AbstractThe reported increase over the last decades in the incidence of eating disorders seems to be accompanied by a shift in the symptomatic spectrum, so that the clinician now more often comes in contact with individuals not matching the picture of the “typical” anorexia nervosa patient. Seventy‐nine patients assessed at the clinic over the last two and a half years were studied with respect to the differential diagnostic problems arising in this context. A considerable overlap in symptomatology was found between all sub‐ categories (anorexia nervosa, bulimia, double diagnosis of anorexia nervosa/bulimia according to the DSM‐III, and “anorexic‐like”). Furthermore, 49% had previously passed through at least one phase when a diagnosis other than that currently applicable would have been appropriate. These results, in combination with similar observations reported several times in the literature, imply the existence of some common feature among all the patients. In the ABC‐model, a conflict between anorectic and bulimic impulses is suggested as a concept capable of expressing this commonality while still representing a phenomenon identifiable at a clinical‐descriptive level. AbstractThe development and validation of a new measure, the Eating Disorder Inventory (EDI) is described. The EDI is a 64 item, self‐report, multiscale measure designed for the assessment of psychological and behavioral traits common in anorexia nervosa (AN) and bulimia. The EDI consists of eight sub‐scales measuring: 1) Drive for Thinness, 2) Bulimia, 3) Body Dissatisfaction, 4) Ineffectiveness, 5) Perfectionism, 6) Interpersonal Distrust, 7) Interoceptive Awareness and 8) Maturity Fears. Reliability (internal consistency) is established for all subscales and several indices of validity are presented. First, AN patients (N = 113) are differentiated from female comparison (FC) subjects (N = 577) using a cross‐validation procedure. Secondly, patient self‐report subscale scores agree with clinician ratings of subscale traits. Thirdly, clinically recovered AN patients score similarly to FCs on all subscales. Finally, convergent and discriminate validity are established for subscales. AbstractThe present study sought to objectively assess body size estimates in subjects not currently undergoing treatment for eating disorders. Two hundred and twenty‐three subjects were asked to rate (a) whether they perceived themselves as underweight, normal weight, or overweight and (b) the number of pounds they felt they were from their ideal weight. These perceptions were then compared to their actual weight. Additionally, subjects were asked to rate their attractiveness, how well they were liked, and how frequently they dated. Results suggest that both overweight males and females significantly underestimated the degree of their obesity, although females were more accurate than males in their ratings. Further, in the normal and underweight groups, females felt they were significantly more overweight than they actually were. AbstractThe MMPI profiles of thirty‐nine outpatient females were contrasted by diagnosis using DSM III criteria for bulimia and anorexia nervosa. The study sample was divided into three groups: restricting anorectics, bulimic anorectics and normal‐weight bulimics. All three groups were highly correlated with each other (D2=.841 —.945). Although depression was the peak scale for both anorectic groups, the bulimic anorectics were more depressed. The psychopathic deviate scale was the peak scale for the normal‐weight bulimic sample. AbstractPrevious research has suggested that inappropriate stimulus control and psychological stress may exercise functional control over eating disturbances such as binge‐eating and self‐induced vomiting (bulimia). This report is a single‐case study of a 26‐year‐old bulimic female in which these factors were hypothesized to be functional elements. The intervention used a self‐reinforcement program to establish appropriate stimulus control over proper nutritional habits (Stimulus Narrowing). Stimulus Narrowing was successfully implemented utilizing self‐reinforcement, but Stimulus Narrowing did not reduce bulimia. Self‐control relaxation was then implemented and was followed by an apparent reduction in the frequency of bulimic episodes. The results suggest that the “treatment package” of stimulus narrowing, self‐reinforcement and self‐control relaxation may be useful in the treatment of bulimia. AbstractReport of a case, initially diagnosed as Anorexia Nervosa (AN) but later on revealing a fourth Ventricular Tumor is presented. Relevant literature is surveyed while drawing attention to the unusual location of the tumor, instead of hypothalamic area as customarily reported. Several years of duration between initial appearance of clinical signs and symptoms and discovery of the tumor require emphasizing the need to keep the diagnosis tentative and not to hesitate in following up any clues pointing to a probable organic base.
