Training physicians in geriatrics is essential to improving health care for the elderly. The study reported here provides a detailed analysis of geriatric education at the undergraduate, graduate, and fellowship levels. The number of programs for medical students and residents has increased but is still relatively few; most are elective, and they are of variable quality. Both the number of fellowship programs (36) and positions (87) and the current number of yearly graduates (40) are quite small. Few of these programs have been thoroughly evaluated. Data are presented on the type of training sites, educational activities, and clinical interaction at different levels of training. Overall, the nursing home is most frequently used as a training site, and trainees have contact with many care providers. Information of this kind may be of value to those planning or conducting geriatric programs. The results suggest that there is a persistent need for high quality programs at all levels of educational continuum and that such efforts should be evaluated to ensure and improve quality.

The SEARCH program (Colorado's area health education program) is designed to alleviate the maldistribution in health manpower by recruiting new professionals to underserved areas through student/resident rotations and to retain those professionals already there by providing accessible continuing education. Data obtained on SEARCH during the first two years of operation are encouraging. Nearly 75 percent of rural physicians practicing in self-assessed areas of need wanted contact with medical students, and the number of University of Colorado medical graduates taking residencies in Colorado increased dramatically. The retention rate of physicians in Colorado was 97.2 percent during this two-year period for those physicians serving as medical student preceptors, whereas it would have been expected to be 83.5 percent based on 1979 data.

It has been widely noted that medical school can be stressful experience for many students and that on completion of their medical education students will enter a profession high in potential stressors. However, very few systematic efforts to teach medical students practical stress management skills have been undertaken. In the study reported here, a group of students volunteered to participate in a six-session program that taught them personal stress management techniques, including self-relaxation training, schedule-planning, priority-setting, leisure time-planning, and cognitive modification techniques. From pretraining to posttraining, the students showed improvement on a variety of measures that included knowledge about stress, self-report inventory scores assessing stress symptoms and life-style, personal ratings of stressful situations, and their daily activity schedules. A control group largely failed to show improvement relative to the stress management training group largely failed to show improvement relative to the stress management training group subjects. The importance of making available to students such specific, behavioral and preventive stress management training is discussed.

Sixty-five senior medical students chose symptom information that would allow them to assess which of two diagnoses was more appropriate for hypothetical patients. Although Bayes' theorem should have governed their data selection, 83 percent of the subjects did not choose the symptom information required for Bayesian computation. Instead, they showed an overwhelming tendency to seek data relevant to a single disease, while ignoring information related to an equally plausible alternative diagnosis. The tendency for subjects to select diagnostically irrelevant information in such tasks has been labeled “pseudodiagnosticity.” The effect result from the difficulty of simultaneously evaluating the relevance of a single symptoms in relation to single diagnosis. Medical educators might incorporate classroom demonstrations of the pseudodiagnosticity effect in order to increase students' accuracy in differential diagnosis.

Physician behaviors that correlate with patient satisfaction |

The behavior of 15 internal medicine residents, each with 10 patients, was observed through a one-way mirror. Ratings by the patients of satisfaction with their physicians were also obtained. Patient satisfaction correlated strongly with ratings for physician courtesy and information-giving, Nonverbal behaviors such as eye contact, bodily positioning, and physical contact did not correlate with patient satisfaction. The correlations between physician behavior and patient satisfaction did not hold for the four women physicians studied.The behavior of 15 internal medicine residents at the University of New Mexico Hospital, each with 10 patients, was observed through a 1-way mirror and the 150 patients rated their satisfaction with their physician. The mean ratings of patient satisfaction were between 6 and 7 on a 7-point scale. Satisfaction ratings did not vary significantly between male or female patients or between Hispanics and Anglos, although patients under 40 years of age tended to provide lower ratings than those over this age. Patient satisfaction correlated highly with physician courtesy (formal greeting and formal closure of the interview) and with information giving. Listening behavior also correlated with patient satisfaction. Physician empathy correlated only weakly with patient satisfaction, while physical attention (eye contact and body positioning) did not correlate at all with this factor. Additional factors that were not significant determinants of patient satisfaction in this study included the physical appearance of the physician, the total time of physician-patient interaction, and the amount of physical contact between patient and physician. There were no significant differences between male and female physicians in the mean ratings of caring behaviors or satisfaction. The satisfaction scores of male patients with female physicians were the same as patients of either sex with male physicians; however, female patients expressed greater satisfaction with their female physicians than did male patients with female physicians or female patients with male physicians. This study confirms the findings of earlier investigators that patient satisfaction is beneficially influenced by friendliness and information provision on the part of the physician.

The evaluation of a behavior and development training program that was integrated into a general pediatric training residency is reported. Thirty-two residents who participated in the program were evaluated over one year on measures designed to assess residents' opinions about the relevance of behavior and development concepts, residents' perceptions of their own competence in behavior and development knowledge and skills, supervisors' ratings of residents' general clinical performance, and residents' attitudes about 10 specific illnesses. Results suggest that residents perceive a high relevance for psychosocial concepts and an increasing competence with these concepts after training. Establishing rapport with patients emerged as an important variable in supervisors' ratings of resident performance and in residents' self-ratings of competence. Attitudes were initially negative toward the more behavioral and psychophysiological disease entities but showed some positive change over the year. These results and their implications are discussed.
