The case of a right-handed man with pure alexia without hemianopia due to intraparenchymal haematoma is reported. Ct scan and nuclear magnetic resonance showed a lesion in the lateral temporo-occipital area confirming the assumption that the anatomic pathways responsible for reading run close to the lateral temporo-occipital or inferior temporal gyrus.

Two patients with subacute sclerosing panencephalitis (SSPE) were treated intramuscularly with recombinant DNA interferon-a2 for 3 and 6 months, respectively. Side effects were minimal. No objective clinical response could be noted. In a patient with stable SSPE, a significant decrease in serum antimeasles antibody levels was observed. This decrease was less pronounced in another patient with evolutive SSPE. The results of this study were compared to findings published previously. The reasons for the lack of clinical efficacy of interferon in SSPE are discussed.

The creatine kinase B (CK-B) subunit was determined by our own highly sensitive solid-phase direct immunoassay in three neurological diseases which represent models of central nervous system (CNS) involvement: (1) idiopathic epilepsy (18 patients), a disease characterised by a transient neuronal hyperactivity; (2) Parkinson’s disease (17 patients), a degenerative disorder of the CNS, and (3) multiple sclerosis (21 patients), which represents a model of demyelination of the CNS. A group of 50 controls was also studied. The results showed that in patients with epilepsy and multiple sclerosis the CK-B activity was considerably lower than in the controls (p < 0.01 and p < 0.005, respectively), while no differences were found between the controls and the patients with Parkinson’s disease. It is suggested that the low CK-B activity observed might be due to the medication taken or to the disease process itself. The octavus nerve neurovascular compression syndrome is a neuropathy of the 8th cranial nerve caused by vascular compression. The clinical, neurophysiological and therapeutic aspects of this syndrome will be discussed on the basis of 10 patients.

We report a case of acute multiple sclerosis in whom computed tomography (CT) scan suggested the presence of a large plaque. The anatomical and functional recovery of the brain tissue was followed by means of repeated CT scan and 123I-HIPDM studies. Single photon emission CT may be successfully employed to monitor the pathophysiological changes associated with the demyelinating process.

Examination by magnetic resonance imaging (MRI), evoked potentials (EP) and cerebrospinal fluid (CSF) analyses was carried out on 97 definite, 20 probable and 40 possible multiple sclerosis (MS) patients (McAlpine’s clinical criteria). MRI of only 4 transverse brain sections at the level of the ventricles and the analysis of the first 4 echoes showed periventricular or parenchymal lesions, or both, in 114 of the 117 definite and probable MS patients and in 25 of the possible MS patients. A 67-year-old man presented with recurrent clinically diverse posterior circulation transient ischemic attacks which continued despite full anticoagulation. Giant cell arteritis was suspected because of a markedly elevated erythrocyte sedimentation rate and confirmed by temporal artery biopsy. Treatment with corticosteroids resulted in a prompt and enduring resolution of clinical symptoms. Giant cell arteritis may represent a treatable cause of posterior circulation transient ischemic attacks, uniquely responsive to corticosteroids.

Repeated cystometry was performed in 17 patients after a cerebrovascular accident. Cranial computerised tomography (CT) was performed in all patients. In this series, urinary continence showed a significant correlation both with cystometric bladder tone and with the CT brain findings. The most favourable prognostic factors for urinary continence were a non-hypertonic bladder and absence of extensive brain lesions.

The present report evaluates the suppression of antiepileptic drugs in a group of 608 epileptics. Conditions for the suppression of antiepileptic treatment included a previous seizure-free period of 5 years under medication, and a careful evaluation of individual social and work-related circumstances. The reduction of medication was realized during a minimum period of 1 year, passing from eventual polytherapy to monotherapy. Of the total 608 epileptics in whom treatment was suppressed, 144 relapses (23.7%) occurred, and 464 (76.3%) patients continued to be seizure-free. A complete remission of the epilepsy was achieved in 14.3% of 3,254 epileptics with an adequate follow-up. Significant risk factors for recurrence included a delay in starting anticonvulsant therapy; symptomatic generalized epilepsy; different types of combined seizures, or atypical absences and/or tonic crisis and/or atonic crisis; or partial complex associated with secondary generalized tonic-clonic seizures; the presence of status epilepticus in the course of the disease and permanent neurologic damage. With the patient’s prior agreement, it was possible to suppress anticonvulsant medication in one fourth of nonselected epileptic patients with a possibility of relapse in about 25% of the cases (generally one isolated seizure
