San Diego County's emergency medical services (EMS) agency has developed and implemented an EMS information system comprising multiple databases that supplement each other and is utilizing this system to integrate EMS with other health services, to conduct EMS research, and to support prevention activities. Central to this system is a custom-designed wide area network that electronically links emergency receiving hospitals, ambulance providers, emergency medical dispatch centers, and the Medical Examiner's office with EMS.

The telecommunications industry continues to invest billions of dollars in mobile satellite (MSAT) systems to provide communications when traditional land line and cellular systems are unavailable. These systems are designed not to compete with but rather to complement cellular services. MSAT includes voice, facsimile, and data for stationary and en route communications. Thus MSAT can be used by emergency medical services providers to supplement radio and cellular communications. Because of the differences in each of the many systems soon to be available, however, the adoption of MSAT requires careful analysis so that the optimum system for each individual provider can be selected.

The telecommunications industry continues to invest billions of dollars in mobile satellite (MSAT) systems Combining geographic information systems with several existing data sources, the San Diego County Division of Emergency Medical Services has developed a fully integrated injury surveillance geographic information system (ISGIS). Use of the system allows for a proactive approach to public health surveillance and helps bolster several multidisciplinary community-based programs. The ISGIS system processes fatal and nonfatal motor vehicle-related crash injury data elements, allowing emergency medical services providers to create subregional maps specific to any data element or series of elements. This system provides for more in-depth knowledge of the cause-and-effect relationship of different types of traffic crashes.

The article describes a study that examined restraint use among children younger than 15 years who were seen by paramedics or emergency medical technicians for injuries sustained in motor vehicle-related crashes. Children who were restrained by child seats or lap and shoulder restraints were significantly less likely to experience severe injuries in motor vehicle collisions. A higher percentage of older children restrained in lap belts only complained of head and neck trauma compared with those who were unrestrained or restrained by a lap and shoulder restraint combination.

Not every ambulance run results in patient transport. Patients may refuse care, or emergency medical services may refuse to transport certain patients. The article reviews the process of refusal, outcome data for patients who are not transported, and methods to reduce care refusal and its medical and legal complications.

An emergency medical services (EMS) system comprises access (911), communication, emergency medical dispatch centers, prehospital personnel, transport agencies, and receiving facilities. These individual components, combined under one operational set of guidelines or protocols, form the whole system. EMS systems in major metropolitan areas are complex. The intricacy of the system is related to how many levels of expertise and what types of agencies exist within that system. Emergency medical care is complex and has many components. Quality management of a prehospital care system needs to encompass all the components of the system.

Ethical decision making has recently been applied to the field of emergency medical services (EMS). The unique aspects of time, resource, and personnel constraints apply limitations to the traditional methods of ethical decision making. The article reviews the decision-making process and applies it to practical issues in system design and patient-provider encounters. The ethical aspects of EMS community education, dispatch, co-responding prehospital providers, receiving hospitals, and system management also are reviewed. Situation-specific examples are cited to clarify the application of these principles in daily practice. Awareness of ethical principles enhances the medical decision-making process in EMS.

The article describes a study examining the relationship of support for organ donation and knowledge of the organ procurement process in predicting willingness to donate organs for transplantation among college students. The study sample consisted of 150 students enrolled in allied health programs. Pearson product-moment correlations were computed using willingness to donate as the dependent variable. Support for organ donation and knowledge and awareness of the organ procurement and transplantation process served as the criterion variables. The significance level was set at 0.05. Based on the data analysis, there was no significant relationship between support for organ donation and willingness to donate organs. Knowledge and awareness, however, showed strong correlation with willingness to donate organs. Because of the sample size and the nonrandom nature of the sample, the results of this study cannot be generalized to the entire student population. Further research is needed to expand the scope of this study. Efforts should be directed toward developing better measures to assess the constructs examined in this study.

The article describes a case of ovarian hyperstimulation syndrome (OHSS) as an unusual cause of pleural effusions in a young woman. A review of the clinical manifestations of OHSS and management issues is presented. Severe OHSS can be life threatening. Emergency health care providers must maintain a high index of suspicion for this condition because the treatment of infertility is becoming more common.
