We have compared the frequency with which various drugs were used in patients with cerebrovascular disease in five countries (UK, Italy, Indonesia, Spain and Yugoslavia). There were very large variations in the use of anti-oedema agents, ‘cerebral vasodilators’, and vitamins, which were not explained by differences in the populations studied. This variation has probably less to do with any scientific validation for the various treatments, which in most cases does not exist, than with medical ‘fashion’ and commercial pressures. Large variations in prescribing habits for what is thought to be the same disease are undesirable, and reflect a dearth of facts and adequate clinical

A case of carpal tunnel syndrome caused by a posttraumatic neuroma-in-continuity of the median nerve is reported. Such a cause of median nerve compression is rare.

The data of 772 patients were analyzed for albumin, immunoglobulins, glucose, and cell counts in cerebrospinal fluid and serum in healthy controls and in patients with various neurological disorders. Using laser-nephelometric end-point determination the IgG/albumin index was established for controls; a value smaller than 0.85 was considered normal. This index was used to distinguish between impairment of the blood-brain barrier and thecal synthesis of immunoglobulins. No correlation was observed between glucose concentration and cell counts in cerebrospinal fluid. Together with the IgG/albumin index, determination of IgA and IgM increased the sensitivity to detect multiple sclerosis from 43 to 58%.

We studied the action of high doses (500 mg/daily for 7 days) of methylprednisolone sodium succinate in 12 patients with supratentorial intracranial tumors. 10 tumors were malignant (5 gliomas and 5 metastases), and 2 benign (meningiomas). Clinical improvement ranged from moderate to marked after 24–48 h of therapy. In the 5 metastases, mean reduction of peritumoral edema was 27%, and in apparent tumor volume was 18%; in the gliomas, corresponding reductions were 31 and 15%. 1 of the meningioma cases showed a decrease in edema volume of 21%. These results indicate that methylprednisolone, at least for a short period of time, produces a definite decrease in apparent tumor size, in addition to the reduction of peritumoral edem

In 7 patients with symptoms of cerebrovascular diseases a somatosensory evoked potential (SSEP) chronotopogram was constructed after median nerve stimulation of seven different EP components: N20, P25, P45, P100, N140, P200 and P300. We examined only a limited number of patients (and controls) because the methodology is very time-consuming. We analyzed the topography in these patients in order to obtain insight in the effect of this disease on EP distributions and to find arguments in favor of the use of this methodology in clinical examination. These chronotopographical maps were constructed on the basis of the localization of maximal amplitude of the different components and their dipole counterparts (if present). 3 patients showed a delay of the specific components, and in 1 patient most of the non-specific components were delayed. The chronotopographical representation in most patients was abnormal. Relations with clinical state and computer tomography (CT) are described. Focal hypodensity on the CT gives rise to a large extension of the area of the specific component N20, overruling the dipole counterpart P20. Slight cortical atrophy in 4 patients causes abnormalities in the P300/N300 dipole direction. Chronotopograms give new information. Therefore, it seems justified to improve the method and implement it for clinical use in the near future.

We have estimated the levels of partially purified multiple sclerosis (MS) specific glycoproteins (MSG2) in gray matter, white matter, spinal cord and brain stem of MS autopsy brains by an enzyme immunoassay in which antisera to a pooled MSG2 was employed. Similarly, the levels of KG2 (non-MS brain isolate corresponding to MSG2) were assayed in various topographic areas of non-MS autopsy brains. Comparison of data on MS and non-MS groups revealed that the MS specific antigens may be preferentially located in the normal appearing white matter of MS brains. The concentration of MS antigens was found to be highest in the normal appearing white matter of MS brains, followed by periplaque and finally plaque. These results indicate that the MS specific antigens are not the degraded products of MS brains. They further support the view that normal appearing white matter of MS brains should be the target tissue for the isolation of MS specific antigens.
