SUMMARYDysfunction in a wide variety of cellular immune mechanisms occurs in atopic dermatitis and contributes to the clinical presentation of the condition.The increased incidence of cutaneous viral and fungal infections, observed clinically in atopic dermatitis, suggests altered T‐lymphocyte function and elevated serum IgE levels suggest deregulation of B‐lymphocyte function. The clinical features of pruritus and erythema raise the possibility of increased release of inflammatory mediators into the circulation. In vitro studies have confirmed abnormalities in T‐lymphocyte subset numbers and function as well as increased releasability of histamine from blood basophils in atopic dermatitis.Recent studies have revealed abnormalities in the metabolism of cyclic AMP, an important mediator in cellular regulatory mechanisms, in atopic dermatitis. Leucocyte cyclic AMP responses to a variety of agonists are blunted in atopic dermatitis and are due, at least in part, to increased cyclic AMP phosphodiesterase activity. It is unclear at present whether this enzyme abnormality is genetically determined or whether it is induced by increased circulating levels of inflammatory mediators. A number of the in vitro abnormalities in atopic dermatitis appear to correlate with the increase in phosphodiesterase activity.Further research aimed at finding ways to inhibit phosphodiesterase activity in atopic dermatitis and consequently reversing other abnormalities in cellular function, will hopefully open up new avenues of therapy for this common and disabling cond

SUMMARYThe parents of children with atopic dermatitis often require considerable encouragement and support to help them cope with this distressing and potentially disabling condition. Over a period of two months a time limited support group was established for parents of children with eczema at the Royal Alexandra Hospital for Children in response to a perceived need for parental support, self help and the alleviation of feelings of isolation and not coping.

SUMMARYThe history is presented of a patient who developed an acral keratotic rash diagnosed as paraneoplastic acrokeratosis, associated with cervical metastases from a carcinoma of the tongue. The skin changes settled with surgical removal of the tumour and metastases. During treatment localized bullous pemphigoid also developed.The relationship of these problems is discussed.

SUMMARYPsoriasis among full‐blood Australian Aborigines appears to be rare or absent. After examination of some three thousand of these people in central, northern, and southern Australia, I have not seen psoriasis. Other medical and nursing observers have also not seen psoriasis in these people. Two references in the literature to psoriasis in Abotigines are reviewed briegly. The author has seen only one, male, part‐Aborigine (more Causcasian than Aborigine), with in a female Aborigine whose father was said to be “not tribal”. A third was recently reported to me.Other conditions common among Aborigines, which should not be confused with psoriasis, are outlined, Certain aspects of the apparent absence of psoriasis in full‐blood Australian Aborigines are discussed.Previous research about the essential causes of psoriasis has not been particularly rewarding. An important question must, therefore, be asked. Should some research projects in future be directed to reasons why some human groups seem not to get psoriasis rather than to why

SUMMARYSporothrix schenckii is a fungus which occurs in soil and on plant material. It has not often been reported in Australia from clinical sources. Most reports have been from Queensland, and it has rarely been reported from the Sydney and Newcastle Metropolitan areas. Twelve isolations from the Sydney area are reported.

SUMMARYTwo cases of human ringworm caused by the dermatophyte Trichophyton equinum var. autotrophicum are reported from victoria. This fungus has been recorded as a cause of ringworm in horses in Australia but infections in man are rare and this is the first report giving details of human infections in Australia. In one of these cases the fungus was also isolated from the patient's horse. Treatment with econazole nitrate was successful in this case.
