Two cases of malignant tumours of the nerve sheaths are reported. In the first patient, who had von Recklinghausen’s disease, light- and electron-microscopic investigations showed an undifferentiated mesenchymal tumour of the radial nerve with the property of producing collagen fibres, an aspect characteristic of sarcoma of the fibroblastic type. In the second case a pleomorphic cell sarcoma of the cranial nerves of the left cerebellopontine angle was present. The morphologic aspects of these tumours show them to be primary mesenchymal tumours of the nerve (neurofibrosarcoma, neurogenic sarcoma), originating in the fibroblastic-like elements of the nerve sheaths. A clinical example of an acute cerebellar infarct mimicking an acute end-organ labyrinthine disease is reported to foster general familiarity with the simulation of a benign disorder by a potentially serious one. The utility of certain neurologic findings in distinguishing a peripheral from a central vestibulo-cerebellar disorder is discussed.

Lymphocyte subpopulations from patients with myasthenia gravis were evaluated during chronic steroid therapy. A marked lymphocytopenia (10–70%) was initially (day 3–21) noticed as well as a preferential depletion of thymus-derived cells which parallelled the clinical deterioration. Thus there was a relative increase in B cells, although their absolute numbers remained rather constant. These changes were reverted within 2–4 weeks in spite of continuous treatment. Superior orbital fissure syndrome is a symptomatologic complex, consisting of retro-orbital pain, paralysis of extraocular muscles, impairment of first trigeminal branches and frequent involvement of the optic nerve. From a review of 130 published cases including two personal observations, it appears that the clinical subdivisions and the several eponymic differentiations of this syndrome are unjustified on the basis of etiologic, therapeutic and prognostic elements. Only the presence – or absence – of optic signs may allow to hold the clinically useful distinction between the often ‘benign’ superior orbital fissure syndrome without optic nerve involvement, and the orbital apex syndrome where orbital exploration may be recommended. This review concludes that repeated neurosurgical and neuroradiologic investigations – with the exceptions of carotid angiography and orbital phlebography – can be avoided in

Human muscle cell growth was studied to investigate the nucleo-cytoplasmic ratio during development and ageing. The mean muscle fibre size, as well as the number and size of myonuclei were estimated in human muscle biopsies from normal individuals whose ages ranged from 1 to 71 years. Changes in muscle fibre size were accompanied by similar changes in nuclear number, the size of which remained the same. This resulted in a constant nucleo-cytoplasmic ratio during the age range studied. The role of the satellite cell in the maintenance of this ratio was discussed.

The authors report the case of a 67-year-old man, affected by diffuse spasms evoked by ischemia and voluntary and reflex movements, electromyographically characterized by an intense repetitive activity. There was also a modest and diffuse muscle wasting, with lower limbs areflexia; no myokymia. The EMG showed severe neurogenic changes and a considerable reduction of the MMCV in the lateral popliteal nerve area.

Dipropylacetic acid (DPA), γ-aminobutyric aicd (GABA), physostigmine, CB 154 and butyrophenones were administered to 26 patients affected by Huntington’s chorea. The evaluation of the pharmacologic activity of the different drugs was determined by means of some clinical parameters and motor performance tests. Butyrophenones succeeded to ameliorate the hyperkinesias and the motor dexterity, CB 154 provoked a worsening of the motor signs of the disorder, while DPA, GABA and physostigmine did not modify the clinical pattern. Nonketotic hyperglycemia has a definite convulsive effect, which may manifest itself in generalized or focal seizures. This report deals specifically with focal convulsive phenomena. Two patients with nonketotic hyperglycemia focal seizures are described. Focal seizures are of motor Jackson, aphasic, adversive or of the ‘epilepsia partialis continua’ type. Different types of focal seizures may appear in one and the same patient. Focal epilepsy can be the first manifestation of a diabetic disorder. Focal epileptic seizures are linked with moderate nonketotic hyperglycemia with values of 360 mg% sugar and 310 osm. Higher values lead to generalized seizures and coma. The glycolytic substrate utilization and energy consumption of hemispheres of adult mice were studied by converting the heads into a closed system by decapitation. It was concluded that progressive insufficiency of renal function is parallelled by an increase in brain permeability for glucose and a progressive reduction in brain energy metabolism.

The urinary clearance of pyridostigmine was studied in six patients with myasthenia gravis. In three patients on pyridostigmine alone, renal clearance ranged from 349 to 481 ml/min, corresponding to a pyridostigmine:creatinine clearance ratio of 2.64 to 3.46. In a patient on bendrofluazide as well as pyridostigmine, a similar clearance ratio was observed. By contrast, the urinary clearance of pyridostigmine and the pyridostigmine creatinine clearance ratio was reduced in two myasthenic patients concurrently treated with other basic drugs. It is suggested that these results may reflect competition for renal tubular excretion.
