AbstractThe present study examines the relation of life event stress to the occurrence of bulimia in young anorexia nervosa patients. Life events recorded in the 18 months preceding symptomatic onset were analyzed in 25 anorexics with bulimia and a matched group of 25 anorexics characterized by strict dietary control (restricters). Scores were derived by summing life change unit values for events experienced during successive six‐month time blocks and were subjected to repeated measures analysis of variance. Findings indicated that bulimics experienced significantly more life stress than restricters in each six‐month period (all ps<.001) and experienced 21/2times the magnitude of life stress observed in normal adolescents (Codding‐ton, 1972) for a comparable time period. Results are discussed with respect to biological, psychological, and cognitive processes of possible relevance to the genesis of binge eating in anorexic patients. AbstractTreatment of anorexia nervosa/bulimia patients with a chronic course who previously were treated is given little attention in the literature. At the Uppsala University Hospital, a special program was developed for this category. It is assumed that successful therapy requires (1) a combination of therapeutic modalities, (2) a stepwise treatment, (3) a combination of anorexia nervosa and bulimia treatment principles, and (4) a treatment goal that is subjectively valid for the patient. In the present article, “Phase 1” is described. This involves teaching eating control through detailed diet and activity regimens supported by daily talk sessions, body‐awareness therapy, and occupational therapy. “Phase 2” psychotherapy is thereby made possible. Results are promising in that a substantial proportion of patients were able to reach eating and weight goals not attained in previous treatments. AbstractThe separate and additive effects of exposure with response prevention (ERP) and energy balance training (EB) in the treatment of bulimia were evaluated in six female bulimics. A multiple‐baseline design was used in which exposure with response prevention was designed to reduce vomiting frequency and energy balance training was designed to reduce the frequency of food intakes. AbstractThis article contends that recent theoretical formulations of anorexia nervosa from an ego psychological and/or object relations perspective and the results of projective (and sometimes objective) psychological tests are convergent. Furthermore, greater understanding of anorexia nervosa has not been possible because of the lack of use of psychodiagnostic test data. The contribution of psychological test results has been overlooked for a number of reasons. The results of studies using objective tests have added little to our knowledge of anorexia nervosa in terms of either etiology or prognosis. Instead, they have been primarily descriptive studies of personality traits found in anorexics. There is a dearth of studies using projective tests and these are limited by methodological problems, including small sample size and differing evaluative emphases, such as conceptual‐perceptual dimensions, personality structures, defenses, and content. However, despite these limitations, the studies are strikingly consistent in that they point to ego weakness and/or defects. The former is presumed to be a hallmark feature of anorexia nervosa, at least as emphasized by ego psychological theorists. Further impeding progress in the development of a comprehensive theory is the lack of integration of experimental investigations of body image with the clinical and research literature; instead, they are often seen in isolation. Recent advances in developmental theory, the results of projective (and occasionally objective) tests and body image investigations are reviewed and found to be complementary and have the potential to offer a comprehensive theory of anorexia nervos

AbstractAn 88‐item questionnaire was used to assess the perceptions of bulimia held by bariatricians, psychiatrists, and clinical psychologists. The questionnaire focused on such elements as etiology, diagnostic criteria, treatment procedures, and time spent with client. Although the three groups of clinicians had similar views on many facets of bulimia, 29 MANOVA comparisons were statistically significant. Approximately 75% of the respondents rejected the definition of bulimia found in DSM‐III, but accepted many of its diagnostic criteria. AbstractNine patients with bulimia participated in a year‐long therapy group that combined behavioral and psychodynamic approaches. Eight of the patients also participated in individual psychotherapy and three had medication trials.
