liée à la fonction, et qui serait présente dans les cellules marquées des radioautographies.ZUSAMMENFASSUNGFilterpapierstreifen, die mit3H‐Glycin‐Lösung getränkt waren, wurden auf den akustischen Cortex von Katzen appliziert. Die Inkorporation wurde dureh Cycloheximid blockiert; auf der Penicfllinbehandelten Seite wurde kein Glycin inkorporiert während die Pyramidenzellen des Spiegelfokus in den Lagen II bis V intensiv markiert sich darstellten. 3 Aminopyridin rief das gleiche Ergebnis hervor. Summary:In view of the observed variation of valproic acid (VPA) free fraction (fp) during a dosing interval and the competitive binding effect of free fatty acids (FFA)in vitro, this study was designed to address the existence of diurnal variations in the fpof VPA. Six subjects were hospitalized at 7 a.m. for 25 h, and plasma samples were collected every 2 h. The protocol was repeated in 4 of the 6 subjects one week later.In vitrobinding of VPA (100 μg/ml) was determined by equilibrium dialysis (14C‐VPA), and FFAs were assayed col‐orimetrically. Phenytoin (PHT) binding was also determined for comparison. VPA fpranged rom 8.10 ± 1.16 to 9.63 ± 1.54. Intrasubject variability was also measured by the ratio of maximum to minimum fp. values (fpmax/fpmin) over 24 h: This ratio ranged from 1.30 to 1.68 (mean 4pL %SD = 1.51 ± 7.7%, n = 10). For PHT, fpranged from 10.88 ± 0.50 to 12.39 ± 1.07, and fpmax from 1.09 to 1.31 (1.17 ± 5.1%, n = 10). The fpmax was observed between 2 and 6 a.m. in 7 out of 10 cases for VPA and 5 out of 10 cases for PHT. FFA levels, although in the normal range, varied two‐to fourfold within 24 h. A significant correlation was observed between mean FFA levels at each sampling time and the corresponding fpvalues for VPA (p<0.001), but not for PHT.RESUMELa variation de la fraction libre (fp) de l'acide val‐proique (VPA) observée entre deux prises médica‐menteuses et l'effet du binding compétitif,in vitro, des acides gras libres (FFA) ont conduit àétudier les modifications diurnes de la fp du VPA. Le binding du VPA (100 μ/ml) in vitro est mesuré par dialyse à l'eAquilibre (14C VPA) et les FFAs évalués par colorimétrie. Le binding de la phenytoine (PHT) est également étudié. Les taux de fp du VPA se répartissent entre 8.10 ± 1.16 et 9.63 ± 1.54. La variabilité intraindividuelle est recherchée par le calcul du rapport des valeurs maximales et minimales de la fp (fpmaxpmin) pendant 24 heures: ce rapport est compris entre 1.30 et 1.68 (moyenne ± DS %= 1.51 ± 7.7%, n = 10). En ce qui concerne la PHT, la fp varie entre 10.88 ± 0.50 et 12.39 ± 1.07 et le rapport fp max/fp min entre 1.09 et 1.31 (1.17 ± 5.1%, n = 10). Summary:We present a patient with epilepsia partialis continua involving the right leg who demonstrated sharp waves paradoxically distributed over the vertex and right hemisphere. Posterior tibial nerve evoked potentials also showed a similar paradoxical lateralization. We postulate that generators situated on the mesial surface of the left hemisphere projected their activity obliquely, leading to paradoxical lateralization of the recorded electrical activity.RÉSUMÉNous présentons un patient ayant une épilepsie partielle continue qui intéressait la jambe droite et chez qui l'on mettait en évidence des “sharp waves” localisées de façon paradoxal sur le vertex et l'hémisphere droit. Les potentiels évoqués du nerf tibial postérieur montraient aussi une latéralisation paradoxale. Nous faisons l‐hypothése que les générateurs situés sur la face mesiale de l'hémisphère gauche projetaient leur aclivité obliquement, conduisant à la lateralisation paradoxale des activités électriques enregistrées.RESUMENSe presenta un pacienta con epilepsia parcial continua afectando a la pierna derecha que presentaba ondas agudas distribuídas de modo paradójico en el vertex y en el hemisferio derecho. Los potenciales evocados tras estímulos del nervio tibial posterior también demostraron una lateralización paradójica similar. Summary:Of 13 patients with complex partial seizures who experienced stuporous states during treatment with sodium valproate (VPA), 4 received VPA only, 4 VPA and phenobarbital (PB) and 5 VPA, PB, and a third anticonvulsant. Seven cases are described in detail. Side effects—stupor or confusion—appeared a few days after efficacious drug plasma levels were attained, persisted until therapy was readjusted, and disappeared 24 to 72 h after VPA withdrawal. Therapeutic trials established the role of VPA in the onset of stuporous states. The adverse effects of VPA were potentiated by the concomitant administration of other anticonvulsants. Stupor was not due to VPA overdoses, and plasma concentration of the drugs were not correlated with the electroclinical signs. The EEG showed spike and wave discharges or continuous sharp theta and delta waves persisting during VPA treatment. The fact that all 13 stuporous, VPA‐treated patients were subject to partial seizures with complex symptomatology, and none were cases of generalized epilepsy, together with the observations that the disturbances of consciousness started with focal symptoms and EEG signs resembling those of spontaneously occurring symptoms and EEG signs resembling those of spontaneously occurring partial seizures, suggest that VPA given alone or in association with other antiepileptics has a paradoxical epileptogenic effect in certain forms of epilepsy.RÉSUMÉParmi les 13 patients avec crises partielles complexes qui ont présenté des états stuporeux pendant un traitement par Valproate de sodium (VPA), 4 recevaient du VPA seul, 4 VPA et phénobarbital (PB), et 5 VPA, PB et un troisième anticonvulsivant. 7 cas sont décrits en detail. Les effets collatéraux (stupeur ou confusion) apparaissaient quelques jours après l'installation d'un niveau plasmatique efficace du médicament, persistaient jusguà ce que le traitement soit modifyé, et disparaissaient 24 à 72 H après l'arrêt du VPA. Des épreuves thérapeutiques ont établi le rôle du VPA dans le déclenchement des états stuporeux. Les effets indésirables du VPA étaient potentialisés par l'administration concomitante d'autres anticonvulsivants. La stupeur nétait pas due à un surdosage en VPA et il n'y avait pas de corrèlation entre les concentrations plasmatiques du médicament et les signes électro‐cliniques. Les EEG montraient des décharges de Pointes‐ondes ou des ondes thêta aiguës et des ondes delta persistant pendant le traitement par VPA. Le fait que ces 13 malades avec état stuporeux traités par le VPA avaient des crises partielles à sémiologie complexe et qu'aucun n'avait une épilepsie généralisée primaire, associé au fait que les troubles de la conscience débutaient avec des signes de localisation et que les anomalies EEG ressemblaient à celles que l'on voit dans les crises partielles spontanées, suggère que le VPA donné seul, ou en association à d'autres antiépileptiques, a un effet épiliptogène paradoxal dans certaines formes dépilepsie.ZUSAMMENFASSUNG13 Patienten mit komplexen Partialanfällen kamen in einen stuporösen Zustand während der Behandlung mit Natrium‐Valproat (VPA): 4 erhielten VPA Monotherapie, 4 VPA und Phenobarbital (PB) und 5 VPA, PB und ein drittes Antikonvulsivum. 7 Patienten werden detailliert beschríeben. Die Nebenwirkungen—Stupor oder Verwirrung—traten wenige Tage nach Erreichen wirksamer Plasmaspiegel auf, hielten an, bis die Therapie korrigiert werden konnte und verschwanden 24 bis 72 Stunden nach Absetzen des VPA. Therapeutische Versuche bewiesen die Rolle des VPA fur das Entstehen des stuporösen Zustands. Die negativen Wirkungen des VPA wurden durch gleichzeitige Gabe anderer Antikonvulsiva potenziert. Der Stupor war nicht Folge einer VPA‐Überdosis und die Plasmakonzentrationen des Medikaments waren nicht korreliert mit den elektroklinischen Symptomen. Das EEG zeigte spike wave Ausbrüche oder kontinuierliche scharfe Theta‐ und Delta‐Wellen, die während der VPA‐Behandlung persistierten. Die Tatsache, daß alle 13 stuporösen, mit VPA behandelten Patienten, an Partialanfällen mit komplexer Symptomatik litten und keiner eine generalisierte Epilepsie hatte und die Beobachtung, daß die Störung des Bewußtseins mit fokalen Symptomen und EEG‐Zeichen begann, die den spontan auftretenden Partialanfällen ähnelten, läßt vermuten, daß das VPA alleine oder in Kombination mit ander
