摘要:
An optical fibre-based goniometer is described. This instrument was designed to measure the angular position of patients’limbs within the core of a magnetic resonance body scanner, via a 40 m remote fibre-optic link. The sensor exploits the advantages of optical fibre-based sensing, which include immunity to electromagnetic interference, intrinsic safety and chemical immunity. The detection electronics and signal processing are based on the principles of chromatic modulation, an inexpensive, intensity-independent technique in which a change in the spectral power distribution is measured over a broad bandwidth, by photodetectors with differing spectral responses. The optical fibre goniometer has an angular range of 90°, with an average resolution of 2’. The long-term accuracy is within±1°, the specified accuracy for the physiological application.

Results are presented to demonstrate and establish a methodology of comparison of pedobarograph (PBG) images. Dynamic PBG images were processed so that contours of equal pressure could be extracted. After normalization for size, position and orientation, a representative contour was selected from each image and compared with the representative contours from other images. The comparisons yielded dissimilarity coefficients. The dissimilarity coefficients obtained from intercomparisons of the contours from the 57 PBG images of unshod and 35 PBG images of shod footsteps used in this study were subjected to agglomerative cluster analysis. The results, involving many hundreds of intercomparisons, group together the images made by an individual subject and clearly separate the three subjects studied.

Non-invasive ultrasonic techniques for measuring the mechanical behaviour of large arteries have a potential clinical application for physiological studies of the circulation and early detection of degenerative arterial disorders. A newly developed system for such purposes, comprising two double-echo trackers with zero-crossing phase-locked circuits and interfacing a B-mode realtime scanner, has been introduced for on-line recording of the diameter in a selected aortic segment. The aim of this report is to draw attention to the limitations of the technique in order to avoid misinterpretation of results. The various problems associated with the use of phase-locked echo followers for tracking sonic echoes of vessel pulse waves are summarized. The high spatial resolution of the measuring system is essential for estimating the elastic properties of the vessel, because the fractional changes of the diameter waveform during a heart cycle are small compared with the swings of intravascular pressure. Measuring errors may originate from either human or technical sources. From several viewpoints correct alignment of the ultrasonic beam vis-à-vis the vessel segment under consideration is crucial for obtaining valid measures. A thorough knowledge of the physics involved is essential for an adequate use of the instrument. With correct use, easily reproducible and reliable estimations are obtained of the mechanical properties of large vessel walls.

The assessment of the functional performance of non-invasive blood pressure (NIBP) monitors using a commercially available NIBP analyser (Dynatech CuffLink) was evaluated. The Cuff Link includes facilities for performing a static pressure calibration, and safety cut-out pressure, leak rate and rapid deflate time measurements. Cuff pressure oscillations simulating a range of blood pressures at a range of heart rates enable dynamic tests of NIBP monitors to be performed. By reducing the magnitude of the cuff pressure oscillations the sensitivity of a monitor to weak pulsations can be determined. The CuffLink does not provide facilities for assessing the ability of the monitor to reject movement artifact. The CuffLink tests were performed on 10 different models of NIBP monitor. The results demonstrate the variable nature of systolic, diastolic and mean blood pressure readings from NIBP monitors (even under stable laboratory conditions), reinforcing the importance of not relying on individual readings, but rather to consider the average of a few successive readings. Normative data for two models of NIBP monitor are provided. Applications of the CuffLink are discussed with reference to a protocol for NIBP monitor performance checks.
