Physicians treating patients in the primary care setting (office, hospital, or emergency department) are key players in quickly identifying those at risk for developing illness from immunocompromise. Even though the current prognosis for HIV infection is poor, the earlier such patients can be identified, the better quality of life we can provide. Emergency physicians are at especially high risk for exposure to the virus during rapid response necessary for life-saving activities. Physicians are cautioned to observe the appropriate precautions (universal) to prevent their becoming victims of HIV illness.

This article presents an initial approach to the possibly HIV-infected patient. Emphasizing early testing in order to prevent complications and spread of disease, the physician is encouraged to actively interact with the local AIDS coordinator and other health care providers to assist HIV-infected patients in managing the physical and emotional consequences of their disease. Additionally, the focused initial physical examination is reviewed. This focused examination can serve as a guideline for monitoring disease progression during routine office visits. Finally, included are flowsheets used to manage two frequent clinical situations in the care of the HIV-infected patient: fever and an acutely altered mental status.

Most HIV-positive patients with AIDS are at great risk of developing a variety of infectious and noninfectious pulmonary complications. Many of these are rapidly lethal if untreated and the most important, such asPneumocystis carinli, tuberculosis, and fungal infections are discussed in considerable detail. As the treatment results of these early complications improve, AIDS patients now survive long enough to develop further pulmonary complications as well as secondary cardiac and cardiovascular problems. The incidence, diagnosis, prevention, and treatment of all aspects of this complex illness are then broadly reviewed. Complications of current therapy are also discussed.

The unfortunate HIV-infected patient can expect that the virus will eventually destroy all ability to fight off opportunistic infection. Additionally, that the HIV can cause direct damage to the CNS is suspected, but probable. Common treatments of HIV-infected patients include medications with CNS and PNS side effects as well as metabolic disturbances that can result in altered mental status and/or seizures. The broad array of potential direct HIV effects, opportunistic infection, and malignancy-related central and peripheral manifestations, and drug actions and side effects are succinctly reviewed.

The acronym, AIDS, conjures up feelings like any other terminal disease. Because the disease came to light among homosexual men and is also associated with IV drug abuse and prostitution, there is a temptation to categorize everyone with the disease as “bad” or at least “foolish.” To avoid dying from AIDS, one must avoid getting it. Prevention seems straight forward, but quickly gets muddled by society's diverse values, which support ideas about the rights and responsibilities of those with and without the disease. Our contention is that medical personnel should not judge AIDS victims, but rather do what they can to help.

Gastrointestinal tract involvement is common in patients with HIV infection. This article discusses some of the major manifestations and diagnostic approaches to patients with AIDS. Clinicians should be particularly astute to diagnostic effects of the wasting syndrome as early as possible. Other AIDS-related manifestations to consider are anemia, leukopenia, endocrine, and musculoskeletal presentations.

Pediatric AIDS patients present a diagnostic dilemma due to the subtle and insidious manifestation as well as higher CD4 counts; therefore prophylaxis guidelines in children are based on the characteristics of the infection in infancy of the age and specific changes in the CD4 cell count. Institution parameters for starting dideoxynucleosides include clinical manifestations (eg, an AIDS-defining infection, failure to thrive, progressive encephalopathy from HIV, AIDS-associated malignancy, two or more episodes of meningitis) and laboratory indications (eg, CD4 counts, thrombocytopenia, hypogammaglobulinemia).

Over the past few years, increasing emphasis has been placed on the use of antimicrobial agents to prevent opportunistic infections in patients (HIV). This article reviews acute and prophylactic agents, including their side effects, as well as the immunization of AIDS patients. Medications discussed include all classes that are used including antiretroviral therapy. The article includes an easy to use chart of all approved AIDS medication as prepared by the Pharmaceutical Manufacturers Association, including drug name, manufacturer, indications and approval status of the product.

AIDS is a chronic debilitating illness culminating in death. Long-term care planning should address the medical, nursing, social, and psychologic needs of the patient, family, and significant others. Identification of social stigmata resulting from the diagnosis of AIDS becomes an integral part of any treatment plan. This article addresses the necessary elements to consider when determining the long-term care needs of an AIDS patient.
