SummaryThe enigma of fixed drug eruption has fascinated dermatologists for over eighty years. The clinical features and investigative studies suggest that many immunopathogenic mechanisms may be involved. Fixed drug eruption offers a unique opportunity to study the early events of inflammation and the factors controlling localizations of skin disease.

SummaryLangerhans cells and melanocyte distribution was determined in 18 different body sites in 76 cadavers, 44 men and 32 women, within 24 hours of death. The Langerhans cell and melanocyte counts were inversely related so that in sun‐exposed (tanned) epidermis Langerhans cell numbers were lower and melanocyte numbers were higher than in non‐exposed (non‐tanned) epidermis. Langerhans cell and melanocyte densities were similar in men and women on non‐exposed (non‐tanned) epidermis when matched by age. In any one subject Langerhans cell densities were even throughout the body except on sun‐exposed (tanned) areas, where Langerhans cell counts were not only significantly lower but the cells were also unevenly distributed. SummaryThis paper examines the theoretical and practical considerations which may limit sunscreen effectiveness and lead to false expectations on the part of sunscreen users. Present evidence for a preventative effect against carcinogenesis and long term damage is inadequate and needs more attention. Determination of more meaningful protection factors requires better replication of field conditions and better definition of the best parameter, in the acute stage, which would correlate with long term damage. Careful patient instruction with regard to time, site and frequency of application is required. The importance of protection from suberythemal damage, reason for application and maximizing physical protection and avoidance, should be stressed.

SummaryIt is widely recognized that topical corticosteroids may be absorbed systemically, resulting in suppression of plasma cortisol levels. However, their effect on the true competence of the adrenal axis has not been fully established.Eight adult inpatients with dermatoses of variable type and severity were treated with betamethasone valerate 0.1% ointment without occlusion. In all cases there was some degree of suppression of serum cortisol levels. In five patients adrenal axis stress tests were performed. The Synacthen test was normal in all patients and only one had a clearly abnormal response to insulin‐induced hypoglycaemia, which had returned to normal one month after cessation of treatment.This suggests that despite suppression of serum cortisol levels, adrenal axis competence was either maintained or, if diminished, returned promptly to normal after treatment cease

SummaryFifty‐seven subjects with clinically proven cutaneous candidiasis were entered into a double blind, parallel, randomized comparison of sulconazole nitrate 1 % cream applied once daily and clotrimazole 1% cream applied twice daily. The duration of active treatment was three weeks with a four‐week follow‐up without treatment for those clinically cured at Week 3 to determine relapse rate.Sixteen patients failed to complete the study, twelve for reasons unrelated to study medication and four with suspected adverse reactions to sulconazole nitrate.There were no statistically significant differences between the two treatments when considering efficacy parameters (which included KOH microscopic preparation, fungal culture and clinical evidence of fungal infection).There were seven sulconazole nitrate and three clotrimazole treatment failures (ascertained by positive KOH microscopic preparation) at Week 3, this difference not being statistically significant. SummaryTwenty‐seven patients with various dermatophyte infections took part in a double blind study. Thirteen patients were given ketoconazole 200 mg daily and 14 griseofulvin 500 mg daily for up to 60 days. Following ketoconazole therapy, 12 of 13 patients were healed, with minimal lesions in one. In the griseofulvin‐treated group ten of 14 were healed, three having minimal residual lesions and one was a treatment failure. There was a tendency for T. rubrum infections to respond more completely to ketoconazole treatment. At six‐month follow up there was one griseofulvin‐treated failure and one relapse while there were three ketoconazole relapses.It is concluded that both ketoconazole and griseofulvin are effective in treating dermatophyte infections and ketoconazole is a safe alternative to griseofulvin if intolerance or allergy to griseofulvin
