SUMMARYWe have isolated the lowest region of human and pig stratum corneum as an integral layer which we have termed the stratum compactum. This preparation is resistant to disruption by enzymes, 6murea, Triton X‐100 and solvents. Our evidence suggests that all cells of the stratum corneum may be equally permeable to aqueous soluble materials but that penetration of materials through the corneum depends on the state of cohesion between cells and of the organization of intercellular lipid species. As the cells move up towards the outside of the stratum corneum the cohesive forces are reduced due to desmosome degradation and lipid modifications with ultimate dyshesion and sloughing of individual cell

SUMMARYEpidermal nuclear deposits of immunoglobulins (Ig) were studied by direct immunofluorescence in three groups of patients: ten scleroderma (SD, systemic sclerosis), seven dermatomyositis (DM) and seven systemic lupus erythematosus (SLE). Each patient had skin biopsies taken from three different sites (nailfold, forearm, buttock) on the same day that a serum sample was also obtained.Epidermal nuclear deposits were observed in nine of twenty‐four patients (five SD, two DM, two SLE). A high serum ANA titre correlated significantly with the presence of epidermal nuclear Ig deposits. The nucleolar epidermal nuclear pattern was limited to the SD group, four of ten patients showing this pattern. SUMMARYAntinuclear antibodies were studied by indirect immunofluorescence and double immunodiffusion in 21 patients with systemic sclerosis and 35 of their relatives. When HEp‐2 cells were used as the substrate, the frequency of antinuclear antibodies in the patients' sera was 100% and that in the relatives was 26%. When rat liver sections were used, the values were 86% and 17%, respectively. Anticentromere antibody was detected in the serum from the mother of one patient whose serum had anti‐Scl‐70 antibody. Antibody ton‐RNP was positive in the sera from the brother and daughter of another patient whose serum was positive for anti‐n‐RNP and anti‐Scl‐70 antibodies. The high frequency of antinuclear antibodies in the sera from the relatives of systemic sclerosis patients suggests that immunological abnormalities play a part in the pathogenesis of

SUMMARYMonoclonal antibodies to human α1‐microglobulin (α1‐m), a glycoprotein present in most biological fluids, reacted with the basement membrane of the epidermo‐dermal junction and that of eccrine sweat glands in normal human skin. This immunofluorescence was specific in that control antibodies of known well‐defined reactivity did not stain these structures. Therefore, α1‐m seems to be a new antigenic component of the junction and it may be an interesting antigen to study in diseases

SUMMARYThe modulating effect of captopril, an inhibitor of angiotensin‐converting enzyme, on the vascular permeability changes induced by intradermal injections of C5a, histamine and compound 48/80, was evaluated in guinea‐pigs. Cutaneous vascular permeability changes were measured by the extravasation of intravenously injected125I‐bovine serum albumin. Intraperitoneal injection of 12.5,25, or 50 mg/kg of captopril 30 min prior to the injection of C5a (10−11mol) significantly enhanced the increase in vascular permeability induced by this agent (P<0.02). This may be explained by the known property of captopril as an inhibitor of carboxypeptidase. No effect was observed when captopril was injected either 2 or 4 h before the injection of C5a. In contrast, the same doses of captopril, when injected intraperitoneally 2 h before the injection of histamine (10−8mol) or compound 48/80 (10 μg), significantly suppressed the increase in vascular permeability induced by these agents (P<0.02). This suppressive effect occurred in a captopril dose‐dependent manner. The ability of captopril to modulate the vascular permeability response induced by vasoactive agents indicates that it is a potentially useful tool to dissect the relative roles of mediators involved in inflammato

SUMMARYAlterations of binding capacity of surface IgG‐Fc and complement receptors were demonstrated in polymorphonuclear neutrophils (PMNs) obtained from the pustular lesions of psoriasis. A marked decrease of C3 receptors, but not of IgG‐Fc, was found in PMNs from the lesions of palmo‐plantar pustulosis (PPP) and bacterial pustules. PMNs from pustular lesions of psoriasis exhibited only a slight decrease in the number of C3 receptors. No significant decrease in membrane receptors was noted in circulating PMNs from psoriatic patients. SUMMARYIn twenty‐eight men with dermatitis herpetiformis (DH) the mean serum testosterone levels were within normal limits, but high levels were found in three patients with partial villous atrophy and two who had normal jejunal biopsies. The introduction of a gluten‐free diet in patients with villous atrophy produced a statistically significant fall in the testosterone level in those able to discontinue their drug treatment but not in those noticing no change or only a reduction in drug requirements. The serum levels of luteinizing hormone or follicle‐stimulating hormone were increased in nine patients; only one of these had a raised testosterone level and six were over 60 years of age. Four out of twenty married males with DH had no children, a figure higher than expected. Three of these men had elevated gonadotrophin levels although none had an increased serum testosterone. SUMMARYThe thin layer rapid use epicutaneous test (TRUE‐test) is a new patch test, in which the allergens are incorporated in a thin, flexible solid vehicle on a water impermeable backing. SUMMARYEtretinate and isotretinoin were compared with respect to their clinical effects and changes in serum lipoprotein concentrations.Sixteen patients with hyperkeratotic and pustular disorders of hands and feet (mainly palmoplantar pustulosis) underwent a double‐blind cross‐over study. The daily doses of etretinate and isotretinoin were 50 and 40 mg, respectively. Each drug was given for 2 months with a 2‐month intermission.The clinical score was reduced both by isotretinoin (P<0.05) and etretinate (P<0.001). Both drugs affected the lipoprotein concentrations. Isotretinoin increased the cholesterol concentration in low‐density lipoprotein (LDL) by 20% and the triglyceride concentration in very low‐density lipoprotein (VLDL) by 35%, but decreased the high‐density lipoprotein cholesterol by 12%. Etretinate elevated LDL‐cholesterol by 10%. SUMMARYTwenty psoriatic patients treated with etretinate have been followed in a prospective study of liver biopsies. Twelve patients were followed up for 3 years, with four liver biopsies each. No significant damage to the liver was found during etretinate therapy. Etretinate may be stored in the fatty tissues of the liver or other body areas for prolonged periods.
