In addition to confirming previously reported findings related to changes in high-energy phosphates consequent to anoxia, 31P nuclear magnetic resonance studies of rat brain indicated that maintenance of energy-rich phosphates is less effective in the aged brain than in the mature cerebrum.

This article argues against the general idea that the patient described by Gowers as distal myopathy in 1902 may have suffered from myotonic dystrophy. A case of distal myopathy with markedly atrophic sternocleidomastoids is presented. Patients of distal myopathy with absence of sternocleidomastoids, sometimes accompanied by facial weakness, are recorded in the literature. All these cases are sporadic and may be called distal myopathy of Gowers, which is different from Welander’s case

Internuclear ophthalmoplegia (INO) is characteristic of lesions of the medial longitudinal fasciculus and usually indicates primary brain stem pathology. Here we describe 2 cases of tuberculous meningitis (TBM) in which INO was a prominent presenting sign. The mechanism of INO in TBM is presumed to be ischemia secondary to involvement of small basilar vessels supplying the MLF in the inflammatory process. In both cases the INO resolved on antituberculous therapy suggesting that the cause of the ischemia is reversible. INO in a patient with meningitis must suggest a basal process probably TBM.

Report of a case of typical bobbing occasionally interpolated by reverse bobbing. The causative lesion, which lay in the most dorsal median portion of the pontine tegmentum, was a small primary hemorrhage, diagnosed in life by CT scanning and confirmed at necropsy. The pathological and clinical correlations of the phenomenon are discussed.

The clinical and computerized tomodensitometric data and postmortem examination findings from a case of xanthogranuloma of the third ventricle, producing hydrocephalus, are described. A review of 8 recently reported cases resembling ours shows that, on clinical and radiological grounds, third ventricle xanthogranulomas are usually indistinguishable from colloid cysts. From an embryologic and pathologic point of view, these entities seem also to be closely related.

A 58-year-old women with long-standing hemifacial spasm experienced dramatic relief of symptomatology while receiving baclofen (Lioresal®) in a daily dosage of 37.5 mg. Clinical response was noted within 48 h and she remained asymptomatic during 12 months of treatment. No toxic or side-effects were observed. Controlled studies are needed to establish the role of baclofen in hemifacial spasm

The results of prospective noninvasive cardiologic investigations, including echocardiography and Holter monitoring are described in 2 siblings with juvenile ceroid lipofuscinosis of the Spielmeyer-Vogt-Sjogren type. In the elder patient, echocardiography revealed ventricular hypertrophy with slowed ventricular relaxation. Holter monitoring showed not only bradycardia but also slow and fast ectopic atrial rhythms, sinus arrests and complex ventricular ectopic activity including ventricular tachycardia. In the younger patient the findings were less severe. These functional disturbances due to cardiac involvement, never reported before in this disease, are discussed.

A man with Fabry’s disease, who died at 52, suffered from lancinating limb pains between the age of 20 and 27, and from severe arthralgia between 20 and 40. The sural nerve showed a severe loss of fibers (1,614 myelinated fibers/mm2), chiefly affecting the small myelinated fibers. Inclusions of both homogeneous and lamellated appearance were present in the perineurial cells, fibroblasts and vessel walls. Teased fibers displayed moderate remyelination, regeneration and the presence of globular focal myelin thickening. The transient time course of painful manifestations in this case is stressed in view of a possible pathogenesis of pain in Fabry’s disease. It appears that small-fiber neuropathy is not directly related to the occurrence of pain symptoms, since the prevalent loss of small myelinated fibers was seen in the absence of actual pain. The small sensory neurons in the dorsal root ganglia, that are especially affected by lipid accumulation, probably give rise to abnormal discharges producing pain via the prevalence of the small-fiber versus the large-fiber input to the dorsal horn of the spinal cord (gate control theory). In this report, the temporal sequence of myopathology during a clofibrate-induced muscular syndrome and its recovery in rats is described using histopathologic, histochemical and electron microscopic technics. Two stages of myopathology are described: an early stage characterized by myopathic features, and a later stage characterized by neurogenic features. Although muscle histology returns to normal after 1 week of discontinuing the drug, electron microscopic abnormalities persist through the second week. It is proposed that clofibrate, in addition to its myotoxic effect, may be neurotoxic.
