摘要:
The use of over-the-counter medications by pregnant women is common. Over-the-counter medications are widely assumed to be harmless because they may be purchased without prescription. Some consumers do not consider them to be drugs. Women may not realize that over-the-counter medications have the potential to harm them or their fetuses. This article reviews selected over-the-counter medications commonly used by pregnant women, including pharmacology, risks, Food and Drug Administration risk classifications, and strategies for counseling clients.

摘要:
An increasing number of women receive epidurals during labor and birth. Although the incidence of adverse reactions remains very low, the potential for life-threatening complications still exists. The Association of Women's Health, Obstetric and Neonatal Nurses (AWHONN) states that the safety of women undergoing procedures, such as the administration of epidural analgesia/anesthesia, depends significantly on the competence, attentiveness, and experience of those responsible for their care. The purpose of this article is to review the significant complications related to obstetric epidural. Recommendations are presented to aid the nurse in preparing for and managing epidural emergencies. Specific responsibilities of nurse managers and educators in competency training, evaluation, and guidance of nurses are also discussed.

摘要:
The challenges of the nursing shortage have and continue to impact all aspects of health care today. Specialty areas and critical care areas have created job opportunities and are now providing education programs for both experienced as well as new graduate nurses. Orientation is a critical time for new graduate nurses of differing ages who come from varied personal, educational, and work backgrounds with varied life and job experiences. For the successful completion of orientation, coaching and mentoring are key components in the retention and recruitment of nurses into the profession.

Background: Little is known about how preterm infants make the transition from breast-feeding and bottle feeding to exclusive breast-feeding in the weeks following hospital discharge. This study examined the breast-feeding patterns of preterm infants born at 30 to 35 weeks' gestation over a 4-week period following hospitalization. Method: Daily feeding diaries were completed by 53 mothers. These diaries were used to describe the proportion of breast milk feeds and feeds directly at breast. Results: Infants received a high proportion of breast milk feeds, with 60% receiving breast milk exclusively for the first week, and 56% receiving breast milk exclusively for the 4-week period. The proportion of feeds at breast increased steadily over the 4 weeks, with 50% primarily breast-feeding in week 4. Infants who received breast milk exclusively in week 1 were significantly more likely to be primarily fed directly at breast in week 4. Conclusions: Adequacy of the milk supply was a key factor in the successful transition from primarily bottle feeding at hospital discharge to primarily breast-feeding at home. The study provides some insight about this complex and poorly understood transition.

Neonatal hypertension, defined as systolic blood pressure greater than 95th percentile for age, size, and gender, is an uncommon but significant problem. Hypertension may develop secondary to thromboembolic complications of central lines, steroid use, or be a sign of an underlying renal or cardiac problem. Pharmacologic management of hypertension in the neonate includes use of beta blockers, angiotensin-converting enzyme inhibitors, calcium channel blockers, and/or diuretics. This article discusses issues in assessment and management of neonatal hypertension and reviews the pharmacologic agents most commonly utilized in the neonatal setting.

Objective: To explore Muslim women's views of neonatal end-of-life-care in Sweden. Methods: Interviews using a standardized questionnaire with open-ended questions about care before birth, directly after birth, and during and after the death of the infant. Content analysis was performed on the data. Participants: Eleven immigrant women of Muslim background living in Sweden. Results: The categories identified were information both useful and threatening, priority of medical facts, maternal feelings, roles of significant others, predetermined lifetime, protection of the dying infant, staff's role, memories aggravate the grief, special tradition, life after death and belief in the future. Conclusions: The women provided suggestions for improvement of care including being given sufficient information of and the need for culturally sensitive care. In providing care for Muslim women it is imperative that care take into account the woman's religion and ethnicity as well as individual preferences.
