We report a patient who in the course of 3 years developed 3 distinct episodes of an acute demyelinating neuropathy, each fulfilling the criteria set up by an ad hoc committee of the National Institute of Neurological and Communicative Disorders and Stroke for acute Guillain-Barre syndrome. Each episode was associated with strong serological evidence for lupus erythematosus. The episodes were separated by 1–2 years and each was followed by near complete clinical recovery. The only systemic feature was a pleural effusion during the last 2 episode

Clinical and radiological findings of 10 breast cancer patients with meningeal carcinomatosis were recorded. Special attention was drawn to CSF findings. Initial CSF cytology revealed malignant cells in all cases with a background of inflammatory cells in 7. Oligoclonal bands were found in the CSF of 3 out of 5 cases examined. Repeated CSF control under therapy showed elevation of lactate to be a better diagnostic parameter than lowered glucose. Six patients responded well to CNS radiation therapy and intrathecal chemotherapy. Median duration of survival was 6.4 months.

Two cases of lithium neurotoxic effects are reported. Both cases are expressed by choreoathetotic movements and in both of them intoxication was caused by combined drug therapy. In our opinion caution is advised when lithium is administered in combination with drugs, especially in elderly patients.

We describe a patient with chronic asymmetric motor neuropathy, which began in the upper extremity. The paretic muscles showed abundant fasciculations and myokymia but only little amyotrophy. Electrophysiologic examination revealed (1) proximal multifocal persistent conduction block (CB) not located at the usual entrapment sites, and (2) arrhythmic isolated or grouped fasciculation potentials originating distally on blocked axons. Over the years, new CBs developed, which led to tetraplegia, and amyotrophy slowly increased with progressive denervation. This patient differs from the cases of chronic acquired demyelinating polyneuropathy described in the literature by the absence of sensory deficit and the proximal location of CB.

A case of peripheral neuropathy due to styrene is described. The diagnosis of peripheral neuropathy was confirmed by a motor-nerve conduction study and sural nerve biopsy.

Thirty alcoholic with a mean age of 44.8 years were examined with the intravenous 133-Xenon method for measurements of regional cerebral flow (rCBF). The results were related to liver histology and hepatic function tests. While there exists no correlation between the rCBF values and hepatic function tests, an important connection between rCBF decrease and the severity of the histological liver analysis is shown. It is suggested that the hepatic biopsy is a good criterion for the long-term evaluation of alcohol impact upon the brain of a given individual and some physiopathological mechanism are discussed.

The authors describe a sample of 148 pregnancies in identified epileptic women from 2 distinct sources: questionnaires sent to a group of women, 15–45 years old, having had an EEG between 1976 and 1983, and a computerized registry involving all pregnancies occurring in 3 maternity wards in Lyon between 1979 and 1983. The analysis of drug therapy during early pregnancy showed that the most common regimen was monotherapy. Either in mono- or in polytherapies, phenobarbitone was the drug used most often, the second being valproic acid (67 and 25% of monotherapies, respectively). The epileptic mothers were younger than the controls, and had had fewer pregnancies. Sex and birthweight distribution did not differ from those of the controls. Twenty-six malformed infants (17.7%) were registered. Among them, 18 (70%) had minor defects only. A case of a 68-year-old female patient with hypokalemic myopathy is presented. Her persistent diarrhea due to radiation enteropathy with a total dose of 8,400 rad for the treatment of cervical cancer caused potassium loss in the stool. It finally induced generalized muscle weakness with hypokalemia as low as 1.6 mEq/1 with elevated CK 3,845 IU/1. The muscle biopsy of the left vastus lateralis demonstrated typical findings of hypokalemic myopathy. Radiation to the pelvic space can cause damage of the terminal ileum. Hypokalemic myopathy associated with radiation enteropathy is very rare in the literature.

The immediate effects of one week of Delta Sleep-Inducing Peptide (DSIP) administration (6 X 30 nmol/kg b.w., i.v.) and late effects after a follow-up period of one week were investigated in a sleep laboratory study. The population of 18 chronic psychophysiological insomniacs was subdivided into two groups of middle-aged (29–59) and older (60–83) insomniacs. In the middle-aged insomniacs, improvement of sleep to normal values occurred by the end of DSIP administration and was maintained during the follow-up week. In the elderly insomniacs, who showed an initial sleep disturbance double as severe as the middle-aged group, the immediate effect was larger, but full normalization of sleep was only obtained by the end of the follow-up period. Although there was an interaction of sleep pathology and age, a regression analysis yielded clear evidence that the effects of DSIP were correlated to the degree of sleep disturbance. Optokinetic nystagmus (OKN) was studied in patients with central visual field defects. Apart from a classic OKN, an inverted nystagmus was observed in some cases. A prerequisite for eliciting this paradoxic OKN was that the attention of the subject was directed to the field defect. Although other factors seem to be co-determining, a central mechanism is thought to process the visual information according to spatial-selective attention.
