摘要:
AbstractSeven studies using structured diagnostic interviews have assessed the prevalence of borderline personality disorder among patients with eating disorders, yielding rates from 0% to 42% in different samples. However, many patients with eating disorders suffer from depression, and there is substantial overlap between the symptoms of depression and those of borderline personality disorder. Thus, structured interviews for borderline personality disorder–most of which are of untested or uncertain specificity–may perhaps frequently produce false‐positive diagnoses of borderline personality disorder in patients with eating disorders and hence yield exaggerated estimates of the true prevalence of borderline personality disorder in this population. Considering the grave implications of the borderline diagnosis, one must remain critical of studies in this area until further data, using instruments of documented specificity, blind ratings, and suitable control groups, become avai

摘要:
AbstractThe families of 30 eating‐disorder patients were matched with 30 normal control families on social class, family size, and age and sex of the patient. We tested if the age (adolescent or young adult) and the symptomatology of the patient (restricting anorexic, bulimic anorexic, and normal‐weight bulimic) had a significant effect on a behavioral and a self‐report measure of cohesion, adaptability, and conflict. The age of the patient proved to be nonsignificant. The eating‐disorder families showed more stability and discussed less openly disagreements between parents and children. On the remaining family measures, the anorexic (restricting and bulimic) and bulimic families revealed a completely opposite interaction pattern. The anorexic family had interpersonal boundary problems and a stable and conflictavoidant way of interacting, which was experienced as nonconflictual and cohesive by the patient too. The bulimic family showed strong interpersonal boundaries, a less stable organization, and less avoidance of disagreements. The patient herself also described her family as conflictual, uncohesive, and badly or

摘要:
AbstractThe Family Environment Scale (FES), an instrument that evaluates several aspects of family functioning, was given to 114 young women subjects and a parent of each. Twenty subjects had restricting anorexia nervosa (AN), 13 had bulimic AN, and 24 had normal‐weight bulimia, whereas 57 were age‐matched controls. The score of each subject and her parent was averaged on each FES subscale to derive a family score. One‐way analyses of variance, used to compare the 4 subject, 4 parent, and 4 family means, revealed significant differences on 5 of the 10 FES subscales: cohesion, expressiveness, conflict, achievement orientation, and activerecreational orientation. These data, together with those of other investigators, suggest that the family environment in eating disorder families differs in important ways from that of normal families. The possible significance of these findings is disc

摘要:
AbstractAmong the diverse psychiatric entities with stagnating monothematic thoughts, anorexia nervosa occupies a significant place as a typical example of a “monoideistic” illness. The dominant idea (about food, dieting, weight loss, and body shape), repeated mentally over and over again and deeply rooted in family and other interpersonal transactions, can engender an hyponotic‐like state. Our interest in anorexia nervosa as one of the most typical monoideistic diseases gained in strength as we noted that in the course of the past 15 years there was an ever‐growing stream of referrals to the Kibbutz Clinic of adolescent girls with anorexia nervosa. For this reason we opened an anorexia nervosa treatment unit and have been able to carry out prospective followups on 66 kibbutz anorectic patients (40 restricters and 26 bulimics) over periods from 4 to 10 years. We were particularly interested in the answer to the question of whether the nosogenesis of the monoideistic pattern, the family and sociocultural factors, and the course and the outcome of the illness are different in the kibbutz population as compared with other s

摘要:
AbstractBody size estimation, body satisfaction, and feeling fat were assessed before and after meals on two occasions, in a repeated measures design, in 20 normal young women. A consistently negative self‐appraisal was identified, including low satisfaction, feeling fat, and overestimation of body size. Body image was more negative in restrained than unrestrained eaters and became more negative after eatin

摘要:
AbstractSixty‐one premenarcheal and 60 postmenarcheal female adolescents (ages 10–15) were measured for levels of body size estimation accuracy, body esteem, self‐esteem, depression, eating disturbance, and teasing history regarding appearance. Age‐matched subsamples of each group were also evaluated. There were few differences between groups on any of the measures, with the exception that postmenarche subjects had higher levels of eating disturbance. In general, there were significant relationships among eating disturbance, teasing history, depression, selfesteem, and body esteem. There was a trend for premenarche subjects to show stronger relationships between body esteem and other measures, whereas postmenarche females had higher correlations between size estimation indices and other variables. The findings are discussed with regard to the need to further investigate developmental issues in body image and eating disorders, particularly the salience of teasing as an etiological

摘要:
AbstractThis study examined the effects of short‐term educative/support groups and subliminal psychodynamic activation on bulimic symptomatology and psychological functioning in 27 female students. Subjects were randomly assigned to one of four conditions: (1) educative/support group plus experimental stimulus; (2) educative/support group plus neutral stimulus; (3) attention/placebo plus experimental stimulus; and (4) attention/placebo plus neutral stimulus. The educative/support group had significant ameliorative effects on binging and symptoms of emotional distress, regardless of whether or not it was paired with subliminal psychodynamic activation. Decreases in several psychological traits associated with eating disorders were found among all subjects irrespective of treatment conditions. Subliminal psychodynamic activation appeared to exert an independent treatment effect on restrained eating and certain symptoms of emotional distress. Clinical and theoretical implications are discussed and related to current proposed etiologies of bulimi

摘要:
AbstractEight patients with bulimia nervosa were treated using purely behavioral techniques in the absence of explicit cognitive procedures. All were systematically assessed before and after treatment; seven were reassessed at a 1‐year follow‐up. As a group, they showed substantial improvement in terms of eating habits, a global index of specific psychopathology, and mental state. These changes were comparable to those previously obtained in an Oxford sample of patients using a cognitive behavioral treatment. These findings raise the question as to whether cognitive procedures are necessary to produce change in patients with bulimia nerv

摘要:
AbstractA higher incidence of bulimia in diabetic populations than in the normal population has been reported. This study examined whether psychosocial similarities between bulimics and diabetics might exist, posing a psychological risk factor for bulimia among diabetics. Diabetics (n = 19), bulimics (n = 19), and controls (n = 19) were administered tests of eating attitudes, psychiatric symptomatology, and family environment. Few significant psychological similarities were found between the bulimic and diabetic groups. The diabetic and control groups exhibited less psychopathology than the bulimic group on almost all indices. Although both diabetics and bulimics were found to be more perfectionistic than controls, this alone is insufficient evidence of a psychological risk factor for bulimia in the diabetic population studied.
