AbstractThe present study was designed to test the relative effectiveness of behavioral (B) and cognitive‐behavioral (CB) group therapy programs in the treatment of 41 bulimic women. Subjects were randomly assigned to the two treatment conditions and a waiting list control (WLC). Multiple behavioral and psychological measures were administered at pretreatment, posttreatment, and 1‐ and 3‐month follow‐up. Results indicated that both treatments were superior to the WLC in decreasing the frequency of binge eating and the use of extreme weight control methods at post‐treatment. However, only the B group was able to maintain reductions in binge eating throughout the follow‐up period. In terms of psychological measures, both treatments evidenced significantly decreased bulimic tendencies and body dissatisfaction and significantly increased feelings of adequacy, security, and control which were maintained at follow‐up. Only the CB group evidenced additional psychological changes in preoccupation with dieting and severity of psychopathologi‐cal symptoms which were maintained at follow‐up. AbstractThough DSM‐III‐R equates several different forms of purging behavior, including use of vomiting, laxatives, diuretics, dieting, and exercise, there is little work that has examined patient subgroups based on divergent forms of purging. An attempt to investigate subgroups based on purging criteria was thwarted because of low base rates for specific purging behaviors, despite having a relatively large clinic sample of bulimia nervosa patients (n= 245). As an intermediary step to the investigation of functional equivalence of different forms of purging, we propose classifying patients based on the number of purging behaviors they employ to control weight. Our findings suggest that patients who employ more than one strategy are generally more disturbed on a number of psychiatric indices, including state and trait depression. This contrasts with recent findings that the frequency of any one purging behavior is unrelated to level of depression or course of treatment in clinical samples. This study suggests that clinicians who must evaluate and effectively triage bulimia nervosa patients would benefit from utilizing the number of purging strategies rather than the frequency of any one purging behavior as an indication of severity and possible comorbid

AbstractA review of the literature on normal development and on psychiatric disorders related to seasonal variation, suggests that this phenomenon is probable to occur in anorexia nervosa. A search of the available literature is entirely negative with respect to seasonal variation in anorexia nervosa. Preliminary analysis suggests the existence of a season of onset, a season of birth, and also oi a season of admission in Danish female anorexia nervosa patients. A formally correct analysis for seasonal variation supports the existence of a season of onset in a selected sample, maximum is in May, amplitude 50%. A season of birth is found in an unselected nationwide sample of patients aged 15 years or older at first psychiatric admission for anorexia nervosa. Maximum late in April, amplitude 40% in the period 1963–1968. Season of admission could not be modelled as a sine wave, but the data suggested a pulse structure (a minimum in one month, followed by a maximum in the next month). Two annual pulses: December/January and July/August. Peak of onset is at a time of the year with maximum increase in height, and peak of other psychiatric conditions. Pattern of birth is similar to that reported for schizophrenia and mania. Replications of this preliminary study should be done in larger countries, at different latitudes and continents. AbstractThe prevalence of bulimia (as defined by DSM‐/I/ [American Psychiatric Association 1980.Diagnostic and statistical manual of mental disorders(3rd ed.). Washing‐ ton, D.C.: Author]) was estimated by structured interview in a random sample of 2,715 adults in the general population, aged 18‐96. Prevalence was 1.1% for the total sample, and among women aged 18‐30, 4.1%. Bulimic behaviors and symptoms were more common among women than men, younger than older respondents, and, on some items, lower socioeconomic status (SES) than higher SES respondents. AbstractThis paper describes the treatment of a series of patients with bulimia nervosa who also had insulin‐dependent diabetes mellitus. The aim was to discover what modifications to an established cognitive behavioral treatment for bulimia nervosa proved necessary to accommodate the presence of diabetes. Treatment of the patients proved more difficult than that of comparable nondiabetic patients. Possible reasons for the difficulties encountered are discussed, together with strategies for overcoming them. It is concluded that cognitive behavior therapy can be successfully modified for the treatment of such patients, although adjunctive psychological and physical treatment techniques are required in some instances. Treatment usually results in improvement in both eating habits and glycemic control, and is likely to reduce the risk of future diabetic complications. AbstractThe severity of eating disorder symptoms and interest in weight reduction vary among cultures and nationalities. The present study is the first to directly compare the extent of these problems between two Western nations. Although several reports have emerged from other European countries, this is the first study of eating disorder symptoms among Spanish youth. There were more American girls with bulimia nervosa symptoms; the overall level of eating disorder symptoms was higher among the Americans; and more American girls were interested in losing weight. Bulimia was equally rare among Spanish and American boys, American boys were more interested in changing their weight, and Spanish boys had higher scores on a questionnaire of eating disorder symptoms. Although statistically different, subjects in both countries scored close to the norm on the eating disorder questionnaire.

AbstractThe Bulimic Investigatory Test, Edinburgh (BITE) is a self‐report measure, claiming to differentiate binge eaters and comparison women on the basis of bulimic symptoms. The present study reports on the clinical utility of this measure in differentiating subgroups of binge eaters. The BITE is found to be unable to categorize anorexics of the bulimic subtype, and its accuracy in detecting other types of bulimia is poorer than has been described by previous research. AbstractThe Body Shape Questionnaire (BSQ) is a 34‐item self‐report questionnaire that measures the degree of body shape dissatisfaction. To date, the BSQ has not been used with adolescents. The present study compared the BSQ scores of five adolescent subject samples: Anorexia Nervosa (AN), Bulimia Nervosa (BN), Subclinical Bulimia Nervosa (SB), Subdinical Anorexia Nervosa (SA), and non‐eating‐disordered adolescent females (Q. Results show that patients with BN have the highest levels of body dissatisfaction. All clinical groups had higher BSQ scores than subjects in the comparison sample but only the BN patients had significantly higher scores. The mean BSQ score for the Comparison group was higher than published means for non‐eating‐disordered adult samples.
