In December 1993 Perinatal Pro-ACT®was implemented at Robert Wood Johnson University Hospital. Pro-ACT®, an acronym for Professionally Advanced Care Team, is a system of health care delivery that includes two components: case management and a restructured practice environment. The perinatal units represent the final service that operationalized the Pro-ACT®model and completed Robert Wood Johnson University Hospital's 5-year goal for implementation on all inpatient units. This model facilitates the institutional commitment to provide cost-effective, highquality care with enhanced patient and staff satisfaction.

The challenge of health care reform is driving the development of new and innovative solutions to the question of how to provide quality, cost-effective care that is satisfying to patients. Case management offers an opportunity to facilitate a childbearing experience for the patient and family that meets these goals. This article describes the development and implementation of an obstetric case management pilot project in a large academic medical center and the lessons learned in the first year of operation. The design of the pilot program is presented. Patient education and care coordination were the case management activities most valued by patients and physicians alike. Initial strategic planning for the second year of operation is also presented.

摘要:
Occupying any new facility takes many years of planning from the initial vision through the actual provision of patient care services. Hospitals present complex logistical and safety problems. The length of time planning depends on the complexities of the facility, the specifics of care provided, and changes necessitated by design of the new facility. In planning a women's hospital, basic principles for planning a change were implemented: identify the need, develop a vision, coordinate a strategic plan, enlist key players, develop the program, implement, and evaluate. The successful occupancy of the Mary Birch Hospital for Women, a 169-bed freestanding hospital, was accomplished through teamwork and a well-developed and orchestrated occupancy plan.

摘要:
The purpose of this study was to determine whether parents perceived their premature infant's transfer among hospitals as stressful. Forty-four parents were interviewed after their infant's transfer from an intensive care nursery (ICN) to a community hospital nursery (CHN). Parents reported stress related to a lack of communication regarding transfer as well as to differences in care practices between nurseries. These differences related mainly to staffing patterns and feeding practices. Implications for nursing practice in both ICNs and CHNs are offered and relate to nursing interventions designed to minimize the stress that these parents associated with transfer.

The article examines the major components of health care reform, the impact of the managed competition model on health care delivery, and the implications for the delivery of perinatal/neonatal care and education of perinatal/neonatal health care providers. The impact of managed competition on the perinatal health care community and resulting changes in the perinatal health care delivery system are also explored. The potential for case management models to increase the consistency, efficiency, and timeliness of neonatal nursing care outcomes is examined. Neonatal nurses must define outcomes expected from nursing management and relate these outcomes to knowledge development, credibility, reimbursement, power, and authority. The need for methods meaningfully to describe and measure neonatal nursing productivity has never been more critical. Changes are also needed in educational systems.

Fluid and electrolyte management for very-low-birth-weight infants is especially critical and challenging during the first week of life because of body fluid composition, physiologic changes that occur after birth, skin and renal immaturity, and environmental stressors encountered in the neonatal intensive care unit. Measures that minimize insensible water loss have a great impact on stablization of fluid and electrolyte needs for these tiny infants.

Despite previous research studies documenting the presence and damaging effects of noise on the premature infant's development, neonatal intensive care units (NICUs) remain noisy. Environmental stimuli may cause sensory overload in premature infants, leading to stress and alterations in development. Noise levels generated from selected caregiving activities (eg, placing formula at bedside table, closing drawers, and discarding plastic equipment in trash cans) surpassed 52 dB, the sound level of light traffic. Caregiving activities continue to contribute to the environmental noise in the NICU. Nurses must be aware of the noise levels generated by specific caretaking activities and attempt to reduce the noise level in the NICU.
