Computer-aided design (CAD) and related technologies are having a dramatic impact upon manufacturing industry. The advantages of these technologies over traditional methods include improved accuracy and quality, reduced product development time and ease of product modification and hence customization. The resultant increases in productivity, whilst maintaining flexibility of design, have obvious potential in the rehabilitation equipment industry. Recent applications of these technologies to the analysis of prostheses and wheelchairs and to the design and production of sockets for amputees, prosthetic implants and custom seating are reviewed. Computer-Aided Rehabilitation Engineering (CARE) is in its infancy. However, given the rapidity of developments in this field, CARE has the potential to radically alter rehabilitation engineering over the next decade.

Radiofrequency electromagnetic fields near short-wave diathermy equipment operating at a frequency of 27.12 MHz can expose the physical therapist to levels above those recommended in standards for radiofrequency exposure in Western countries. Electric and magnetic fields around air-gap, diplode, monode and circuplode applicators were mapped by the author.Large differences in stray field intensities were found for the various applicators. The air-gap electrodes caused the highest levels of unwanted radiation, and the circuplode caused the lowest levels. The use of the circuplode would normally ensure an operator exposure far below the levels in recommended standards. In order to reduce the exposure during the first few minutes of a treatment, when air-gap electrodes or diplode are used, the operator should stand at the end of the diathermy console opposite to the applicator and cables and not, as is often the case, at one side. It is recommended that manufacturers change the design of the diathermy console (a minor modification) in order to ensure this operating position.

Intradiscal hypoxia has been suspected of playing a role in the pathogenesis of degenerative back diseases. Investigating this hypothesis requires quantitative methods for measuring Po, in the intervertebral disc.Tissue oxygen tensions are usually measured with a membrane-covered oxygen-consuming sensor. Calibrating this sensor is complicated by the existence of an oxygen-diffusion gradient in joint of the membrane. This article describes a method based on mass spectrometry to measure the oxygen tension in the intervertebral disc and an‘in situ’calibration technique, which is able to compensate for errors caused by the oxygen-diffusion gradient. The principle of the calibration technique is to measure the signal of an inert reference gas, whose partial pressure in the intervertebral disc is known, and to utilize the measured signal to estimate the magnitude of the oxygen-diffusion gradient.The oxygen-diffusion gradient is shown to cause a mean error of -53.8% in the measured oxygen partial pressure; this error falls to +07% after correction by the‘in situ’calibration technique described.

An engineering approach to functional assessment has been utilized to assist the moderate-to-severely physically handicapped. These computer-aided assessments are directed at work, keyboard operation and driving. In order to improve the quality of life for this population, independent living systems, recreational and mobility aids, and automated retrieval systems for information on assistive devices have been developed. This paper discusses their implementation and applications.

摘要:
For those severely physically disabled persons who are unable to use keyboards, the computer has to be adapted to operate from the limited signals that can be obtained from controlled movements. These movements need to be identified and appropriate devices provided which can produce suitable signals to operate a computer. To minimize programming effort, it is also necessary to standardize on a particular form of input signal—in our case, one or two on/off contacts -for which the programs are developed. It is desirable for user input devices to be linked to the computer through an Interface Control Unit, which can also be operated from a remote infra-red unit mounted on a wheelchair. Such units are described.

摘要:
During the last few years a number of systems for graphics generation and image processing have been developed by the authors. Depending on the application, one can consider many kinds of systems; for example for simple analysis of cellular images, a system that realizes the video mixing between a camera and graphics data from a microcomputer would be adequate. For a more elaborate analysis, such as angiographic and echographic images requiring a high level of interactivity, a semi-automatic acquisition system linked to a host computer would be necessary. Limits in precision and execution time of these semi-automatic systems leads to independent work-stations that realize digital acquisition, processing, and display of images.

As part of the continuing evaluation programme of medical equipment sponsored by the UK Health Departments, the evaluation of patient monitors is being carried out within the Northern Regional Health Authority.'Health Equipment Information', Number 142 published in June 1985, contains evaluation reports on a further four monitors. These are the Cardiac Recorders Medifax, the Gould IM 1000, the Siemens Sirecust 404, and the Spacelabs Alpha 9. For fill details of the evaluation findings readers should consult‘HEI’142. The following is extracted from the report.Monitors reviewed in‘HEI’142 are designed to monitor blood pressure and body temperature, as well as ECG.
