The effects of a 15-week quantitated training program were evaluated in nine men, 32 to 59 years old. All had been blind for 10 years or more but were otherwise in good health. They were sedentary with a stable activity pattern. Training sessions were held three times per week and consisted of four 3-minute exercise periods on a bicycle ergometer, each followed by a rest period of equal duration. Heart rates at the end of the fourth exercise period averaged 27 beats below individual maximal heart rates.Maximal oxygen uptake increased from 24.0 to 28.5 ml/kg × min or by 19%. Total heart volume and mean serum cholesterol decreased significantly, and psychological tests showed improvement.Five subjects continued exercising at the same intensity but only once weekly for another 14-week period. Mean maximal oxygen uptake decreased to 6% above the control level. Four subjects who discontinued training after 15 weeks were retested at the same time and had a mean value 5% below control maximal oxygen uptake.

In an epidemiologic study of the total population of Tecumseh, Michigan, 98 deaths from coronary heart disease were observed between 1959 and 1965. Forty-five of the fatalities occurred within 1 hour of the onset of symptoms and were classified as sudden. The proportion of sudden deaths among men was nearly twice that among women, and the incidence increased progressively with age in both sexes.Hypertensive heart disease, coronary heart disease, or diabetes mellitus had been detected on prior examination in 62% of those who died suddenly. Physiologic abnormalities associated with a high risk of coronary heart disease were also found more frequently than in the total Tecumseh population.All but seven of the persons who died suddenly had abnormalities including arrhythmias and conduction defects which were detected in the standard 12-lead electrocardiogram.Although sudden deaths often seem to occur without warning, the victims are predisposed by conditions which are detectable long before the catastrophic event. The identification of conditions which are precursors of sudden death from coronary heart disease permits a rational consideration of possible preventive measures.

The family, reported on, have a syndrome of prolonged Q-T interval, potentially fatal episodes of ventricular arrhythmias, and normal hearing. The direct familial transmission demonstrated in this pedigree is consistent with an autosomal dominant mode of inheritance. To protect other members of the family from the consequences of ventricular arrhythmia experienced by the irreversibly brain-damaged index patient, a series of physiologic investigations was undertaken on this patient in a search for a suitable prophylactic medical regimen. The attacks of ventricular arrhythmia were found to be precipitated by increase in systemic pressure, sinus tachycardia, and extrasystoles occurring in the supernormal phase of repolarization (R in T syndrome). Propranolol was demonstrated to be an effective agent in preventing the ventricular arrhythmia in the index case and in other family members. Since administration of the beta-adrenergic stimulator isoproterenol did not produce ventricular fibrillation, it is assumed that the action of propranolol in preventing ventricular arrhythmia is not mediated through beta-adrenergic blockade, but through its direct antiarrhythmic properties. The subject experienced no ill effect from the series of investigations.

Dermatoglyphics form in utero during early gestation and may be influenced by genetic or environmental factors operating at that time. Since cardiac embryogenesis also occurs during early gestation, an analysis of dermatoglyphics in congenital heart disease (CHD) might reveal some types which are associated with aberrant dermatoglyphics.Ten dermatoglyphic traits were analyzed in 225 individuals with CHD proved, in most instances, by extensive radiologic studies and at surgery. These traits were compared with dermatoglyphics in 200 normal control subjects. The analysis revealed that the atd angle was significantly increased in CHD. Except for endocardial fibroelastosis and atrioventricular canal, all subtypes of CHD had a wider atd angle than controls. In ventricular septal defect, patent ductus arteriosus, tetralogy of Fallot, and multiple cardiac defects, the increase was statistically significant. The present study confirms previous reports that the atd angle may be increased in CHD.

Blood samples from the superior vena cava (SVC) are usually considered an acceptable representation of mixed systemic venous return in identifying and quantifying left-to-right atrial shunts. However, if during sampling from the SVC unrecognized entrance of the catheter into the azygos vein takes place, diagnostic errors may result. Blood samples from the azygos vein were collected from 60 children with congenital heart disease and the oxygen saturations ranged from 20% to 92%. These saturations were compared to those of samples taken from the superior vena cava above the entrance of the azygos vein. No substantial difference (saturation within ±4%) was found in 24 patients, while 28 patients had a significantly higher saturation in the azygos vein (from +5% to +21%) and eight patients had a lower saturation (from −5% to −16%). A significant left-to-right shunt may be overlooked or erroneously thought to be present when sampling of blood takes place unknowingly from the azygos vein. Examples are presented.Approaching from the groin it was possible to catheterize the azygos vein in two thirds of the cases when a deliberate attempt was made to do so.
