摘要:
The article presents an overview of evaluating and treating the infected joint replacement, focusing on the hip and knee. Current principles employed to minimize infection are discussed. An emphasis is placed on gathering information from several sources—history, examination, wound appearance, plain radiographs, hematologic studies, tissue cultures, and nuclear medicine imaging—to arrive at the most accurate diagnosis. The importance of aggressive surgical debridement to reach definitive cure is reviewed. Finally, the need for a multidisciplinary approach to minimize this devastating complication is emphasized.

Compartment syndrome is a surgical emergency. Delays in fasciotomy lead to muscle ischemia, neurologic injury, and potentially crush syndrome and death. The signs of compartment syndrome are pain out of proportion to injury and, on physical examination, tense swelling of the compartment, weakness of the involved muscles, pain with passive stretching of the involved muscles, and numbness in the distribution of the involved nerves.

The patient presenting to the emergency department with signs and symptoms suggestive of pulmonary embolism often represents a diagnostic challenge. Many conditions may mimic the signs and symptoms of acute pulmonary embolism. A combination of studies to evaluate both the pulmonary vascular bed (ventilation perfusion scan and pulmonary angiogram) and the deep venous system of the lower extremities (impedance plethysmography, Doppler ultrasound, and venography) can be used to evaluate a patient with suspected pulmonary embolism. Once the diagnosis has been confirmed, secondary preventive therapy (unfractionated heparin and inferior vena cava interruption) or primary therapy (thrombolytic agents and embolectomy) can be instituted. Newer management strategies for the diagnosis and treatment for pulmonary embolism are on the horizon.

Surgery of the lower extremity in patients older than 65 years is often complicated by high morbidity and mortality. The article discusses the most common postsurgical complications in this patient population: death, deep venous thrombosis and pulmonary embolism, infection, delerium, and pressure ulceration. Proper planning in the emergency setting can minimize or prevent many of these complications.

Primary hip dislocations usually occur after the flexed femur sustains a significant amount of force, usually after a motor vehicle accident, and can be associated with concomitant fractures. Prosthetic hip dislocations, on the other hand, are common and occur with the routine motions associated with activities of daily living after total hip arthroplasty. Periprosthetic fractures associated with total joint arthroplasties of the hip and knee are becoming more prevalent. With more than 300,000 arthroplasties being performed each year in an aging population, the incidence will continue to increase. The article reviews the evaluation and management of these orthopedic challenges.
