This article describes the diagnostic algorithm being used for the management of the 148 families affected by Duchenne or Becker muscular dystrophy who are known to the Molecular Neurogenetics Laboratory in the Department of Neuropathology, Royal Perth Hospital. In 60 families from whom DNA has been obtained, 41 mutations (39 deletions and two duplications) of the Duchenne muscular dystrophy gene (DMD) have been identified by means of complementary DNA (eDNA) probes. DNAbased screening has clarified the carrier status of 45 at‐risk women, and 13 pregnancies have been monitored. In addition, eDNA screening of all relevant patients with autosomal recessive muscular dystrophy, spinal muscular atrophy or limb‐girdle muscular dystrophy facilitated the correct diagnosis of Becker muscular dystrophy in three patients.

We reviewed 36 cases of symptomatic malignant pericardial effusion managed with pericardiocentesis at our institution from 1982 to 1989. There were 13 men and 23 women, aged 49 ± 12 years (range, 33–76 years). The commonest underlying tumours were lung cancer (12 cases, 33%) and breast cancer (11 cases, 30%). Pericardiocentesis was successful as the initial management in 34 of 36 patients (94%); one patient died as a result of the procedure and another required subxiphoid incision and tube drainage of the effusion. When intrapericardial sclerotherapy was performed, only three of 28 patients required repeat pericardiocentesls, and when sclerotherapy was not performed Initially, four of seven patients had recurrent symptomatic effusions. Median survival following pericardiocentesis in breast cancer patients was 10 months (range, 0–36 months) and in all other malignancies was four months (range, 0–12 months). We conclude that perlcardlocentesis with intrapericardial sclerotherapy provides good local control for symptomatic malignant pericardlal effusion in the majority of patients. In spite of this, the median survival of such patients is poor, especially in patients with malignancies other than breast cancer, with few patients surviving more than a few months.(Med J Aust 1991; 154: 19–22) Received 1 Jun 1990 Accepted 6 Aug 1990

The relationships between different levels of participation in physical activity and biological indices of cardiovascular risk were examined in a sample of 6814 male and female adults who took part in a national risk factor prevalence survey. Participation in physical activity was classified as “aerobic” (14.6% of total), “moderate” (53.6%) or “inactive” (31.8%). Bivariate analyses found significant associations between level of exercise participation and diastolic blood pressuresIn men, but not in women; significant associations were found between reported physical activity and systolic blood pressure, total serum cholesterol, high‐density lipoprotein (HOL) cholesterol, HOLltotal cholesterol ratio and trlglycerides for both men and women. Further analyses were controlled for age, education, and the survey site, which were potential confounders of the association between physical activity and other coronary heart disease risk factors. After adjustment, significant associations remained between levels of exercise participation and HDL cholesterol level, HOLltotal cholesterol ratio and serum triglyceride levels for both sexes. In women, there was also a significant relationshipbetween activity level and bodymass index. In addition, for men only, there was a significant inverse relationship between physical activity and total cholesterol, and, for older men, between physical activity and systolic blood pressure. It is well recognised that physical activity has an independent effect on reducing the risk of coronary heart disease. The resultsof thisstudysuggestthat there may be small supplementary effects mediated through the relationship between exercise and other biological cardiovascular risk factors.
