摘要:
Readers may have noticed that Dr Stuart Meldrum is no longer co-editor of this journal. Having resigned his post at St Bartholomew's Hospital and joined Reynolds Medical Ltd., he felt that he may not be seen to be impartial and decided to resign. Personally, I regret that very much. We worked together on JMET (and in other spheres) very amicably for many years. We shared a common purpose and rarely had serious disagreements on editorial policy, on whether to accept contributions for publication or anything else. The whole editorial team will miss his wide experience and expertise.

摘要:
Although many aids for the disabled appear to be quite simple, their design is subject to many difficulties. These problems arise because of the intimate relationship between the equipment and the human body. Equipment design is therefore affected by many biological variables that are difficult to define at the beginning of the design exercise and many will not even be obvious until a device is tried out with a user. Standard design methodologies rely on all the variables being defined in a thorough specification at the start of the design process and therefore often lead to ineffective devices or very long development programmes. An alternative methodology is presented which overcomes the problems by separating the user interface aspects of the design from the supporting features. The user interface aspects are then allowed to evolve in conjunction with tests with potential users and finally integrated with the supporting features once a satisfactory solution has been found. Much more effective devices result with much shorter development effort.

摘要:
We have developed and evaluated a prototype external pulse train stimulator for use with a new technique of circulatory assistance. This involves the use of skeletal muscle (latissimus dorsi) wrapped around the thoracic aorta as an entirely autologous extra-aortic counterpulsation system. The stimulator has been successful in initiating both muscle contraction at the desired part of the cardiac cycle (diastole) and the necessary fibre-type transformation required for long-term work (fatigue resistance). The background to the development of the stimulator and the early haemodynamic results are described in this paper. The evaluation of the prototype design and the areas for future development are discussed.

Incorporation of a ligand into polyvinylchloride allows the production of ion selective electrodes that are soft-bodied and disposable. With features make them especially suitable for clinical, particularly gastroenterological, investigations. We report here on the construction of a combined reference and pH electrode suitable for use at jejunal biopsy. With this type of pH electrode, the pH of the mucosal surface of the jejunum in UK and in Indian subjects, without evidence of upper gastrointestinal disease, was about pH 6.0. This was almost identical to previous values measured using a separate reference electrode. Both the polymer electrode and a suitable data logger can be conveniently produced in the laboratory and compare favourably with commercially available systems, provided the range of pH likely to be encountered is within the operating range of the incorporated ligand.

摘要:
The management of a patient with an enteric fistula producing a large volume of effluent is lime-consuming and complicated. In patients requiring defunctioning procedures the eventual aim is restoration of intestinal continuity. Prior to surgery however it is prudent to ensure that the bowel distal to the stoma will cope with the sudden return to its normal role without diarrhoea or colic. This paper describes a simple, cheap, portable device which can be easily made. The device is closed, self-regulating and can be adjusted to patients’requirements.
