Acute ischemia of the brain induces a cascade of biochemical and physiological events. The final consequences depend on the fact whether ischemia is of transient or permanent, total or partial nature. Alteration of extracellular potassium concentration, intracellular calcium and potassium concentration, development of cytotoxic and vasogenic edema, postishemic hyperfusion and no-reflow phenomenon are important factors which decide about the final fate of functional capacity. CO2 reactivity, autoregulation and hemorheology must be considered when therapeutic approaches are used to influence basic flow during ischemic condition. At present there exists no therapy which has been fully accepted and is able to guarantee benefit to the hypoperfused tissue. Since the calcium metabolism is altered by ischemic processes, substances which act on this metabolism might be of value in the treatment of ischemia and its consequences. However, their beneficial effect on cerebral infarction has not been proven yet. In subarachnoid hemorrhage and migraine calcium antagonists are used to prevent and treat ischemia. In epilepsia calcium overload blockers have been tried by one group with promising results.

Isolated abnormalities of pursuit gaze were studied in 8 patients with unilateral acute (hemorrhage) or chronic (tumor) lesions of the parieto-occipital lobes. Ipsilateral pursuit was impaired in all patients, being superimposed with cogwheel saccades. Foveal pursuit was more disturbed than full-field pursuit. Impairment of contralateral pursuit was observed in all patients, but was severe only after acute involvement of the nondominant hemisphere. These findings provide further evidence for some asymmetric organization of hemispheric command of ocular movements. In these patients with hemianopia, the bilateral alterations of pursuit gaze probably correspond to an inter- and intrahemispheric disconnection syndrome.

The concentrations of aliphatic amino acids (glycine, serine, valine, threonine, alanine, leucine, isoleucine) in lumbar cerebrospinal fluid from patients with consciousness disturbances and normal controls were measured. Thyrotropin-releasing hormone (TRH) was administered to patients and the changes of levels of consciousness and amino acid concentrations were followed before and after TRH administration. The concentrations of glycine and serine from patients were significantly higher than those from controls; they returned to the control values with improvement of consciousness disturbances. The influence of TRH upon these amino acid concentrations was not observed. These results suggest that within the central nervous system, an alteration of the level of consciousness is associated with a distortion of glycine and serine metabolism and that TRH itself has no influence upon these amino acid concentrations.

An early afternoon polysomnographic recording was performed in 112 epileptic patients in order to better define their epileptic disease. 96 subjects (85.7%) slept spontaneously and sleep EEG tracing resulted in diagnostically useful data in 47.8% of cases, a significantly higher percentage than that obtained from tests performed in basal conditions. EEG activation, mostly linked to light NREM sleep stages, was greater in infanto-juvenile subjects than in adults.

Calcium ions play a major role in the regulation of cell functions. Minor alterations of the cellular calcium ion homeostasis lead to major functional aberrations. Alterations of the calcium homeostasis occur under conditions of hypoxia, the major cause being an extracellular increase of potassium ions which gate voltage-dependent calcium entry. A hypothesis is forewarded which tries to find a link between parenchymatous and vascular reactions to hypoxia and which offers an explanation for the spread of hypoxic damage to neighboring normoxic cells.
