摘要:
The delivery of health care to people with a learning disability living in the community has come under increasing scrutiny. Concerns have been raised over both the current quality of this care and its future organisation. In this article we review the evidence on current practice focusing on epidemiology, health status and policy directives affecting primary health care. Four specific areas for service developments are identified: an individual focus within an epidemiological framework, the evaluation of the effectiveness of current therapies, practice organisation and the awareness of health within social care. It is proposed that an effective model of primary health care is possible, supported by research, training, inter‐service collaboration and purchasing guideline

The paper explores the perspectives of pupils involved in an integration link. Data from the mainstream pupils demonstrate a generally positive acceptance of the scheme, and in some cases the development of friendly relationships with the special school pupils. A significant number would wish to see more functional integration in lessons. The special school group also show positive feelings about their involvement. Profiles of individual pupils, however, reveal significant variations in the ways they respond to the opportunity for interaction with their mainstream peers. If integration schemes are to promote successful interactive experiences for all the pupils involved, then aspects of individual diversity need to be taken into account. Specifically, variation in motivation for interaction and in the perception of shared interests and experiences warrant consideration. In order to respond to such diversity, the planning of integration schemes should be informed by the views of the pupils themselves.

This paper considers the training of staff who care for people with learning disability in the community. Details are given of findings from a survey of staff in community units in the West Midlands which suggest that although most staff receive some training, this is largely a mixture of short in‐service courses and there is a lack of consistency across the various agencies. Despite the considerable changes taking place within services for people with learning disability, which require new knowledge and skills, we are still far removed from having an explicit, relevant and agreed strategy for training ‘front‐line carers’. 摘要:
Personal experience suggests that more and more centres are using some form of massage as a therapy approach, but there are few published accounts. In this article, we report a case study with Ms K, a lady who had long displayed severely challenging and disturbed behaviour. A simple form of massage was used as a medium of communicating positive regard with the aim of developing a therapeutic relationship. We believe that Ms K enjoyed this and became more sociable, which led to a change in her lifestyle. More should be done to enable clients with severely problematic behaviour to experience caring relationships, and we believe that a structured approach to using touch therapeutically by appropriately trained professionals is indicated.

A questionnaire was sent to teachers and speech and language therapists who work together with children who have communication problems and severe learning disabilities. In the study the effect of the exchange of information on the collaborative working practices of these professionals and the subsequent implications for professional development is considered.
