This article reinforces the importance of pain management in the older adult population. It is important to first understand the complexity of pain in older adults as well as the myths health care providers may believe, which could lead to undertreatment of their pain. The older adult patient experiences many types of pain; in some cases, these individuals may have combined types of pain. Many medications can be given safely to older patients; nonopioids, opioids, and adjuvant medications can be tried for pain relief.

摘要:
This article updates information on the essential components of pediatric pain management in the Emergency Department: assessment, treatment, and education. It reviews pediatric pain assessment scales for developmental ages infancy through adolescence and makes recommendations for pain scale selection. It discusses pharmacologic and nonpharmacologic treatment modalities and presents the most common medications, including conscious sedation dosage guidelines. The article also reviews the role of the parent in assessment and treatment of the child's pain, emphasizing the role of education. Although many myths and barriers still exist, great strides have been made in pediatric pain management. These advances must be incorporated into each health care provider's practice to ensure maximum pain relief for all pediatric patients.

摘要:
Post dural puncture headache (PDPH) results from cerebrospinal fluid loss through a puncture site in the dura. Symptoms include headache, ocular disturbance, and auditory difficulty along with shoulder pain, double vision, tinnitus, and vomiting. Women age 20 to 29 are at greatest risk. Treatment may be conservative, administering oral opioid and nonopioid medication, bed rest, abdominal binder, and fluids, especially caffeinated beverages. Blood patch is the standard treatment, yielding relief in 95% of patients. Injection of 10 mL of normal saline into the subarachnoid space at the time of wet tap has shown promise. PDPH usually resolves spontaneously in 2 weeks; however, most patients opt for treatment.

It is intended for the use of all medical personnel in the treatment of neuropathic nociceptive and mixed pain states. It discusses antidepressants, anticonvulsants, local anesthetics, antiarrhythmics, steroids, N-methyl-d-aspartate (NMDA) blockers, topical agents, and muscle relaxants, with emphasis on special drug-drug interactions of which all practitioners should be aware.

Assessing pain in the patient with cognitive impairment is challenging but not impossible. Patients with mild to moderate cognitive impairment can successfully complete standard pain assessment tools. However, patients with severe cognitive impairment require other strategies to assess pain. This paper will discuss a variety of tools and methods that can be used to assess pain in the individual with any degree of cognitive impairment.

Pain is a frequent problem in any medical practice, whether associated with acute or chronic illness. Patients' inadequate pain relief can be attributed to multiple factors, including inadequate knowledge and understanding about pain medications, dosing regimens, and routes of administration. This article presents some practical treatment strategies to optimize opioid analgesic therapy and limit analgesic drug-induced toxicities.
